Bladder training in older women with urinary incontinence: relationship between outcome and changes in urodynamic observations.

The purpose of this study was to clarify the mechanism by which bladder training affects urinary incontinence. Urodynamic data and specific urodynamic diagnoses of 108 women with urinary incontinence were compared before and 6 months after treatment with bladder training. Before treatment, 76 women had sphincteric incompetence, 11 had detrusor instability, and 16 had both. After treatment, 33 women no longer fulfilled the urodynamic diagnostic criteria for either sphincter or detrusor dysfunction. Controlling for severity before treatment, the number of incontinent episodes post-treatment was not associated with change in urodynamic diagnosis. Only the first sensation to void, voided volume, compliance, functional urethral length, and flow time showed any significant changes between pre- and post-treatment evaluations; however, none were correlated with change in the number of incontinent episodes. Bladder training does not appear to affect lower tract urodynamic variables or specific urodynamic diagnosis, and it is likely that its mechanism of action reflects adaptive behavioral changes. Physiologic changes not detected with techniques and/or criteria used in this study may still occur.

Cloning a fragment from the telomere of the long arm of human chromosome 9 in a YAC vector.

The construction of a yeast artificial chromosome containing a human DNA insert is reported. This molecule of about 200 kb behaves as a native yeast chromosome since it has a very high mitotic stability and is present in the yeast transformant clone at a copy number similar to that of the resident chromosomes. Hybridization with the TTAGGG sequence demonstrates that this chromosome contains human telomeric sequences. In situ hybridization of the biotin-labelled artificial chromosome to metaphase human chromosomes shows that the insert occupies a telomeric position on the long arm of chromosome 9. Since the fragment was cloned as an EcoRI insert and not as a telomere, it is situated medially to the telomeric sequences and harbours telomere-associated sequences, that have been shown to contain the TTAGGG sequence. The fragment represents the end of the genetic map of chromosome 9 and thus can be used to characterize the sequence and the structure of the chromosomal region that runs from the end of the chromosome to the first gene.

[Surgical technique aspects in multiorgan transplantation: personal experience].

The Authors report their experience with multiorgan transplantation performed in 3 patients affected by multifocal cancer of the liver, either primitive (2 cases) or secondary (1 case). The rationale for this new approach was the unfeasible single liver transplantation taking into account the extracapsular diffusion of the neoplasia.

[Transplantation of the pancreas: surgical aspects].

Based on their personal as well as international experiences, the authors describe different surgical techniques employed in pancreatic allograft related to different complications rate and patient and graft outcome. In fact, the surgical technique represents an only partially solved problem, because of the high percentage of complications dealing with the different approaches. Even though several procedures concerning either the site of implantation either the treatment of the exocrine function have been so far used, the results up to now obtained are the main limiting factor for a large clinical application of pancreatic transplantation.

[Pathology of sports injuries of the foot].

Foot injuries caused by sport have increased by number, due both to the fact that more people are practising sport at an amateur level, and to subliminal performance with particular engagement of the foot as an active instrument in most sports. Generic, usually occasional, traumatology must be distinguished from specific traumatology. The latter is substantially microtraumatic, but not necessarily of less importance, since the sequelae of an initially mild clinical picture are well known. Training (proprioceptivity of the ligaments) is acquiring a different meaning in the prevention of sports injuries. Ligamentous injuries of the tarsotibial joint, diseases of the subastragalic and the plantar flexor apparatus are of particular interest and referred to in the text.
