Medial meniscus replacement by a tendon autograft. Experiments in sheep.

In 20 skeletally mature female merino sheep, divided into four groups, we performed total medial meniscectomy, removal of the middle third of the patellar tendon, and tenotomy of the calcaneal tendon of the right hind leg. Group I (control) had no additional procedures. In the other three groups the medial meniscus was replaced by the middle third of the patellar tendon from the ipsilateral knee. The animals were killed at three (group II), six (group III), or 12 months (group IV) and the tendon-meniscus examined macroscopically, by light and scanning electron microscopy, and biomechanically. Remodelling of the tissue had taken place by 12 months but the failure stress and tensile modulus for the tendon-meniscus were lower than for the normal meniscus. Our evidence suggests that, in sheep, replacement of a meniscus by a tendon autograft may decrease the severity of the degenerative changes that occur after meniscectomy.

The wedge punctoplasty for treatment of punctal stenosis.

We describe a new instrument, the Reiss punctal punch, which is specifically designed to performa "wedge" punctoplasty. The Reiss punctal punch was used alone to treat three eyelids of three patients, and in conjunction with horizontal tightening of the eyelid to treat 35 eyelids of 18 patients with symptomatic epiphora related to punctal ectropion with secondary punctal stenosis. After a mean follow up of 12 months, 36/38 (95%) of the eyelids demonstrated a patent punctum, and symptomatic relief was achieved in 35/38 (92%) of the eyes.

Introducing the Reiss punctal punch.

We describe a new instrument specifically designed to perform a "wedge" punctoplasty. The Reiss punctal punch was designed to be an enhancement of the one-snip punctoplasty by providing greater uniformity and improved tear access to the punctum via an excised posterior tissue wedge as well as inhibiting reapproximation of the cut edges leading to recurrent punctal stenosis.

Investigation of intracellular organelles in the human organ of Corti using backscattered electrons (BSE).

Using ligand-mediated osmification (OTOTO technique), human cochleae were prepared for scanning electron microscopy. The surface of the organ of Corti and the bodies of the sensory cells were observed with secondary and backscattered electrons. Detecting the backscattered electrons emitted from the depth of the cells, intracellular organelles such as subsurface cisternae, mitochondria and nuclei became visible in their reciprocal arrangement.

Scanning electron microscopy in a case of infantile inborn deafness.

The temporal bones of a one-year-old, congenitally deaf boy were removed 3 1/2 days post mortem, fixed in formaldehyde and prepared for scanning electron microscopy (SEM). In the apical coil the organ of Corti had the normal configuration. In the second coil there was a continuous decrease of both the number of sensory cells and their conservation. Altered sensory cells with giant or bullous distended hairs were found. Throughout the basal coil the hairs of the sensory cells were absent, while the other cells of the organ of Corti appeared normal.

[Submicroscopic changes in the jejunum in ectopic sites. A scanning and transmission electron microscopy study following free, microvascular jejunum transfer].

Biopsies of the jejunal epithelium were analysed by scanning and transmission electron microscopy of specimens taken from patients at least 1 year after reconstruction of defects from resection of oro-hypopharyngeal cancer by free microvascular anastomized jejunum graft. The villi were compressed and flattened, their surface uneven due to calibre changes of columnar cells. The microvilli were rarefied and grossly altered. In the subapical area there was a distinct enhancement of desmosomal connections. Paneth cells and endocrine cells were unaltered. Autonomic nerves were intact, though the muscularis mucosa seemed to be denervated.

Muscular venous blood metabolites during rhythmic forearm exercise while breathing air or normoxic helium and argon gas mixtures.

The effects of breathing normoxic helium or argon gas mixture on the local muscular metabolism during exercise were compared to those of room air in four healthy subjects. For this purpose, PO2, PCO2, pH, the concentrations of lactate (LA), glucose (Gl) haemoglobin (Hb) and K+ and osmolarity were repeatedly measured in efferent muscular venous blood during 12 min of rhythmic forearm exercise and 16 min of recovery. The time courses and magnitude of the changes in PCO2, pH, [Gl], [Hb], [K+] and osmolarity during exercise and recovery were similar for breathing both the helium and argon gas mixtures. The main finding was that during exercise, the [LA] curve reached a peak value significantly higher by 25% under normoxic helium than under room air or normoxic argon breathing. This rise in [LA] was accompanied by a slight reduction in muscular venous PO2, too small to signify muscular hypoxia. The possibility that this decline in PO2 might be due to a shift in muscular metabolism towards lipid oxidation was confirmed by the lower muscular respiratory quotient observed during helium breathing. However, such a shift did not explain why [LA] rose during this breathing. The probable explanation is that helium facilitates LA diffusion out of the myocytes.

[Scanning electron microscopy of intracellular structures in the hair cells of the guinea pig cochlea using backscatter electrons].

Guinea pig cochleae were prepared for scanning electron microscopy (SEM). The lateral surface of the outer hair cells was studied first with secondary electrons. It was possible to find a network of tubular cisterns and long mitochondria stained by osmium immediately under the plasma membrane detected by backscattered electrons. The lateral arrangement of the mitochondria and the tubular cisterns was confirmed by observing the embedded SEM tissues by transmission electron microscopy.

[Psychologic adjustment process following high cervical spine paralysis and its support by a bedside reading apparatus].

The process of psychological adjustment, which has to go on after high levelled lesions of the cervical spinal cord, passes through several phases. It needs a lot of empathy, plenty of time for confidential talks and psychological knowledge as well as an individualizing cooperation between the several levels and stages of combined treatment during rehabilitation. We could give this process a considerable advance by a (half-) automatic bedside-projection of belletristic literature, which can be continuously read on a screen from long film strips by means of a modified Pentacon system. The introduction of this new object in life proved a good support for the threatened personality of those badly handicapped patients.

Comparative effects of the ionophore A23187 on the mechanical responses of muscle in normal Pietrain pigs and pigs with malignant hyperthermia.

Since increased intracellular Ca2+ is believed to be the main factor causing skeletal muscle contracture in human and porcine malignant hyperthermia, the potential effects of the ionophore A23187, which enhances intracytoplasmic Ca2+, were investigated in Pietrain pig muscles. These effects were compared with those of caffeine, known to induce dose-dependent contracture in vitro in isolated muscle from human subjects with malignant hyperthermia. For this purpose, the mechanical and biochemical actions of caffeine and A23187 were tested in intercostal muscle biopsies from 10 normal pigs and 10 with malignant hyperthermia. The results show that A23187 allowed very clear differentiation between the muscles of normal and pathological animals. In view of the wide spectrum of drug sensitivity characterizing subjects with malignant hyperthermia, it is suggested that exposure to A23187 be added to the halothane and caffeine tests currently used to detect this disease.

[On the origin, course and influx-vessels of the V. basalis and the V. cerebri interna (author's transl)].

Origin, course and influx-vessels of the basal vein are investigated on 100 brains. An anterior formation of the basal vein (textbook) was found in 41%, a posterior formation in 34%. The different possibilities of drainage are examined procentually at the different types. Course and number of the different variations of the influx-vessels are taken into account: Vv. thalamostriata inferiores, gyri olfactorii, ventricularis inferior, peduncularis, cerebri interna, thalamostriata superioris, (terminalis), septi pellucidi anterior, septi pellucidi posterior, atrii medialis, atrii lateralis, nuclei caudati.
