[A rare cumulation of vena cava catheter complications in the same patient (author's transl)].

Two complications that occurred in connection with the use of CVP catheters in the SVC and IVC in one patient are described: 1. Extravascular injection in the superior mediastinum and in the extrapleural space. 2. Massive thrombosis of the IVC. The differential diagnosis of a wedening of superior mediastinum is discussed. Recommendations are made to prevent these complications.

[Control of respiratory therapy by means of rheography (author's transl)].

Rheography measures changes in electrical conductivity. In measurements taken at the thorax it was appeared that the changes in conductivity caused by breathing are superimposed by the changes caused by circulatory factors and exceed them in amplitude quite far. This makes the expansion of rheography for electrical respiration control possible. The changes in electrical conductivity are explained by inspiration into the alveoles, by the increase in the distance of the electrodes on thoracic expansion, and by the change in the intrathoracic blood volume. When using respiratory therapy the physiotherapist must pay great attention how the patient reacts to respiratory therapy and whether he is adapted correctly to the mechanical respiratory support devices. The method of "rheospirography" was tested as to whether it allows an objective, direct evaluation of the interplay between respiratory therapy and patient. The method appears to be able to control respiratory therapeutic measures as it allows an evaluation of the respiratory type, the regional ventilation, and the adaption of the patient to the device.

[Influencing of the kininogen-Kinin-System by means of a kallikrein-inhibitor during extracorporeal circulation in open heart surgery (author's transl)].

During extracorporeal circulation for open heart surgery an activation of the kininogen-kininsystem occurs with a decrease in plasma-kininogen-levels significantly exceeding the effect of haemodilution, and thus being due to an increased liberation of kinin. This effect could be suppressed by kallikrein inhibition by means of a protease-inhibitor.

[Rheographic monitoring of lung ventilation (author's transl)].

Electrical conductivity changes are measured with rheography. Changes of conductivity on the thorax caused by respiration, superimpose and exceed those caused by different circulatory conditions. Therefore rheography was adapted for monitoring of respiration. Changes in electric conductivity can be explained by the inflow of air to the alveoli, by increase of the distance between electrode, and by changes of the intrathoracic blood volume. Rheography developed by Polzer and Schuhfried was tested in the intensive care unit and the respiration therapy department. It was shown that this method is suitable for control of respiration, for demonstrating regional differences of pulmonary ventilation, for checking of respiration therapy and for monitoring the ventilated patient.

[Pulmonary edema due to unilateral pneumothorax (unilateral shock lung) (author's transl)].

Unilateral pulmonary edema due to chest drainage is reported in the present paper. Drainage was carried out, because of pneumothorax. The disorder could be treated successfully with differenciated mechanical respirator therapy and additionally with methylprednisolon, heparin, spirolactone and dopamin. Based upon clinical symptoms and on present literature we suggest to account this so called "pulmonary edema exvacuo" to the main complex of acute respiratory insufficiency (unilateral shock lung).

[Clinical and experimental results with acupuncture-analgesia (author's transl)].

Operations under acupuncture analgesia have been performed in Vienna since March 1972. A total of 102 procedures has been performed with "classical" Chinese acupuncture analgesia. Additionally given analgesics, psychodynamic factors and neurophysiological components are of joint importance. Our success rate in these cases was 64 p.c. Since October 1972 we call "two-phase-acupuncture-analgesia". Up to June 1975 72 operations were performed under some modifications of this method. In a neurophysiological study 17 volunteers and in 2 patients undergoing stereothalamotomies depression or even disappearance of evoked potentials was be observed during acupuncture with alectrical stimulation. Thus an interaction between pain stimuli and acupuncture stimuli in the thalamus could for the first time be demonstrated in man.

[A case report of caesarian section carred out under electroanasthesia (authors transl)].

The case is reported of a 19 year-old patient in whom Caesarian section was successfully performed under combined method of anaesthesia. Square waves, which consist of high frequency impulses were transmitted to the cerebrum, and in addition, a 50:50 nitrous oxide and oxygen gas mixture was administered to the patient. The intraoperative condition of the patient was satisfactory. No reactions to pain were seen, as judged from constant observation of the blood pressure, pulse rate, colour of the face, pupils and the condition of the body extremities.

[The influence of acupuncture analgesia on the sensation and feeling of pain and the autonomic state of the organism (author's transl)].

This study reports the effect of acupuncture on the sensation of pain, feeling of pain and on the autonomic state of the organism. In order to elucidate the problem physiological and psychological parameters were employed. It was observed that the insertion of the needles with electrical stimulation raised the pain threshold. The rise occurred whether a classical acupuncture point or a placebo acupuncture point was used. When the pain was assessed subjectively, it was found that the needling of a classical acupuncture point favourably influenced the experience of pain. Furthermore, persons in whom acupuncture worked well were characterized by a low subjective and objective pain threshold, a slow pulse rate and a high anxiety score.

Telemetric ECG data (biorhythms) in operations with acupuncture analgesia in the People's Republic of China.

During seven operations with acupuncture analgesia in the People's Republic of China, recordings were made with a self-developed device for telemetric transmittance of the ECG. Among others, the amplitude of the QRS complex as well as the period of time between two cardiac cycles were electronically measured and registered for evaluation. Through comparison of the values from China with our own material, the following statements can be made: (1) All patients were in conditions ranging from restlessness and slight excitation to weariness and drowsiness. A condition similar to narcosis must be excluded. (2) In a large number of cases a preoperatively performed strong psychological sedation through pharmacological agents can be excluded with certainty. (3) The massive reaction to events such as the cleansing of the operative field refutes-at least partly-our concept of "Asiatic indifference". (4) Acupuncture cannot depress reflexes. (5) Finally, the hypothesis that acupuncture has a stabilizing effect on the circulation is invalid.

[Telemetric ECG data (biorhythms) during operations under acupuncture analgesia in the People's Republic of China (author's transl)].

On an educational journey to China, we were able to record and accumulate electrocardiograms during operations under acupuncture analgesia with the aid of a self-developed apparatus. To elaborate this among others the amplitude of the QRS-complex as well as the time period between two cardiac cycles were electronically measrued and registered. With examples from the comprehensive material it is possible to draw the following conclusions: 1. The patients were in conditions ranging from restlessness and irritation to weariness and drowsiness. A condition similar to that resulting from narcosis must be excluded. 2. A pre-operatively undertaken strong psychological sedation by means of drugs can be excluded with certainty. 3. The massive reaction to events such as the washing out of the operative field refutes, at least partly, our concept of "Asiatic indifference". 4. Acupuncture is not able to depress reflexes. 5. Finally, the hypothesis that acupuncture has a stabilizing effect on the circulation is not valid.
