The influence of vitamin D intake on serum calcium in tuberculosis.

We report the first prospective controlled study designed to determine the effect of vitamin D ingestion on serum calcium concentration in patients with tuberculosis. Every patient admitted to the tuberculosis ward over a 6 month period, who was free of any condition which might influence serum calcium concentration, was randomly assigned to one of two groups. The diet of the first group was substituted with ergocalciferol 5000 units daily. The diet of the second group was not supplemented. In addition, the second group was randomly subdivided into two subgroups. The first subgroup received a diet unrestricted in vitamin D. The second subgroup received a diet containing less than 50 units of vitamin D. Serum calcium was determined at weekly intervals. In contradistinction to the results of a previously reported retrospective study, there was no significant difference between the group receiving supplemental vitamin D and the control group at any time during the entire period of study. Furthermore, there was no significant difference between the subgroup of patients receiving normal dietary vitamin D and the subgroup maintained on the diet restricted in vitamin D.

[A rare case of sigmoidoduodenal neoplastic fistula (author's transl)].

The clinical case of a patient with sigmoidoduodenal neoplastic fistula is taken as the starting-point for a review of the relevant literature. The great rarity of fistulisation of a sigmoid tumour of the 4th portion of the duodenum is stressed. A clinical definition of the disease is followed by a discussion of the various methods of surgical treatment, with emphasis on the problem presented by reconstruction of the duodenum owing to its anatomosurgical difficulties.

[Preliminary study of ultrastructural changes of the gastric mucosa due to cholecysto-gastrostomy. Experimental research].

After reviewing the literature on anatomo-histological lesions of the gastric mucosa in so-called "biliary reflux gastritis", the Authors undertook an experimental study of ultrastructural alterations of the gastric mucosa after cholecysto-gastrostomy operation conducted in the dog. In this preliminary note they report the alterations observed 60 days after the operation and consider it necessary to continue observations for a longer space of time after the operation.

Beauvericin and fumonisin B1 in preharvest Fusarium moniliforme maize ear rot in Sardinia.

Six selected samples of preharvest maize ear rot, from different localities in Sardinia, Italy, were examined for causal Fusarium species and associated mycotoxins. All samples were almost exclusively found to be affected by Fusarium moniliforme, which was isolated from all infected ear sample kernels (100%). In two samples, in addition to F. moniliforme, F. proliferatum was also present but in a reduced percentage of kernels (up to 42%). All samples were found to be contaminated by fumonisin B1 (up to 250 mg/kg). Four samples were also found to be contaminated by beauvericin (up to 10 mg/kg), with higher concentration in samples also infected by F. proliferatum. When cultured on autoclaved maize kernels for 4 weeks at 25 degrees C, all 13 strains of F. moniliforme examined produced fumonisin B1 (up to 3750 mg/kg), whereas only three strains also produced beauvericin, but in very low amounts (5 mg/kg). In the same assay, four isolates of F. proliferatum also produced high amounts of fumonisin B1 (up to 2500 mg/kg) but this was associated with higher concentrations of beauvericin (up to 175 mg/kg). This is the first indication of the production of beauvericin by F. moniliforme, as well as of its co-occurrence with fumonisin B1 in preharvest F. moniliforme maize ear rot.

[Toxic adenoma of the thyroid gland: personal experience].

The Authors analyzed the thyroid diseases series of the Institute of 1a Clinica Chirurgica of the University of Catania which includes 1022 patients over the last 17 years, reviewing clinical and laboratory tests usually used in a register of diagnosis for autonomous adenomas of the thyroid. The Authors discuss pathogenesis, likely etiology and epidemiology of Plummer's adenoma, taking into consideration the therapy and the complications related to the therapy itself. They conclude stating that the adenoma is a pathology where the surgical operation solves the dismetabolic condition in a lasting way.

[Gilbert's syndrome: consideration of several diagnostic aspects].

Gilbert's syndrome is a frequent clinical condition with indirect hyperbilirubinemia, with controversial pathogenesis, nosography, and diagnostic criteria. Reliability of diagnostic test (caloric intake restriction and phenobarbital induction), as well as bilirubin modifications after physical exercise, were assessed in 7 patients with the disease. Phenobarbital induction appears to be the most suitable and reliable test for clinical non-invasive diagnosis of Gilbert's syndrome.
