Long-term follow-up of dermatitis herpetiformis in children.

Dermatitis herpetiformis was diagnosed in seventy-six children by its clinical features and by detection of granular IgA deposits in the papillary dermis of perilesional skin. Enteric alterations demonstrated by measurement of D-xylose absorption and by small intestinal biopsies were detected in over 90% of all patients. A complete follow-up was obtained for all children who were followed for 3 to 10 years. Treatment with a gluten-free diet alone led to a reversal of the intestinal abnormality in 100% of our children and to the disappearance of cutaneous lesions in 82% of reported cases. This suggests that a complete remission of symptoms can be obtained with a gluten-free diet alone. Dapsone alone is effective therapy for the rash but does not affect the intestinal alterations. Furthermore, the lack of side effects to dietetic therapy for a long period of time makes the diet the treatment of choice in this disease.

Methionine-enkephalin, substance P, and homovanillic acid in the CSF of parkinsonian patients.

Methionine-enkephalin, substance P, and homovanillic acid concentrations were measured in the CSF of subjects not affected by neurologic disorders (group 1), and in parkinsonian patients who had a slight or moderate (group 2) or severe (group 3) disability. Homovanillic acid and substance P concentrations in the CSF of groups 2 and 3 were respectively lower and higher than in group 1. On the contrary, an increase in CSF methionine-enkephalin content was found only in group 2. Our results confirm in humans the close relation between the dopaminergic and peptidergic transmissions in the nigrostriatal system that has been observed in experimental animals.

[Nosographic limits of the Netherton syndrome].

This paper present the case of a 5 year old girl affected by ichthyosis vulgaris, atopic diathesis (asthma - eczema), hair anomalies as pili torti, trichorrhexis invaginata. Such data are compared with the most frequent symptoms Netherton's syndrome. Although some difference be extant, the authors deem it appropriate that this case be included within the framework of this syndrome. The problem is therefore posed once again of the boundaries of Netherton's syndrome. Also considered are the most interesting hypotheses recently set forth by different authors with the aim of better defining this disease whose biological mechanism is still unknown.
