Mechanical small bowel obstruction due to an inflamed appendix wrapping around the last loop of ileum.

Acute apendicitis rarely presents with a clinical picture of mechanical small-bowel obstruction. The Authors report a case of this inusual clinical occurrence, arised like a complication of a common disease, characterized by a chronically inflamed appendix (mucocele) wrapping around the last loop of ileum that produced volvolus and strangulation. The few similar cases reported in the literature are moreover reviewed.

Gastrointestinal bleeding in emergency setting: two cases of intestinal angiodysplasia and unusual conservative surgical treatment.

The authors report two cases of gastrointestinal bleeding in an emergency setting caused by angiodysplasia of colon and small bowel. They stress the rarity of the lesion and consider the difficulties involved in obtaining a preoperative diagnosis. The optimal management is uncertain and depends on the severity and rate of bleeding. A conservative medical approach is indicated for many patients, while surgery constitutes definitive treatment in case of massive hemorrhage or recurrent bleeding. In the cases reported the diagnosis was performed only via intra-operative enteroscopy. An unusual conservative surgical treatment was performed based on ligation of the vascular elements of the angiodysplasia. This method makes it possible to avoid an intestinal resection and yelds good results.

[Evoked oto-acoustic emissions in patients with retinitis pigmentosa].

Evoked otoacoustic emissions (EOE) offer a unique opportunity to measure objectively the function of outer hair cells in human beings. In this paper the Authors report a study carried out employing EOE evaluation on cochlear function in subjects with retinitis pigmentosa. Nine patients with retinitis pigmentosa with normal hearing in both ears upon traditional audiometric evaluation were studied. The frequency spectrum of the EOE was taken into consideration, evaluated in frequency bands of 50 Hz, in order to ascertain the presence or absence of the emissions and, when present, their amplitude. These data were compared statistically, using the Student's T Test, to those obtained in a homogeneous control group of normal subjects. In subjects with retinitis pigmentosa, average EOE intensity values were statistically lower than those found in normal subjects. In fact, the Student's T Test evaluation revealed significant differences in EOE intensity in 64 of the 127 frequency bands examined. Moreover, the distribution of the EOE in patients with retinitis pigmentosa proved to be more discontinuous than that observed in the normal subjects. These data appear to indicate authentic alterations in the cochlear mechanics in patients with retinitis pigmentosa. Such alterations may well be related to outer hair cell lesions. It seems likely that the auditory system is involved in retinitis pigmentosa degenerative processes more frequently than traditional audiometric tests have been able to show.

[Functional evaluation of the perineum in pregnancy. Hypertonia of the levator ani muscles].

In this study, the role of "levator ani muscles hypertonia" among pathogenetic factors of perineal obstetric injuries is analysed. For this purpose, clinical data of 142 pregnant women, who underwent functional evaluation of perineum during pregnancy and 30 days after delivery were examined. Data here reported are not sufficient to confirm the importance of this factor within the pathogenesis of postpartum perineal deficiency. Levator ani hypertonia on one hand seems to be able to obstruct fetal progression, on the other hand it presumably constitutes, as an expression of a good voluntary control of perineal muscles, a favourable prognostic factor.

[Functional evaluation of the perineum in pregnancy. Inversion of control].

This study concerns clinical data of 142 women, who were subjected to functional evaluation of perineum during the last part of pregnancy. The 16 cases characterized by synergic activation, during the examination of perineal muscles ("perineal testing"), of antagonist muscles ("inverted control"), were examined taking into account data regarding evolution of labour and perineal outcome. "Inverted control" seems to influence, in this study, length of the second stage and frequency of operative deliveries whereas we can't confirm the importance of this disorder within pathogenesis of perineal obstetric complications.

[Changes in the monosynaptic reflex during wakefulness and sleep of children with cerebral paralysis].

