[Reoperation of hip endoprostheses].

Experiences gained in 106 reoperations of 88 patients following 1968 total and 18 Wagner arthroplasties, performed in the Department of the authors between 30. 05. 1969. and 30. 06. 1991., are described. They think that after the THR yearly control and radiological examinations are necessary. Radiological signs in symptomless patients do not mean operative indication, in these cases regular control is thought to be even more important. Big loss of bone with loosening makes reoperation in symptomless patients necessary as with the progression of bone destruction the danger of fracture increases and the possibility of reimplantation becomes dubious. In dubious cases bone scintigraphy may be of help. The technical solutions used by them are described. At the planning of reoperation the age of the patient, the state of the acetabulum and femur are considered. Under 60 years of age and at loosening with big bone loss cementless prosthesis and bone transplantation are suggested. The most important is thought to be to reach stable fixation. Attention is called that the performance of reoperation needs good technical skills.

Osteoporosis--a modifying factor of surgical treatment.

Increasing knowledge of fragile bone has been gained by non-invasive mineral assessments. Its future importance seems to be twofold. First by, it seems likely that patients with low bone mineral will be treated to try to increase this bone mineral or at least to keep it steady. Secondly, in the presence of osteoporosis special strategies must be taken into consideration. In many locations osteoporotic fractures may need special solutions. It seems that osteosynthesis with plates and screws which have their definite indications in younger patients may be replaced by alternatives, such as cerclage wiring and intramedullary implants. Polymethylmethacrylate has been a good adjunct to strengthen screw fixation and to fill defects after compression of fragile cancellous bone.

[Rehabilitation of knee joint instability caused by pseudarthrosis of the lateral femoral condyle].

Authors report on the successful operative treatment, 15 years after the development of a pseudarthrosis, following the fracture of the lateral femoral condyle and causing knee instability. In this case the refreshing of the avascular pseudarthrosis gap reduction and screw osteosynthesis were performed. The basic principles of the distribution and treatment of pseudarthrosis are reviewed. Attention is called to the importance of exact reduction and stable fixation and to the fact that reconstruction can be carried out even long after the injury.

[Case report of vertebral osteomyelitis following operation for herniated disk (author's transl)].

Authors describe a case of vertebral osteomyelitis as assessed with help of X-ray pictures and clinical signs, which developed following an operation for herniated disk, and healed on conservative treatment. This complication was only once encountered in a material of 1406 operated cases. It is pointed out that with increasing number of hemilaminectomies this complication might appear more often.

Effect of denervation on the longitudinal growth of tubular bones.

The effect of sciatic nerve lesion on the longitudinal growth of tubular bones of the affected extremity has been studied in adolescent rabbits. For the observation of circulation in the bones, intravenously injected Sr85Cl2 solution was used. A retardation of longitudinal growth of the tibia and of the femur on the operated side was noted. Radioactivity in the bones of the operated extremity was always lower than on the control side. For the retarded longitudinal growth, a circulatory disturbance may have been responsible.
