Free amino acid excretion in tetraplegic patients.

Free amino acid chromatographic fingerprints obtained in automated analysers in 24-hour urines may help to quantitate muscle and collagen changes occurring after cerival spinal cord injury. The technique was used to compare the urine of six healthy and ten tetraplegic men, 18 to 25 years old, three of whom became paralysed in diving, one in a mountain fall, one one in the fall of a pipe, four in car accidents, and one in a football game. Nine weeks to 14 years after injury, the tetraplegics excreted less total free amino acids, lysine, and 3-methylhistidine than the controls, whereas hydroxyproline and hydroxylysine were similar in both groups. Values of urine 3-methylhistidine, measured 9, 13, 19 and 20 weeks post onset in one patient, and 9, 12, 16, 18, 23, and 26 weeks post onset in another, were related to the duration of paralysis. These results indicate that collagen and muscle metabolism may not be quantitated by measuring urine free amino acids.

Relationship between outer ear resonance and permanent noise-induced hearing loss.

INTRODUCTION: Prolonged exposure to noise produces permanent hearing loss in the frequency region centered around 4,000 Hz despite differences in the spectral and temporal characteristics of the noise. It is unclear to what extent properties of the auditory system contribute to the development of permanent hearing loss in this frequency region. This study was designed to evaluate the relationship between the frequency of peak outer ear resonance and the frequency of maximum hearing loss in a population of patients with a history of noise exposure. MATERIALS AND METHODS: Analysis of the frequency of maximum hearing loss was completed with sweep frequency Bekesy audiometry with resolution of 100 Hz. Peak outer ear resonant frequency was determined with a resolution of 175 Hz. RESULTS: The mean frequency of maximum hearing loss was 4,481 Hz whereas the mean peak outer ear resonant frequency was 2814 Hz for 43 ears. Pearson product correlation coefficient equaled .64 (P < .0001). CONCLUSIONS: Despite differences in the type and duration of noise exposure reported by the patients, the positive correlation between outer ear resonant frequency and frequency of maximum hearing loss emphasizes the role that external ear properties play in the development of the 4,000 Hz audiometric notch.

Dietary and serum lipids in individuals with spinal cord injury living in the community.

A cross-sectional study of 189 community-dwelling persons with spinal cord injury (SCI) (a) assessed levels of dietary and serum lipids, (b) determined the proportion of persons whose levels were out of the recommended/desired range, and (c) identified predictors of dietary and serum lipids. Lipid levels were out of range for a substantial proportion of the sample. Older persons were likely to have higher serum cholesterol and higher triglyceride levels than younger persons. Men tended to have higher intake of dietary cholesterol and lower levels of HDL than women. Caucasians and Hispanic-Americans tended to have higher triglycerides than African-Americans. Persons who had lived with SCI less time tended to have higher saturated fat intake and higher triglycerides than those who had lived with it longer. Greater saturated fat intake was associated with higher serum cholesterol after controlling for age. Studies are needed that test the effectiveness of various interventions on controlling dietary and serum lipids for persons with SCI.
