Therapeutic surgery in failures of medical treatment for fungal keratitis.

Medical treatment failure necessitated surgery in nine cases of fungal keratitis. Therapeutic surgery eliminated fungal infection in seven cases, and useful vision was retained in five out of six penetrating keratoplasties. In three cases Natamycin (Pimaricin) therapy rendered fungi non-viable, but two were demonstrable by histopathology. These results suggest that antifungal treatment should be applied for as long as possible before therapeutic surgery in order to improve the final visual outcome.

Dematiaceous fungal keratitis. Clinical isolates and management.

Clinical and laboratory features of 16 cases of keratitis that were caused by dematiaceous pigmented fungi are reported. Management, including the treatment of nine cases with Natamycin (Pimaricin), resulted in corneal healing in 14 cases, and therapeutic surgery in two cases.

A herpesvirus-type agent associated with skin lesions of green sea turtles in aquaculture.

Nine successive groups of green sea turtles (Chelonia mydas) were observed in aquaculture during the posthatchling period. During the first 6 months of growth, each group underwent an epizootic of skin lesions, named gray-patch disease. Two types of skin lesions are associated with gray-patch disease: papules and, more characteristically, spreading gray patches which appear 7 to 8 weeks after hatching. In both types of lesions, intranuclear inclusions are found in keratinocytes in the malpighian layer of the epidermis. Electron microscopic examination of scrapings from lesions and biopsies revealed many viral particles, with features characteristic of the herpesvirus group. Transmission of gray-patch disease is possible with bacteria-free preparations of viral particles.

Activity of tetracycline, doxycycline, and minocycline against methicillin-susceptible and -resistant staphylococci.

Tetracycline, doxycycline, and minocycline were evaluated for their antibacterial activity against methicillin-susceptible and -resistant isolates of Staphylococcus. At clinically achievable levels both doxycycline and minocycline were more active than tetracycline against methicillin-susceptible organisms. Tetracycline and doxycycline had no activity against methicillin-resistant staphylococci, whereas minocycline at 2 mug/ml inhibited six of 13 strains and, at 3 mug/ml, 10 of 13 strains.

Pitted keratolysis.

Pitted keratolysis is a superficial infection of the skin confined to the stratum corneum. The causative organism has not been identified. The clinical syndrome is distinct and is characterized by pits and superficial erosions in the pressure areas of the soles and in the lateral surfaces of the toes. The histopathology of the lesions is presented and world literature is reviewed. It is concluded that this disease is not limited to the tropical climates, although cases tend to be more severe in these areas.

Griseofulvin-containing medium for simplified diagnosis of dermatophytosis.

By inoculating paired tubes of standard Sabouraud dextrose-chloramphenicol-cycloheximide media, one of which contained in addition 20 micrograms/ml of griseofulvin, 86 of 88 griseofulvin-sensitive dermatophytes were recognized in 226 primary isolation cultures. Most yeast, bacteria, and mold contaminants were not selectively inhibited by the media. The method facilitates the selection of patients for oral griseofulvin therapy by those with relatively little training in mycology.
