Lichenoid photocontact dermatitis to musk ambrette.

A patient is described with a pigmentary dermatitis of the face with histological features of a lichenoid reaction, which proved to be a photocontact dermatitis to musk ambrette. Photopatch tests produced a pigmented dermatitis that reflected the clinical and histological features of a lichenoid reaction, which occurred 1 month after application.

[Use of vasopressin in the treatment of acute hemorrhagic lesions of the stomach. Our experience].

Since bleeding from acute lesions of the gastric mucosa can cease spontaneously and the mortality rate of emergency surgery is high, conservative treatment is always preferable. Satisfactory results were obtained with continuous infusions of vasopressin in low doses (0.2 U/kg/hr for 8 hours) so that this treatment appears a valid alternative to more recent techniques (somatostatin).

[Use of vasopressin in the treatment of bleeding esophageal varices. Our experiences].

Results obtained in the control of oesophageal varix rupture haemorrhage by intravenous vasopressin perfusion or selective intraarterial administration are reported. This comparative study shows intravenous administration to be the best method since it produces the same therapeutic effects with fewer undesirable side-effects than when administered arterially. In view of the high level of complications caused by selective arterial catheters, this administration method would only appear justified in cases where selective arterial catheterisation is to be carried out in any case.

Comparison between sensitivity of a viscometric method and sensitivity of the alkaline elution assay for the determination of DNA damage induced by dimethylsulfate in vitro.

DNA damage induced by dimethylsulfate (DMS) was measured with a new oscillating crucible viscometer, having a U-shaped circular channel. Rat liver nuclei were treated in vitro. Viscosity was measured by lysing nuclei in an aklaline lysing solution (pH 12.5; 25 degrees C). Nuclei were lysed immediately in the viscometer and released DNA started to uncoil. In control samples the viscosity increased very slowly with time, reaching a maximum only after about 8 h. A progressively more rapid increase in viscosity was seen with increasing concentrations of DMS. The time of DNA disentanglement was sensitive to about 30 times less breaks than the alkaline elution assay.

Dynamic helical CT of breast tumors.

PURPOSE: The purpose of our study was to test dynamic helical CT (hCT) in the evaluation of breast tumors. METHOD: Thirty-six patients with 39 suspected lesions underwent breast hCT before and 1, 3, and 8 min after intravenous contrast medium administration. A marked enhancement (> or = 100%) at 1 or 3 min without increase at 8 minutes was considered as the primary indicator for malignancy. Twenty carcinomas, one metastatic non-Hodgkin lymphoma, six fibroadenomas, and six other benign findings were histologically assessed. Six cases had negative fine needle aspiration cytology and at least a 2 year negative follow-up. RESULTS: hCT showed a 100% sensitivity and 83.3% specificity. Considering carcinomas and fibroadenomas, significant differences were found for the percent enhancement at 1 min (p = 0.002) as well as for the density increase or decrease at 3 versus 1 min (p = 0.0035), at 8 versus 1 min (p = 0.0027), and at 8 versus 3 min (p = 0.0180). CONCLUSION: hCT proved to have a high diagnostic efficacy in evaluating breast tumors. Even though it involves some exposure to radiation, it should be considered in patients in whom MR is contraindicated.

[Treatment of breast carcinoma. Our experience].

The results of a study on 848 patients with breast carcinoma having maximum diameter less than or equal to 2 cm, that underwent a surgical treatment at the Institute of Special Surgical Pathology B (1980-82) and at the Institute of Surgical Clinic I (1980-89) are presented. The study has the purpose of evaluating, by the analysis of ten years surviving curves, the effectiveness of different types of interventions: Halsted radical mastectomy (98 patients), Patey radical mastectomy (245 patients) and Madden radical mastectomy (151 patients), quadrantectomy (260 patients). The age of patients ranges from 29 years to 92 years (average 56 years). The 68.04% of all tumors have resulted invasive ductal carcinoma (NOS) the 9.08% lobular carcinoma. Of the 848 patients included in the study 698 are alive at the end of the follow-up, with a survival rate, calculated with the actuarial method, equivalent to 63.38%. Results show the survival is independent of the type of surgical treatment (71.9% for quadrantectomy; 70.7% for Halsted radical mastectomy, 68.9% for Patey radical mastectomy; 70.7% for Madden radical mastectomy).(ABSTRACT TRUNCATED AT 250 WORDS)

Feasibility of thymidine labelling index on fine needle aspirates from breast cancer: comparison with surgical samples.

Fine-needle aspiration (FNA) provides a suitable diagnostic tool in the management of patients with breast cancer lesions. The current study reports on tumor proliferative activity, by 3H-Thymidine Labelling Index (TLI), assessed on 59 FNA (TLI1) and 28 surgical specimens (TLI2) from the same breast cancer patients. Median TLI values from FNA and surgical material were 1.0% and 0.7%, respectively. In the 28 patients, evaluable for the comparison between TLI1 and TLI2, the association was found to be highly significant (p = 0.000). Moreover, no change in tumor proliferative activity was observed in the majority (79%) of cases when evaluated preoperatively and at surgery. This study confirms the feasibility of TLI analysis on FNA from breast cancer and provides results superimposable on those obtained in a tissue sample from the same patient.
