[Significance of residual hepatocholedochal dilatation following principal methods of surgical treatment in benign stenoses of the terminal choledochus].

Out of 305 cases, diagnosed and operated in the clinic over a period of 14 years, the authors have investigated in 164 of them--controlled between 2 and 14 years after surgery--the residual dilatation of the hepatic segment of the choledocus following the application of the major methods of surgical treatment of benign stenoses of the terminal choledocus duct. In view of reducing the disturbances that might occur after surgery of the biliary pathways, the material was investigated and, in the light of the data from the literature, the conditions are discussed, in which the residual hepato-choledocal dilatation should be considered as pathological, the methods of investigation, the difference between the duodeno-biliary reflux and the jejuno-biliary reflux, the anatomo-pathophysiological basis of a more complete and differentiated surgical therapy, the precautions necessary for avoiding remaining intra-hepatic abscesses that might complicate the postoperative evolution.

[Problems in the diagnosis and treatment of intrahepatic lithiasis].

Following the study of 44 cases of intra-hepatic lithiasis the authors have reached these conclusions:--intra-hepatic lithiasis (both primary and secondary) represents 3% of the biliary lithiases;--the characteristic element of the evolution of biliary lithiasis is the so-called "ageing phenomenon" leading to the formation of multiple stones, progressively obstructing the biliary pathways and finally involving the intra-hepatic segments. These are the so-called biliary panlithiases representing 50% of the cases with intra-hepatic lithiasis in the author's statistics;--cholangiography (both intravenous and intra-canalicular) should be considered as completed only when it provides a representation of the entire biliary system;--one cannot speak at present of a "remaining" or "forgotten" hepato-choledocus stone before the exclusion of the intra-hepatic "forgotten" stone;--the significant reduction of "post-cholecystectomy disturbances" depends on the correct diagnosis and treatment of all stones, indifferent of their site of occurrence (extra- and intra-hepatic).

Recurrent cranial polyneuropathy.

General considerations on recurrent cranial polyneuropathy are made. In addition, a study carried out in 42 patients with recurrent cranial polyneuropathy (RCP) permitted the assessment of some parameters closely related to age, sex and evolution in order to elaborate an etiopathologic hypothesis. Female cases prevailed (31 females, 11 males); the interval between two episodes varied from 1 month to 39 years. 117 episodes were found (from 2 to 8 episodes in one patient). Immune, vascular and entrapment aspects are discussed.

Outcome in ischaemic stroke: carotid versus vertebro-basilar territory.

Clinical study and laboratory investigations were performed in 166 carotid ischaemic stroke (CIS) patients and 64 vertebro-basilar ischaemic stroke (VBIS) patients. The study revealed some features of VBIS: significant male predominance (p < 0.001), better outcome (for modified Rankin Scale (MRS)) (p < 0.01), significant number of different triggering factors (p < 0.001). Nonsignificant differences in mortality were found in both groups (p > 0.05). Myocardial infarction was an important associated cause of death.

Statistical study of lethal stroke.

Deceased cases with cerebral infarction and hemorrhage constituted the subjects of the present study. They were divided into two groups: 127 cases in group 1 (1954-1959) and 427 cases in group 2 (1977-1982). The purpose of the study was to compare clinical data to evidence any statistical difference between groups. No statistically significant difference was noticed, except the increase of age mean in both diagnoses due to the overall increase of age mean of the population (2.5-10 yrs).

[Chronic reactive inflammations in the etiopathogenesis of malignant hematologic proliferations].

The malignant proliferations are induced by a multitude of etiological factors and possible pathogenic mechanisms. Three new clinical cases and other four previously reported cases of haematological malignant proliferations occurring during the evolution of some chronic reactive inflammatory processes due to various forms of immune deficit are discussed. A review of the literature and of the opinions regarding the pathogenic mechanism responsible for the occurrence and growth of malignant proliferations, especially in the cases with inborn or acquired immunodeficiency, are also included. The authors' pathogenic hypothesis for the cases with various acquired immune deficits and with secondary infectious or allergic reactions is that altered immune responses made possible a lasting antigenic stimulation of certain cell clones of the reactive inflammatory process. By this excessive demand, and instability of the genes has occurred and during their rearrangement a mutation with the depression of an oncogene, responsible for malignant growth, has resulted.

