Retroperitoneal metastases from carcinoma of the bladder.

We report two patients of metastatic retroperitoneal tumours from carcinoma of the bladder, one presenting with a retroperitoneal mass and the other with retroperitoneal fibrosis. Both cases had been previously treated by radiotherapy for invasive transitional cell carcinoma of the bladder and had no evident of recurrent tumour in the bladder during follow up or at the time of presentation with secondary tumours. Literature reviews revealed that urothelial tumours from the bladder rarely metastasize to the retroperitoneal space.

Transitional cell carcinoma of the renal pelvis and ureter.

In a retrospective study of 185 patients with transitional cell carcinoma of the renal pelvis and ureter, of whom 127 were treated by total nephroureterectomy and 58 by conservative resection, the survival of those with superficial well differentiated tumours was greater than 90% in each group. When urothelium was left behind after conservative resection, there was a 22% rate of recurrence on the same side but this almost only occurred when the original tumour had been multifocal. Post-operative radiotherapy did not improve survival.

Nephroureterectomy after radical cystectomy.

We report 6 cases of upper tract transitional cell carcinoma that developed after total cystectomy with ileal loop diversion. We conclude that if nephroureterectomy is to be used as treatment, excision of the ureteroileal anastomoses should be done.

Diagnosis easily missed--upper urothelial tumour.

Four patients with transitional cell tumour of the renal pelvis and ureter who had atypical presentations are described. The associated presenting problem delayed an earlier diagnosis in two patients and facilitated it in the other two.

Ileal replacement for gunshot injury of the ureter.

A case of gunshot injury of the upper third of the ureter is reported. The injury was managed by ileal replacement with complete conservation of the kidney, after an attempt at primary repair had failed. A survey of the literature has not disclosed any previous report of a gunshot injury of ureter being treated by this method. However, we recommend it only in cases where the conventional procedure of end-to-end repair fails.
