Evaluation of the ultrasonographic volume of breast fibroadenomas in women treated with tamoxifen.

BACKGROUND: Fibroadenomas are the most frequent benign breast neoplasias. Although they are hormone-dependent, no hormonal treatment of proven effectiveness is available for these neoplasias. The objective of the present study was to evaluate the ultrasonographic volume of fibroadenomas in premenopausal women treated with tamoxifen at the doses of 5, 10, 20 mg/day or with placebo for 50 days, starting on the 1st day of the menstrual cycle. METHODS: A prospective and randomized study was conducted on 62 eumenorrheic women aged 15 to 45 years with no hormonal treatment or pregnancy during the last 12 months, with a clinical, cytologic and ultrasonographic diagnosis of fibroadenoma, later followed by a biopsy diagnosis. The patients were divided at random into 4 groups: A (n=15; placebo), B (n= 16; 5 mg/day tamoxifen), C (n=16; 10 mg/day tamoxifen), and D (n=15; 20 mg/day tamoxifen). Fibroadenoma volume was measured by ultrasound at 3 different times: on the 22nd day of the cycle that preceded the beginning of tamoxifen treatment, after 1 month of treatment, and on the day of the biopsy (50th day). The mean volume obtained for groups A, B, C and D was 3, 3.3, 1.9, and 2.3 cm3, respecti-vely. RESULTS: Statistical analysis revealed a significant reduction in nodule size only in group D (p=0.0024). CONCLUSIONS: We conclude that tamoxifen significantly reduced fibroadenoma volume when administered for 50 days at the dose of 20 mg/day. Further clinical studies are needed using the drug for a longer period of time, and in order to exclude the need for unnecessary treatment in some women.

Morphometric analysis of fibroblasts of the mammary lobular stroma during the follicular and luteal phases of the menstrual cycle.

We determined the nuclear volume of fibroblasts of the normal mammary lobular stroma during the follicular and luteal phases of the menstrual cycle. Twenty patients aged 15 to 35 years and eumenorrheic for at least 6 months were randomly assigned to 2 groups, i.e., 10 women in the follicular phase and 10 in the luteal phase. The nuclear volume was 34.4 micron 3 and 98.8 micron 3 for the follicular and luteal phases, respectively, with the difference being statistically significant (p < 0.05). These data suggest a higher metabolic activity in the mammary intralobular stroma during the luteal phase of the menstrual cycle, probably due to a synergistic action of estradiol and progesterone.

Morphological action of tamoxifen in the endometrium of persistent estrous rats.

BACKGROUND: The aim of this study was to evaluate the action of tamoxifen on the endometrium in states of chronic anovulation. METHODS: Thirty-eight rats inducted to persistent estrous (testosterone propionate) confirmed by hormonal colpocytology were divided into a control and an experimental group; the latter received tamoxifen and had fragments of the uterine horns processed for morphological and morphometrical analysis. Data were analysed statistically by the Mann-Whitney and Student's t tests. RESULTS: Our findings revealed minor uterine weight, epithelial thickness; number of endometrial glands and low eosinophil counts in the group that received tamoxifen. These results were statistically significant. We often observed areas of metaplasic stratified squamous epithelium between cylindrical epithelial cells in both groups. CONCLUSIONS: Our results indicate that antiestrogenic effect of tamoxifen was only partial in persistent estrous, since there was no blocking against the squamous metaplasia of the endometrium.

Invasive ductal carcinoma, fibroadenoma and adjacent breast stroma angiogenesis: an immunohistochemical and morphometrical study.

The authors investigate the differences between the angiogenesis that occurs in malignant and benign breast tumours (10 specimens of invasive ductal carcinoma and 10 specimens of fibroadenoma) as well as in the adjacent breast stroma of these women highlighting the microvessels by staining their endothelial cells for factor VIII immunohistochemically. The number of vessels counted in invasive ductal carcinoma was significantly higher than the number of vessels counted in fibroadenoma and in the adjacent breast stroma. There was no difference between the number of vessels counted in the fibroadenoma and its adjacent stroma. Even though the patients with invasive ductal carcinoma showed a adjacent stroma with a higher number of vessels, the difference was not significant when compared with the adjacent stroma obtained from patients with fibroadenoma.

