Behçet's syndrome complicated by cutaneous leukocytoclastic vasculitis. Response to prednisone and chlorambucil.

Traditionally described as a triad consisting of recurrent aphthous stomatitis, genital ulcerations, and uveitis, Behçet's syndrome is now recognized as a multisystem disease with protean manifestations. We studied a patient with chronic recurrent migratory superficial thrombophlebitis and marked cutaneous hyperreactivity (pathergy) who developed leukocytoclastic vasculitis with recalcitrant leg ulcerations nine years after the onset of his illness. Although he was treated with topical and systemic antibiotics for presumed bacterial superinfection, cutaneous ulcerations continued to develop and enlarge; only after prednisone therapy was begun was there a dramatic response with complete resolution. Despite high-dose prednisone therapy, the pathergy test remained markedly positive; however, no new skin ulcerations appeared.

Streptococcus anginosus-constellatus infections of the nervous system.

Two patients are presented with central nervous system infections due to an uncommon type of viridans streptococci, S anginosus-constellatus. Of the various streptococci, S pneumoniae is the most common etiologic agent in meningitis, but many species, including those which are facultative anaerobes or microaerophiles, have been isolated from cerebrospinal fluid, subdural empyema, and abscesses involving the brain and epidural space.

Gastroenteritis: etiology, pathophysiology and clinical manifestations.

The infectious etiologic agents of gastroenteritis are manifold and have only recently been studied in greater depth. Part of the problem in studies pertaining to diarrhea is the difficulty in identifying fecal "pathogens" among an enormous number of indigenous fecal microorganisms. Finally, the recognition of the pathogenicity of enterotoxins has created an awareness of the complexity of gastrointestinal physiology with host-parasite interactions.

Unusual features of Hemophilus infections in adults.

Infections due to species of Hemophilus, especially H influenzae type B, are usually equated with the pediatric age group. However, during the past several years the medical literature has emphasized the occurrence of such infections in adults. Four adult patients are presented who demonstrate certain unique clinical features which have not been recently or previously reported.

Mycoplasma pneumonia presenting as meningoencephalitis and hemolytic anemia.

Simultaneous occurrence of cold agglutinin hemolytic anemia and central nervous system disease due to Mycoplasma pneumonia is extremely rare. A patient is described who initially presented with severe hemolysis and diffuse neurologic dysfunction which appeared during the period of roentgenographic resolution of her pneumonia. Spectrum, pathogenesis, and antimicrobial treatment of the neurologic complications are reviewed.
