Inhibition of serum androgen levels by chronic intranasal and subcutaneous administration of a potent luteinizing hormone-releasing hormone (LH-RH) agonist in adult men.

The effect of chronic treatment with the luteinizing hormone-releasing hormone (LH-RH) agonist Buserelin (Hoechst AG, Frankfurt/Main, West Germany) ([D-Ser(TBU)6,des-Gly-NH2(10)]LH-RH ethylamide) administrered by nasal spray (200 or 500 micrograms, twice daily) or subcutaneously (50 micrograms daily) for periods of 1 to 8 months was studied on serum sex steroids and LH levels in 18 patients with cancer of the prostate. Basal serum testosterone concentration decreases to 71.1 +/- 18.3 (NS) and 28.6 +/- 9.3%, (P less than 0.01) of control in patients receiving the 200-micrograms and 500-micrograms dose by nasal spray, respectively. In patients treated subcutaneously, a more rapid inhibition of serum testosterone levels to 19.6 +/- 6.4% of control (P less than 0.01) is observed. The finding of decreased levels of 17-OH-progesterone, testosterone, and dihydrotestosterone in the presence of unchanged pregnenolone concentration indicates that the decrease in androgen biosynthesis induced by Buserelin treatment is due to a blockage at the level of 17-hydroxylase and 17,20-desmolase activities. The present data indicate that chronic administration of Buserelin could be a safe and effective means of reducing serum androgens in patients with cancer of the prostate.

[Analysis of silicone testicular implants after explantation].

Two juvenile size silicone testicular prostheses were analyzed following replacement by adult size devices after 72 and 99 months in situ. Scanning electron microscopy and electron microprobe studies were carried out on the explanted silicone prostheses and their attached tissue capsules. The material at the surface of the prostheses gave no evidence of biodeterioration. However, localized sites of calcification were observed. The adjacent tissue capsule consisted of two distinct concentric layers with different morphologies; the one adjacent to the prosthesis consisted of dense arrays of collagen fibres oriented parallel to the silicone surface and extending outward to a thickness of 100-120 microns. The outer layer was less dense, more disordered and was uneven in thickness. It contained numerous discrete particles of silicon-rich material. Technical and clinical implications of these results are discussed.

[Practical method for the preoperative spatial localisation of small intracranial neoplasms from scanner data (author's transl)].

When only transverse axial slices of a CT examination already performed are available, the localisation in space of small intracranial space occupying lesions is easy only if a lateral view is available at the same time. In terms of three dimensional localisation, it is possible to rely on the coordinates of transverse axial sections only if the examination has been carried out under stereotaxic conditions. In all other cases, uncertainty concerning the site of the plane of reference of the examination would be too great. The method suggested may be used to identify the site of the plane of reference of a computed tomography examination on a lateral skull X-ray with a margin of error of +/- 2 degrees. It uses the lateral skull X-ray of the patient examined by computed tomography, the transverse axial slices of the examination and a pre-established template. Once the plane of reference is identified, the lesion may be rapidly reconstructed using two other templates. The method has the advantage of being applicable to examinations already carried out and thus does not prolong the duration of investigations.

[Spontaneous hypoglycemia and chronic kidney insufficiency].

Although glucose intolerance occurs as a consequence of chronic renal failure, improvement of a diabetic state by deterioration of renal function is a well known phenomenon. Recently occasional cases of spontaneous hypoglycemia in patients with chronic renal failure have been reported; two such cases and the results of metabolic studies are described in this paper. Pituitary, thyroid and adrenal function appeared to be normal. The results of an oral glucose tolerance test were normal; an appropriate insulin response was demonstrated in one patient, and a slightly elevated basal insulin value with a delayed insulin response to oral administration of glucose was demonstrated in the other. An insulin tolerance test did not support the hypothesis of increased insulin sensitivity as a factor, and the growth hormone response to hypoglycemia was normal. An intravenous glucagon test caused a subnormal increase in plasma glucose concentration, and the intravenous administration of tolbutamide produced hypoglycemia without an increase insulin sensitivity as a factor, and the growth hormone response to hypoglycemia was normal. An intravenous glucagon test caused a subnormal increase in plasma glucose concnetration, and the intravenous administration of tolbutamide produced hypoglycemia without an increase in insulin values. The plasma alanine concentration was low and the proinsulin/insulin ratio was increased. The origin of this hypoglycemia is not clear but is probably multifactorial. However, low hepatic glycogen stores and inadequate gluconeogenesis due to substrate deficiency seem to be involved.

[Voluntuary stramonium poisoning. 2 clinical cases].

Two cases of acute voluptuary poisoning by infusions of dry leaves of stramonium are reported. Symptomatology was characterized by confusion and hallucination (visual hallucinations, loss of space-time relationships, etc.) associated with neurovegetal symptomatology ascribable to the active principles contained in the preparation used: mydriasis, disturbances in visual accomodation, dryness of the fauces. Tachycardia was observed in one case, bradycardia in the other. Symptoms were resolved in one case by administration of neuroleptics, and spontaneously in the other after about 12 hours. A brief description of the preparation employed is given, and risk of poisoning stressed.

[Comparative trials on sultopride and fluanisone].

A therapeutic trial has been carried out on 50 patients, most of them being hospitalized, in order to compare the characteristics of a new benzamide, Sultopride, to those of a butyrophenone, Fluanisone, for the treatment of important and chronic agitation states. Clinically, they were essentially psychotic states, with dissociation. From this trial it appeared that sultopride is superior to fluanizone with regard to their sedative effects and that it has specific characteristics: a very powerful antipsychotic action, an increasing efficiency over time, as well as an improvement of incoercible agitation in children. Thus, the present trial has checked out the well-known sedative action of Fluanisone, and has revealed a new major antipsychotic drug, Sultopride, the superiority of which lies on a much wider therapeutic field.

[Chronology of diagnostic and therapeutic activities in the 3 weeks following rupture of an intracranial saccular aneurysm (series of 316 cases)].

The authors advocate to operate grades I, II and III patients on the 6th or 7th day following the onset of SAH, that is to say between the "oedema" period and the rebleeding period. They study all investigations and therapy which are necessary previously to the operation and advocate to perform a CT scan as the first investigation as well as to control the intraventricular pressure. Their opinion is based on the study of 316 cases admitted over a period of 5 years 218 have been operated upon by a team of 7 neurosurgeons with an overall mortality of 18.3 p. 100. The mortality in grade I and II patients has been 13.3 p. 100. This percentage includes all fatalities occuring in the six months following surgery. The senior author assumes that a good audio-visual teaching of the trainees can improve the results.
