[Clinical significance of serum NCC-ST 439 as a tumor marker for colorectal cancer].

Clinical significance of preoperative serum NCC-ST 439 level was studied in 119 cases of colorectal cancer (90 of primary, 29 cases of recurrent). The positive rates for serum NCC-ST 439 were 28.9% in primary and 65.5% in recurrent cases. The false positive rates for benign disease were 5.6%. These rates were low when compared with those for other tumor markers. The positive rates for serum NCC-ST 439 exhibited a strong correlation with wall invasion, lymph node and liver metastases. A combination assay of NCC-ST 439, CEA and CA 19-9 produced a high positive rates as 43.3% in primary and 86.2% in recurrent cases. These results demonstrate that measurement of serum NCC-ST 439 may be useful for cancer staging and improves the diagnostic rate in combination with CEA and CA 19-9.

Upper GI examinations in older premature infants with persistent apnea: correlation with simultaneous cardiorespiratory monitoring.

Upper gastrointestinal examinations with simultaneous cardiorespiratory monitoring were performed in 39 older premature infants with persistent apnea. Swallowing incoordination was documented to be causatively related to persistent apnea in such infants, especially with feeding. Direct relationship between apnea and gastroesophageal reflux was not documented in this study.

Intraarterial digital subtraction angiography with carbon dioxide: superior detectability of arteriovenous shunting.

Intraarterial digital subtraction angiography (IADSA) with carbon dioxide (CO2) was performed on 41 patients with liver or renal diseases. CO2 produced no hypersensitivity reactions, and the pain or feeling of warmth was relatively mild compared with iodinated contrast media. Although the image quality of the arterial or capillary phase was inferior to that with iodinated contrast media, the detectability of arteriovenous shunting was excellent. IADSA with CO2 may become an effective method for detecting arteriovenous shunting which cannot be demonstrated with conventional angiography or DSA with iodinated contrast medium.

[Malignancy of gastric cancer according to DNA ploidy pattern and tumor markers].

Microfluorometric analysis of DNA content was performed on samples from 172 gastrectomy specimens of cancer. Histograms of DNA content were classified into three ploidy patterns; type I, II and III. Also, immunocytochemical examination for tumor markers (CEA, AFP and hCG) was performed. Type I was predominant in intramucosal carcinoma. Type III was predominant in differentiated carcinomas, in elevated type of early carcinomas and in gross type 1 and 2 of advanced carcinomas. Prognostic serosal factor exerted a greater influence on the prognosis than ploidy pattern. But in the patient without prognostic serosal factor, type III had a poorer prognosis than the other types because of vessel invasion, lymph node metastasis and liver metastasis. AFP and hCG were highly positive in type III. Furthermore type III cancer producing more than two tumor markers had an extremely poor prognosis. Analysis of ploidy pattern and tumor markers may prove to be a highly useful adjunct in the evaluation of malignancy in gastric cancer.

[A case of parotid tumor showing remarkable regression following hyperthermo-chemo-radiotherapy].

A 72-year-old woman developed adenocarcinoma of the left parotid gland. Because of the excessive size of her tumor and the fact that she suffered from severe liver dysfunction, she was treated by hyperthermo-chemo-radiotherapy (HCR therapy). After ten sessions of radiofrequency hyperthermia with HEH 500 (13.56 MHz radiofrequency wave), 50-Gy irradiation from a linac and administration of 33.0g of tegafur in suppository form, the tumor mass showed remarkable regression decreasing in size by as much as 84% on computed tomography. Histologically the tumor which was resected under local anesthesia, showed almost total necrosis. The multidisciplinary HCR therapy was well tolerated and effective as a therapy for cancer in this case.

[Huge Virchow's metastasis in gastric cancer made resectable by hyperthermochemoradiotherapy].