Cell kinetics of the human mammary lobule during the proliferative and secretory phase of the menstrual cycle.

We evaluated the mitotic index and nuclear volume of the epithelium of the human mammary lobule during the menstrual cycle. Thirty patients were selected and divided at random into two groups, i.e., 15 women in the proliferative phase and 15 in the secretory phase. Normal mammary tissue was obtained on the occasion of fibroadenoma enucleation. Mitotic indices (/1000) and nuclear volume (micron 3) were 0.0 and 53.8, respectively, during the proliferative phase, and 11.6 and 130.0, respectively, during the luteal phase, the difference being statistically significant (p < 0.05). These data indicate that the proliferative stimulus of the mammary lobule is more intense during the luteal phase of the normal cycle and may probably be due to a synergistic action between progesterone and estradiol.

[Treatment of human papillomavirus infection of the uterine cervix with gel interferon].

Treatment of uterine cervix infection caused by human papillomavirus still is an enigma. Some drugs have been tested with cure rates between 60 to 70%. Most of such agents are substances which produce a strong epithelial desquamation and have shown strictly toxic side effects. The use of interferon in this infection has been studied, but different authors disagree in their therapeutic findings. A randomized, double blind trial was conducted to compare interferon to placebo. Preliminary results are presented in this article. 47 patients were followed so far. 18 of them were evaluated and the results are shown in this article.

[Pathology associated with uterine leiomyoma: review of 600 cases].

The authors analyzed 600 patients with uterine leiomyoma examined at the Escola Paulista de Medicina and submitted to surgical treatment between 1976 and 1987. The uteri were carefully studied and leiomyomas were confirmed. Other diseases were also found in 71.17% of the cases. Uteri from hysterectomies performed for conditions other than leiomyoma were also studied for control purposes. Major conditions found were chronic cervicitis; adenomyosis; follicular cysts; tubal changes (congestion, edema, and salpingitis); uterine polyps; endometrial hyperplasia; endometriosis; ovarian and endometrial neoplasias.

Peritumorous lymph-vascular invasion, grade of histologic differentiation, and myometrial infiltration as prognostic factors of endometrial carcinoma.

The authors evaluated 50 patients with endometrial carcinoma studying the prognostic parameters: histologic grade, myometrial invasion and lymph-vascular space invasion. The patients were divided into two groups: A--a good prognosis group (33 cases)--with no recurrences and/or metastasis occurring for five years; and B--a bad prognosis group (17 cases)--with recurrence and/or metastasis or death within five years. We concluded that lymph-vascular space invasion was more frequent in group B, and was a reliable parameter for bad prognosis. The lymph-vascular space invasion was always accompanied by myometrial invasion and patients whose myometrium had not been involved did not have lymph-vascular space invasion. In both groups, in well-differentiated tumors, there was no lymph-vascular space involvement. The well-differentiated tumors were statistically more frequent in the good prognosis group. Myometrial invasion was not statistically significant as a prognostic parameter.

Age and loss of bone density in pre-menopausal women.

Some studies have suggested that premenopausal women show a decline in bone mass in function of age while other studies have not. Bone density in the spinal column and proximal femur (neck, Ward's triangle and trochanter) in 75 healthy white women between the ages of 30 and 49 without a history of irregular menstrual periods was correlated with age, weight, height and body mass. No statistically significant decline in bone density with age was seen during the premenopausal period. There was a positive correlation between bone density in the spine and proximal femur and weight and body mass indices. Our results suggest that healthy women do not show loss before menopause and it also seems that obesity has a favorable effect on these women's bone mass.
