Stress assessment by means of questionnaires.

A comprehensive methodology for the measurement of stress comprises objective physiological and biochemical methods as well as application of self-reporting scales. This overview focuses on the subjective procedures, their psycho-physiological theoretical background and practical issues. We discuss the most valuable procedures used to assess stress factors, reactions to stress and relationships between the stressed subject and the physical and psychosocial environment. The strengths and limitations of self-reports and the conditions to obtain reliable data and to interpret the findings are outlined. The results of several investigations performed by our teams concern especially the assessment of occupational stress.

[Transsacral fistula of an ileal pouch-anal anastomosis].

For patients with ulcerative colitis and familial adenomatous polyposis the restorative proctocolectomy with ileo-anal-pouch anastomosis is the surgical treatment of choice. Leakage from the ileo-pouch anastomosis is the surgical treatment of choice. Leakage from the ileo-pouch-anastomosis can be a difficult to manage complication, which in some cases resists all attempts at local repair. A surprising complication of a 28 years old woman patient with an ileo-anal-pouch anastomotic fistula is presented. The fistula developed the 4th day postoperatively. Local irrigation and transanal drainage seemed to have a good result, the patient being examined after two weeks. During an apparently better evolution, after one month, the patient developed a transsacral fistula with local abscess and osteolysis. The ileo-anal-pouch anastomosis was converted to a less comfortable conventional ileostomy, but with good local and general final result.

[Experimental basis for a new surgical-therapeutic method for duodenal ulcer: gastric mucoslerosis].

On the basis of studies carried out by chronic experiments in dogs the authors noted, by clinical, chemical, radiological and histological methods, that the chemical sclerosis of the gastric mucosa performed with a sterile, fresh hypertonic solution of glucose at 60%, injected in the sub-mucosa, represents an intervention which is:--physiological, since the sclero-distrophy is achieved of the acid-secreting glands and "targeted" intra-gastric vago-sympathic denervation, while the storage function of the stomach is maintained;--feasible, since it can be easily performed from the technical view point, without hemorrhagic, perforative intra-operatory risks or hepato-renal toxicity;--fiable, since it was constantly accompanied by good clinical and functional results. The radiological examination of the barium passage in the stomach, antrum, pylorus and duodenum is normal.

[Multivisceral operations for carcinoma of the upper stomach and cardia].

In this study are noted technical problems regarding "en bloc" multiple organ resections and the anatomic and functional reconstruction for carcinoma of the upper stomach and cardia. From 1997 to 2002, a total of 264 patients with cancers of the stomach were operated in the service. 75 patients presented cancers localized at the proximal stomach and cardia (97.33% adenocc.). The rate of resectability was 27.77% (27 pt.). Types of operations in this series were: standard esophagogastrectomy in 7 patients; total gastrectomy with regional lymphadenectomy in 9 patients; 11 patients underwent "en bloc" multiple organ resection, with the removal of the stomach, partial or total esophagectomy and, occasionally, ablation of the spleen, pancreas, left hepatectomy, resection of the diaphragm and an extensive lymphadenectomy. Surgical mortality for the complex multivisceral resections was noted in 3 patients (8.88%). The global 5 years survival in the service is poor: 15.9%.

[Cervical anastomotic fistula in surgery of the esophagus].

Cervical anastomotic fistula are reported in the surgical literature in 10-30% of the patients, providing a much longer hospitalisation, a higher morbidity and in some cases even mortality. Between 1997-2003, 91 patients underwent surgical treatment for esophageal cancers and 14 patients for chemical burns. In the cancer group the rate of resection was 67,03% (61 patients). In 8 patients with non-resection tumours a retrosternal esophageal by-pass with stomach was carried out. Cervical anastomosis were performed in 68 patients, by hand sutures. Anastomotic fistula were noted in 9 patients (13,24%). In 6 cases temporarily fistula occurred, with spontaneous healing by local treatment, in 8-28 days. 2 patients required reoperation and one patient a definitive feeding jejunostomy. Most common causes of fistula are technical problems, ischemic gastric or colonic tube, postoperative respiratory failure, with prolonged hypoxia. An anastomosis in the neck results in less postoperative complications than one of the lower level.

[Computed tomography in establishing the therapeutic management of lung cancer].

Computer tomography (CT), a noninvasive method of high complexity, compelled recognition in pneumological practice, becoming indispensable. The present study deals with 59 patients suspected of bronchopulmonary cancer, who were submitted to computer tomography, besides other examinations. CT contribution in establishing the diagnosis, the therapeutical line (particularly in determining the surgery moment) as well as the correlations (concordance and discordance) between the information offered by CT and by other conventional (radiological, bronchological) methods are analysed. The association of thin needle trans-thoracic biopsy (TNB) to CT ensures an important histologic confirmation which amounts to 75% of investigated cases, according to our statistical data.

[Mediastinal-pulmonary lymphangioleiomyomatosis. The anatomicoclinical aspects].

The lymphangioleiomyomatosis is a rare disease which has a severe course and prognosis and occurs exclusively in females. The symptomatology is not characteristic and the radiological picture is that of a reticular and micronodular diffuse interstitial process. The morphological examination is the only which elucidates the diagnosis. Two cases are reported, the diagnosis of which was established postmortem. In the first case, the site was predominantly pulmonary; in the second case, in addition to the lung, the mediastinal lymph system was also strongly damaged. The morphological changes are characterized by an abnormal proliferation of muscular tissue at the level of the pulmonary interstitium and of the pulmonary and mediastinal lymphatics. The process develops towards destructive lesions of the bronchiolo-alveolar and vascular structures. The pathogenesis is unknown, but genetic and endocrine factors seem to be involved.

[Ergonomic study of neuropsychic impairment in relation to health status in printing occupations].

The exertion and risk factors in relation with the state of health, with the psychophysiological reactivity and with the adaptation and fatigue symptomatology were studied by means of complex psychological, psychophysiological, medical) methods in two professional groups that perform an activity implying important mental and sensorial components: monotasterers and correctors, from three polygraphic enterprises and two publishing houses, totalling 167 subjects (74 monotasterers and 93 correctors). The ergonomic work analysis demonstrated that the exertion and risk factors are conditioned both by the contents of work and by the working environment. The occupational exertion is more complex in monotasterers (mental, sensorial and physical) and predominantly sensorial (visual) in correctors. The work hazards are especially brought about by overstraining of the attention and of the visual analyser, with consequences both on the health of workers and on the quality of production. The investigation of the state of health, of the psychophysiological reactivity, as well as of the adaptation and fatigue symptomatology has detected maladjustment and fatigue phenomena, with impairments of the psychosomatic balance, more marked in monotasterers. The suggestions regarding improvement measures were directed both towards aspects of medico-psychological prophylaxis and elements of corrective ergonomics.

Persistence of viruses in the nasopharynx of apparently healthy children aged 0-5 years. Results of investigations performed in 1982-83.

Virus isolation attempts were made with nasopharyngeal secretions from 400 apparently healthy 0-5-year-old children of a semi-closed community. Repeated virus isolations were achieved in 25 cases subjected to periodic investigations in 1982-83. One-two reisolation(s), 1-5 months apart, of the initially detected viral agents could be obtained in 15 out of the 25 children. The following viruses were reisolated (in decreasing order of frequency): adenoviruses, respiratory syncytial virus, Coxsackie B1 virus, herpes virus type 1, parainfluenza virus type 3. The problems raised by the carriage of respiratory viruses in the nasopharynx of apparently healthy children are discussed.
