Correlation between plasma lipoprotein Lp(a) and sex hormone concentrations: a cross-sectional study in healthy males.

High concentrations of lipoprotein Lp(a) have been related to atherosclerotic disease, both at coronary and cerebrovascular levels. Although Lp(a) levels are under a strict genetic control, being inversely related to the molecular weight of apo(a) isoforms, an interference of endogenous sex steroids on Lp(a) metabolism has been hypothesized. The aim of this study was to investigate the interrelationship between plasma Lp(a) and sex hormone concentrations in 98 healthy males, controlled as for their nutritional status by anthropometric measurements. Statistical evaluation was performed employing simple and multiple stepwise regression analysis. No significant correlation was found between Lp(a) levels and fT, E2 and gonadotropins, while they were positively and independently related to LDL-cholesterol and DHEA-S. As for the other lipoproteins, a positive correlation between HDL-cholesterol and E2 and an inverse correlation between triglycerides and SHBG were observed. These data suggest that endogenous testosterone and estradiol do not affect Lp(a) metabolism in males, at least in physiological concentrations. However Lp(a) might be affected by DHEA-S, the most abundant product of the adrenal gland. The positive correlation of HDL-cholesterol to E2 suggests that estrogens play a major role in lipid metabolism also in males, in spite of their low concentrations; more complex to be explained is the finding of an inverse relationship between Tg and SHBG. Further studies are needed in order to clarify the influence of sex steroids on lipid metabolism, mainly on Lp(a), under physiological conditions; population samples homogeneous in terms of apo a isoforms could be the ideal objects of such studies, in order to avoid the great interindividual variability of Lp(a) concentrations.

A case of trichorhinophalangeal syndrome, type I.

A single case of trichorhinophalangeal syndrome, type I, is reported. The patient manifested the complete triad of the syndrome: thin, sparse, slow-growing scalp hairs; a pear-shaped nose; and cone-shaped epiphysis in some interphalangeal articulations of both hands. In addition, tooth decay, pectus carinatum, lumbar scoliosis, and polycystic ovary were observed. This multisystemic disorder is apparently rare, but its prevalence is probably underestimated in the dermatologic literature.

Melanocytes freshly isolated from normal human skin express the cell membrane receptor for the adhesive glycoprotein thrombospondin.

Thrombospondin (TSP) is an adhesive protein with multiple binding sites, which is able to mediate several cell-to-cell and cell-to-matrix interactions, particularly through its cell membrane receptor (TSP-R). Because human keratinocytes are able to synthesize and express TSP, and as TSP is also localized at the dermal-epidermal junction in normal human skin, we questioned whether epidermal cells are able to bind available TSP, that is, to express TSP-R. To investigate this, we employed gold immunoelectron microscopy on epidermal cells freshly isolated from normal human skin; the TSP-R was detected by OKM5 monoclonal antibody. Epidermal cells showing ultrastructural characteristics of melanocytes were gold-stained on their plasma membrane, whereas keratinocytes, Langerhans cells and lymphocytes were unstained. Although functional studies are clearly necessary to clarify the role(s) played by the TSP-R on the cell surface of melanocytes, it is tempting to speculate that the TSP-R may be important for melanocyte adhesion to the dermal-epidermal junction and to keratinocytes. Such adhesion may not only subserve the steric localization of melanocytes, but also have important implications for those functional activities of melanocytes which have been shown to require close contact between these cells and adjacent keratinocytes and/or basement membrane components.

[Neurological changes in aged patients with fractures of the femur: prevalence and clinical aspects].

This study evaluates the prevalence and clinical significance of the main neurological diseases in 246 hospitalized elderly patients with hip fracture, who have to undergo rehabilitative treatment. Eighty-three patients already had clinically noticeable neurological diseases before fracture; they were compared to other components the survey. As for the type of fracture and surgical treatment, differences have not been found. Medical conditions have proved worse among neurological disease bearers; the same patients also had to run into more complications. What results from the study, considering the high prevalence of neurological diseases in patients with hip fractures, suggests that these neurological diseases are important risk factors for accidental falls and it's necessary to provide these patients with careful, continuous hospital assistance.

[Evaluation of the effects of reduced glutathione in subjects with acquired and congenital changes in heme biosynthesis].

In order to evaluate the possible therapeutical effects of reduced glutathione in subjects affected by acquired or congenital heme biosynthesis alterations, two groups of subjects have been considered: the first, of 5 subjects with abnormal lead absorption, the second of 10 patients suffering from porphyria cutanea tarda (PCT). The drug was administered i.v. at the dosage of 600-1200 mg for a variable period of 3-5 days in the first group, for 7-10 days in the patients with PCT. In the lead-exposed subjects an increase in the activity of ALA-dehydratases enzyme was observed, with a decrease in the urinary excretion of delta-aminolevulinic acid, and a decrease in the percentage of urinary coproporphyrins as well. In the patients with PCT, after the therapy, a relevant reduction of urinary excretion of porphyrins, prevalently uro- and heptaporphyrins, was noticed. The results of this preliminary study suggest that glutathione can exert an effect of "normalization" on heme biosynthesis when altered in acquired and/or congenital conditions.

[Proposal for a therapeutic protocol in Crosti lymphoma].

Three patients with Crosti's lymphoma were treated with radiotherapy using electron accelerators. The energy radiations ranged from 5 to 9 Mev. with large fields. The total dose was 40 Gy, delivered in 2 Gy fractions daily. The treatment lasted four weeks. No side effects were reported and no recurrence was remarked after 24 months.

[Sweet syndrome. Description of 12 cases].

The clinical course of 12 patients (5 males, 7 females) with Sweet's syndrome is described. Clinical and histological features and the dramatic response to corticosteroids were suggestive of Sweet's syndrome. Four patients experience recurrences in previously involved sites, controlled with administration of prednisone. In no cases the dermatosis was associated with systemic diseases.