Prognostic criteria in haemorrhagic stroke with good outcome potential.

168 patients with haemorrhagic stroke (H.S.) were clinically and paraclinically studied. In 74 (44%) patients, the outcome was favourable whereas 94 (56%) died. The prognostic criteria were assessed according to: location and size of haematoma, consciousness vegetative disorders, medical history, comorbidity, pupillary and oculomotor disturbances.

Diagnosis difficulties in multiple myeloma.

Some difficulties in the diagnosis of multiple myeloma are presented. The atypical onset (pseudorheumatismal, anaemic, renal or neurological one) as well as the presence of a biological syndrome during some other diseases which may generate the confusion with the multiple myeloma are discussed with reference to the personal casuistry (30 cases of multiple myeloma in a lapse of 12 years). Some peculiar cases of the two categories of difficulties are presented in detail. A careful clinical and biological examination should be performed in every case, as both situations may involve, affect, the prognosis quo ad vitam of the patient.

Lennert's lymphoma.

Four cases (3 M, 1 F) of Lennert's lymphoma were presented. The age (38-76 years), the clinical aspect and the morphopathological features were characteristic. Despite an intensive chemotherapy, 3 of the 4 patients died between 2 and 16 months (mean survival time 7 months). The controversies about the nosological delimitation and the cellular nature of Lennert's lymphoma are presented. Because of the poor prognosis, it should be considered as a high-grade malignant non-Hodgkin lymphoma.

[Indications for pancreatectomy in chronic pancreatitis].

The authors have considered the experience acquired in connection with 15 cases of chronic pancreatitis in whom surgery was performed, and define the parameters which make mandatory surgical interventions in this affection.

[Pseudoadenopathies].

The results of a study concerned with the etiology of some lymph node enlargement in 511 cases aged between 16-80 years, admitted to the III-rd Medical Clinic of Iaşi during the last 7 years are presented. A thorough investigation, by histopathological, cytological, haematological, biochemical, immunological, lymphographic, echographic, radiological examinations, has been done. In 44 cases, the results of the above mentioned investigations indicated that there was no lymph node enlargement, but some other masses sharing the same sites with the lymph nodes, which generated the misdiagnosis in outpatient practice. It is important to note that in 28 cases (63% of the 44 cases) the pseudoadenopathies were localized in the cervical area. The pseudoadenopathies, which are the clinical expression of some tumoral diseases, could be classified as follows: a.--single peripheral; b.--generalized, peripheral; c.--deep. The symptoms which permit the differential diagnosis between the true and pseudoadenopathies are analysed.

[The experience of the 3rd Medical Clinic with lymph node pathology].

In the last 8 years, 511 patients (267 men and 244 women) were investigated. It was found that 44 cases (8.6%) were false adenopathies (various types of tumoral masses) but placed in the nodes areas (localized, generalized or deep). There were 467 cases of true lymphadenopathies, 58 new cases yearly (2.32% of all admitted patients and 6.9% of those with blood diseases). Out of these 467 cases, 330 (70.6%) were malignant neoplastic diseases: malignant lymphomas--206 cases (62.4% of all malignancies), leukemias--99 cases (30%), carcinomatous metastases--25 cases (7.6%). Nonmalignant lymphadenopathies were found in 137 cases (29.4%): specific infections (tuberculosis) and nonspecific ones in 87 cases (63.5%), nonimmune diseases (SLE, PAN, sarcoidosis) in 50 cases. Generalized adenopathies were recorded in 47% of the cases, the involvement of a single node group in 21.8% of the cases, other types of distribution being rare. The general symptoms were absent in 20.5% of the cases, being present in the remainder of 79.5%, especially in the malignant lymphomas, leukemias, nonimmune diseases. The main complications occurring against the background of the etiological affections of lymphadenopathies were: infections (respiratory, urinary, tegumental) in 19.7% of the cases and cardiovascular disturbances (myocardiopathies, rythm and conduction disturbances) in 9.6% of the cases.
