[Value of the serum measurement of alpha-L-fucosidase in the diagnosis of hepatocarcinoma].

The serum catalytic concentration of alpha-L-fucosidase (AFU) was measured to evaluate its usefulness for the diagnosis of hepatocarcinoma (HPC). Fifty-one patients with hepatocarcinoma, 30 with hepatic metastases, 40 with hepatic cirrhosis, 22 with other hepatic diseases and 48 healthy individuals were evaluated. The values of serum AFU were significantly higher in all groups (p less than 0.001), than in the reference group; there were also significant differences between those with HPC and hepatic cirrhosis (p less than 0.01), and between HPC and other hepatic diseases (p less than 0.05). The highest effectiveness of the test was achieved with a catalytic concentration of serum AFU of 210 nKat/l, with a 37% sensitivity and a 91% specificity. When the AFU measurement was used concomitantly with that of alpha-fetoprotein (AFP) for the diagnosis of HPC, sensitivity increased to 72% when AFI or AFP were elevated, and specificity reached 99% when both were increased. We think that the measurement of serum AFU may be helpful for the diagnosis of hepatocarcinoma.

Adverse outcomes and lack of health insurance among newborns in an eight-county area of California, 1982 to 1986.

In this study of hospital discharge data on births to residents of an eight-county region of California, we found an increasing lack of health insurance that was associated with an elevated and increasing risk of adverse outcomes in newborns. Between 1982 and 1986, the percentage of newborns without health insurance increased overall by 45 percent (from 5.5 to 8.0 percent; P less than 0.001); the increases were larger among Asians (by 54 percent [from 7.8 to 12.0 percent]; P less than 0.001) and Latinos (by 140 percent [from 8.2 to 19.7 percent]; P less than 0.001). By 1986, the odds ratio for an adverse hospital outcome (defined as a prolonged hospital stay, transfer of the newborn to another institution, or death) was 1.31 (95 percent confidence interval, 1.17 to 1.46) in uninsured as compared with privately insured newborns, with control for race or ethnic group. There was a significant increase in risk over time (P less than 0.03); the comparable odds ratios in 1982 and 1984 were 1.11 (95 percent confidence interval, 0.93 to 1.33) and 1.19 (1.05 to 1.35), respectively. In 1986 the risks were especially elevated for uninsured as compared with privately insured blacks and Latinos (odds ratios, 2.24 [95 percent confidence interval, 1.60 to 3.13] and 1.56 [1.26 to 1.94], respectively); the increases in risk over time were the most marked in these groups. We believe that the elevated and increasing risks for uninsured newborns are explained at least in part by inadequate and diminishing access to care and that this burden is borne disproportionately by blacks and Latinos.

A pentameric form of human serum amyloid P component. Crystallization, X-ray diffraction and neutron scattering studies.

Human serum amyloid P component crystallizes from sodium acetate buffer (pH 5.5) in the presence of calcium and polyethylene glycol 6000, at 4 degrees C. The space group is P2(1) and the cell parameters are a = 69.0 A (1 A = 0.1 nm), b = 99.3 A, c = 96.8 A, beta = 96.1. Density considerations supported by neutron scattering and gel filtration experiments indicate that the species crystallized is pentameric. The orientation of the pentamer 5-fold axis is determined and a crystal packing for the discs is proposed.

[Standardization of a microtechnic for passive hemagglutination to detect antibodies against infectious bovine rhinotracheitis].

A micromethod for passive Hemagglutination is described for the detection of IBR antibodies in serum samples of nine herds. The samples correspond to different zones of the province Buenos Aires. In animals without clinical signs the proportion of positives was 43.60%; in herds with signs consistent with the disease, this value attained 66.4% positivity (Tables 1 and 2). The feasibility of this method is discussed. Some variants introduced into the original technique are described.

The evolution of renal function after surgical treatment of malignant lithiasis with severely compromised renal function.

16 patients with calcic nephrolithiasis and renal failure were admitted to our institute and underwent surgery. 4 patients had bilateral nephrolithiasis and 12 cases had relapsed. Selective nephrotomy was performed in all cases, pyelotomy was done in 8 and partial nephrectomy in 2 patients. In 8 patients renal function improved but no difference was observed in 6 patients. Renal function worsened in 2 patients who had shown significant preoperative proteinuria. According to our experience, the success of this therapy depends on: (1) accurate patient selection; (2) adequate preoperative treatment, and (3) personalization of surgical management.

[Carcinoma of the penis: our current therapeutic attitude].

24 cases of penile carcinoma, occurred from 1970 to 1979, have been examinated. The role of limphoadenectomy, the treatment of the primitive lesions and their consequences are discussed. It is opinion of the authors that the risks and the benefits of the treatment must be carefully evalued.

Synthesis and biological activity of some 15-oxaestranes.

The estrogenic activity of orally administered 15-oxaestrone was evaluated by the uterotropic assay in rats and was found to be 12 times greater than that of estrone. In addition, several analogues of 15-oxaestrone were prepared and their estrogenic potency was determined.
