Histochemical and confocal laser scanning microscopy study of the bone-titanium interface: an experimental study in rabbits.

The aim of our study was an analysis of the presence of an unmineralized bone matrix between mineralized bone and titanium screws in rabbit tibiae. A microscopical analysis, using a histochemical technique, was performed on the titanium-bone interface of commercially pure titanium implants placed in rabbit tibiae and harvested after 2 months. Thin ground sections of the specimens were prepared by the cutting-grinding system and stained using the von Kossa method for calcium salts and basic fuchsin for osteoid. The microscopical and morphometrical evaluation showed that bone covered about 40% (+/- 7.5%) of all implants. Mineralized bone was, however, in direct contact with the titanium surface on only about 10% of the implant, while in the remaining 30% the mineralized bone was separated from the implant by an unmineralized tissue. This basophilic, probably osteoid matrix, could represent the medium that allows the biochemical exchanges between bone and cells under the influence of the implant. A small, optically translucent gap (1-5 microns), probably an artifact, was present in some areas between titanium and bone. Confocal laser scanning microscopy (CLSM) in a fluorescent mode showed the presence at the interface of a fluorescent material. Results from our study showed that light microscopy of thin ground sections allowed a good analysis of the real nature of the titanium-bone interface. Moreover, this double staining technique showed the presence of an unmineralized bone matrix at most of the bone-titanium interface.

Light and confocal laser scanning microscopic evaluation of hydroxyapatite resorption patterns in medullary and cortical bone.

An animal study was conducted with unloaded blocks and hydroxyapatite (HA)-coated titanium implants. Four HA blocks were positioned in rabbit tibiae and four HA-coated titanium implants were positioned in pig tibiae. Implants were positioned so that half was placed in cortical bone and half in medullary space. Biopsy specimens were taken 4 months after implant placement for histologic evaluation. Light microscopy and confocal laser scanning microscopy demonstrated that the HA resorption rate was higher in the medullary spaces, whereas resorption was almost absent in the areas embedded in cortical bone.

[Nimesulide: multicenter clinical study for evaluation of its therapeutic effect in odontostomatology].

A Polycentric Study was carried out so as to assess the anti-inflammatory effectiveness and pain-killing activity of Nimesulide in Odontostomatological practice, in 78 patients suffering from alveolo-gingival and parodontal abscesses, consequences of tooth extraction, suppurated radicular cysts, dysodontiasis, osteitis, periodontitis, periostitis. The results obtained confirm the anti-inflammatory and analgesic effect as well as the good tolerability of this drug.

Popliteal pterygium syndrome presenting with orofacial abnormalities. Report of a family.

A family with popliteal pterygium syndrome is reported: two children presented a mild form of the syndrome, while their mother exhibited full expression of the gene. Only 11 families with 29 affected members and 25 sporadic cases have been published. This malformation syndrome is inherited in an autosomal dominant pattern, but environmental factors or genetic heterogeneity cannot be excluded in sporadic occurrences. The variability of expression is discussed and a comparison with previously published familial cases is made: minor anomalies, if present in relatives of patients with full gene expressions, are as helpful in making the diagnosis as are major abnormalities. Careful evaluation of the proband's family is essential for diagnosis and subsequent genetic counselling.

[Experimental arterialization of the portal vein in the rat].

The article reports an original technique of arterialisation of the portal vein in the rat, after porto-cava shunt, with termino-latreal anastomosis between the proximal stump of the portal vein and the aorta; compared with other techniques, this method has the advantage of avoiding right nephrectomy.

[Notes on the embryology and anatomy of the diaphragm].

The main steps in the embryogenesis of the diaphragm are summarised, with particular reference to details of major importance in the formation of congenital hernias. Morphogenetic abnormalities of the diaphragm are described. Malformations occurring during the different stages of embryonal development (agenesia, total or partial aplasia or eventratio, restructuring defects) and their incidence are also illustrated. A personal view concerning the topography of congenital hernias of the diaphragm is expressed, with particular reference to those proceeding through incostant foramina.

[Acute complications of hernias caused by congenital defects of the diaphragm].

Following a classification of the various types of congenital hernia of the diaphragm, the acute complications that may be caused by such defects of diaphragmatic morphogenesis are reviewed. Relative physiopathology is considered, particularly as regards the thoracic dimension of large hernias of the cupola. As regards visceral hernia, the most typical pictures of the two commonest complications, volvulus and strangulation, are described among other aspects. The clinical situation is outlined bearing in mind the diversity of anatomoclinical pictures in relation to the age of the patient, the type of hernia involved, and the period in which the observation was made. The surgical problems involved in acute complications concern the approach--although agreement is almost unanimous depending on the type of hernia--viscous reduction modalities and the type of plastic surgery which is required on each occasion. Postoperative complications and the prognosis for these hernias are reviewed briefly.