Changes of the H reflex during sleep were studied in 13 children with cerebral palsy (8 with spastic tetraplegia, 2 with a mixed form of cerebral palsy without spasticity, 3 with hypotonic diplegia or tetraplegia). These modifications were compared with those of 5 normal children of the same age. During repeated night recordings, responses in the calf muscle elicited by electrical stimuli to the posterior tibial nerve were studied at the same time as the EEG, EOG and EMG of the mental muscles. The results show that:--in normal children the max H reflex progressively decreases in amplitude from wakefulness to REM sleep; -- in spastic patients there is only a slight decrease in the H reflex in NREM sleep and no significant change in REM sleep; the amplitude of the H reflex is always greater than that in the control group; -- in the dystonic and hypotonic group the results obtained are similar to those of the control group. From these results one may draw the conclusion that in spastic patients as opposed to the control, dystonic and hypotonic groups, normal balance between the function of supraspinal systems regulating the amplitude of the spinal reflexes is alterated probably through the scarce functionality of the supraspinal inhibitory structures.

Our experience with direct intraperitoneal insemination (DIPI) as a treatment for infertile couples.

The therapeutic value of Direct Intraperitoneal Insemination (DIPI) is of particular interest in cases of unexplained infertility and failed AID. The same technique is less likely to be successful where functional or morphological semen abnormalities or multifactorial infertility exist. The Authors report their own experience with DIPI as a treatment on 76 couples with a long-standing infertility.

Use of GnRH depot analogue in the treatment of uterine fibroids.

Twelve patients with symptomatic uterine fibroid were treated with Goserelin acetate, a depot Gn-RH analogue administered s.c. in a dosage of 3.6 mg every 28 days for a 6 month period. Mean age of the patients was 39 (26-47). A menopausal-like state, evidenced by hot flushes, depression, vaginal dryness, hysteroscopic endometrial atrophy, and hormonal values (FSH, LH, E2), was seen in all patients. After only two months of treatment, a significant reduction was noted in the size of the fibroid, being even greater than that in the following four months. Only one patient in the series underwent surgery. General tolerability of the drug was optimal. These data suggest the efficiency of this type of treatment in obtaining a significant reduction of fibroid volume as well as subjective symptomatology. The cost-benefit ratio is optimal, allowing postponement of surgery and the possibility of vaginal, as opposed to abdominal, hysterectomy and affronting problems of anaesthesiology and post-operative recovery for patients "at risk".

[Static anomalies of the pelvis caused by labor. Maternal weight increase during pregnancy as a risk factor].

In this retrospective study, the role of Gravidic Maternal Ponderal Increase (GMPI) within pathogenetic factors of perineal obstetric injuries, is analysed. TO this purpose, clinical data of 58 puerperae, who underwent a functional evaluation of perineum 30 days after delivery, were examined. The results of this study seem to indicate an autonomous pathogenetic role of GMPI, besides the indirect relation deriving from the association with other risk factors (age, parity, diabetes, fetal megalosomia, ecc).

[Mallory-Weiss syndrome. Personal experience and review of the literature].

The Mallory-Weiss syndrome is a more common cause of upper gastrointestinal bleeding than that previously recognised. Personal experience is reported of twelve cases of Mallory-Weiss tear, constituting 6.4% of all cases of upper gastrointestinal tract hemorrhage observed in the three year period from 1984 to 1986. History of alcoholism was present in 50% and hiatus hernia was associated in 41% of all the cases. Ten patients (84%) were managed nonoperatively, two patients underwent operation. None of the twelve patients in this series died. The literature of the last few years, is reviewed with particular reference to progress in the treatment of the syndrome.

[EEG and biochemical considerations on 40 cirrhotic patients surgically treated by portacaval anastomosis or mesenterico-caval bypass].

Starting from biochemical hypotheses on hepatic encephalopathy, which presuppose at the basis of this disease first of all some modifications of the plasmatic values of octopamine, ammonia, serotonin and of particular aromatic aminoacids, the Authors compared the levels of ammoniemia and of octopaminemia with eegraphic data, in two groups of cirrhotic patients, operated of porto-cava anastomosis (16 patients) ad of mesenterico-cava by-pass (24 patients). All this in order to value if there are significant correlations between these values and the cerebral damage and also if there are differences between the two types of operation. Attained data show a good correlation between neuropsychiatric symptomatology and importance of the cerebral damage noticeable at the E.E.G. As to the possible correlations existing between biochemical data and E.E.G., while as to plasmatic octopamine no significant modifications were found, as to ammoniemia, highest values correspond to findings of greater cerebral compromission. Moreover, it seems that between the two types of operations, the mesenterico-cava by-pass is the one that allows a better clinical and eegraphic evolution.
