Diuretic actions in man of a selective kappa opioid agonist: U-62,066E.

The effect of a selective kappa opioid agonist, U-62,066E, on urine formation in human volunteers was assessed. Volunteers received single intramuscular injections of either placebo or 2, 3, 4, 5 or 6 micrograms/kg of U-62,066E in a randomized, double-blind study. U-62,066E caused dose-dependent maximal increases in urine volume of 2.6 times control in the first 4 hr after administration. A corresponding decrease in urine osmolality to 20% of base-line values occurred. No changes in total urinary Na, K or Cl excretion were identified. Kappa agonists produce a water-only diuresis at low doses in humans. The mechanism of this effect was not examined in this study but probably relates to alterations in antidiuretic hormone activity.

A guide to organizing joint ventures with physicians.

Catholic health care facilities must consider the business and legal risks, canon law, and other constraints when planning a joint venture with physicians. Participants should first establish goals and compatibility, then determine the venture's type (property, service), form ("true," lease, contract), and structure (corporation, partnership, joint property ownership, trust). The administrator must decide whether the facility will participate directly in the venture or form a separate organization. Participants must determine their relationships with the venture, choosing among many options. The administrator should consider whether a venture raises any canon law issues, especially regarding ecclesiastical and secular assets, approval by the local bishop or Holy See, and need for consultation. Other pertinent legal issues include: Fraud and abuse. The venture should not appear as compensation to induce referrals. Physician referrals. Many states prohibit or restrict referrals by physician participants. Antitrust law. Participants may be liable for actions constituting on antitrust violation. Securities low. Organizers must clarify Securities and Exchange Commission registration exemptions and observe state "blue sky" laws. Tax issues. Catholic health care facilities must consider such factors as tax-exempt status, unrelated business income, taxable subsidiaries, and public charity status. Other considerations include tax ramifications for physicians; tax shelter registration; certificate of need (CON), licensing, and building standards; effects on reimbursement and pension plans; organizational and bond documents; corporate medical practice and fee-splitting questions; and labor and contractual issues.

Treatment of paracoccidioidomycosis with itraconazole in a murine model.

A new triazole, itraconazole, was studied as oral therapy of paracoccidioidomycosis in a murine model. The Paracoccidioides brasiliensis isolate, susceptible to itraconazole in vitro, was given by intranasal challenge, producing acute pulmonary and disseminated disease. Therapy was given twice daily over 4 weeks, and animals observed over 2 months. The infection was lethal for 70-80% of controls (untreated or polyethylene glycol diluent), whereas all treated animals, given 10-200 mg kg-1 day-1, survived. Itraconazole was ineffective in eradicating lung disease in survivors, though effective in treatment of disseminated sites. Since the highest doses did not give a better response than the lower doses, pharmacokinetic studies were performed. These showed irregular curves and small increases in peak serum concentrations and total area under the serum concentration-time curves, which were not proportional to the dose. This non-linearity appears to be best explained by poor absorption. Itraconazole, from these studies, appears to have promise for the therapy of human paracoccidioidomycosis but possibly with a different formulation.

Life satisfaction and activity theory: a multidimensional approach.

Life satisfaction is examined utilizing a multidimensional approach. Hypotheses were proposed and tested for each of the dimensions of LSIA (Life Satisfaction Index A) within an activity theory perspective. The results provide qualified support for the theory. Some dimensions; however, show a better fit than others. Lack of a more general confirmation proved to be due to problems with activity theory and its attendant measures rather than with the multidimensional approach. The patterns of relationships found across the dimensions provide further support for using a multidimensional analysis when dealing with LSIA.

The voluntary associations of the aged.

This paper examines the voluntary associations of the aged. A substantial majority of respondents were affiliated and more than one-half had multiple affiliations. Rate of affiliation remained high even for individuals over 80. Contrary to expectation, women were as likely as men to be affiliated and to have multiple memberships, and were significantly more active in voluntary groups than men. Those belonging to multiple associations were more active in each association than individuals with only one or two affiliations. Overlapping memberships were common. Reasons for active involvement are provided. The bulk of the memberships were of three types which, in function, are integrative at the personality level.

Organizational position and perceptions of problems in a nursing home.

This paper examines perceptions of problems expressed by persons living and working in a 100-bed skilled nursing home. Data were obtained from administrative staff, professional and nonprofessional nursing staffs, therapy staff, auxiliary staff, and residents by means of interview, observation, and analyses of records. The relationship between perceptions of problems and location within the organization and the scope and content of roles played was studied. We sought to ascertain the extent to which members agree or disagree with one another as to what are problems, to locate areas of agreement and disagreement, and to relate the results to the broader issue of the quality of care provided elderly institutionalized persons. Position and associated roles of members were found to be related to perceptions of problems. Differential perceptions of the numbers of problems and their priority were observed. Some problems were mentioned more frequently than others. Perceptions of problems varied both within and between organizational units. A person's location within the nursing home was related to the identification of problems as important and the frequency with which different problems were mentioned.