A 75-year-old woman who had undergone gastrectomy for gastric cancer at the age of 61 developed a huge left supraclavicular tumor (15 X 10 X 10 cm) suspected of being Virchow's node. She was treated with hyperthermochemoradiotherapy (HCT-therapy). After 10 sessions of radiofrequency hyperthermia by a Thermotron RF8, irradiation with 52.8 Gy of 60Co and injection of 8 mg of MMC, the tumor mass decreased by 77% on CT. The histologically resected specimen revealed coagulation necrosis in almost all areas. This case, which was considered HCR-therapy-effective, was estimated as Grade 3 according to The General Rules for the Gastric Cancer Study (The 11th edition.)

[A study of malignancy in gastro-intestinal carcinoids with special reference to the association of the prognosis and peptide hormone or oncofetal protein producing pattern].

It is difficult to differentiate malignant carcinoid tumors from benign ones early in the course of the illness. We investigated retrospectively a series of thirty-nine gastro-intestinal carcinoids not only for primary tumor anatomic site, macroscopic type, tumor size, histological depth, vessel invasion and mitosis, but also for peptide, ectopic hormone and oncofetal proteins by immunohistochemistry. Carcinoids primary in the appendix or rectum rarely metastasize, but those primary in the stomach or colon grow rapidly. Those greater than 2 cm in diameter and Borrmann's type had a poor prognosis. Carcinoids with deep penetration, vessel invasion or mitosis easily metastasize to lymph nodes or to the liver. Peptide hormone producing carcinoids were present in 21 of 39 (54%) tumors, but there were no correlation between the prognosis and peptide hormone producing patterns. Carcinoids producing carcinoembryonic antigen (CEA) were demonstrated in 11 of 37 (29%) tumors. These tumors were greater in size, more deeply penetrating and had more remarkable vessel invasion and mitosis than CEA non-producing ones. These results show that CEA producing carcinoids grow rapidly and easily metastasize. We concluded that these findings in carcinoid tissue appeared to be a reliable markers for metastasis and poor prognosis.

Malignant teratoma in the brain. An immunohistochemical study.

A case of intracranial malignant teratoma found in a 27-year-old man was reported. This unique tumor was found in the right frontal lobe separated from the pineal region and revealed various tissue components such as stratified squamous epithelium, glandular tissues, neuron, glia, ependyma, fibromuscular tissue, cartilage, bone, hemangiomatous lesion, melanin-laden cells, and some germ cell components. An immunohistochemical study demonstrated the presence of S-100 protein, glial fibrillary acidic protein (GFAP), neuron-specific enolase (NSE), carcinoembryonic antigen (CEA), and Factor VIII in some tumourous components. In particular, the distribution of S-100 protein in some germ cells suggested the possibility of the neuroectodermal origin of the germ cells or, alternatively, differentiation to the neuroectoderm.

Herpes simplex viral infection in human neonates: an immunohistochemical and electron microscopic study.

Specimens obtained at autopsy from six neonates with herpes simplex virus (HSV) infections were examined microscopically, electron microscopically, and immunohistochemically. Coagulative necrosis with inclusions was found in the livers and adrenal glands in all cases, as well as in various other organs, including the spleen, bone marrow, lungs, esophagus, tongue, and thymus, in some cases. Distinct hemorrhagic diathesis was found in three cases. No characteristic clinical findings, such as skin rashes or elevated titers of the antibody to HSV, were found, and clinical diagnosis was therefore difficult. In three cases isolation and typing of the causative virus were performed virologically, and type 1 HSV (HSV-1) was identified as the causative virus. Immunohistochemically, the type and distribution of the virus were evaluated in all cases with type-specific antisera to types 1 and 2 (HSV-2) antigens by the peroxidase-antiperoxidase method. In five cases the infections were found to be due to HSV-1 and in only one case to HSV-2. In the placenta in one case of HSV-2 infection, HSV antigen was demonstrated in the chorionic villi. Electron microscopic study confirmed the existence of viral particles in the placenta in that case and, thus, the possibility of a transplacental route of infection.

[Diagnosis of dissecting aortic aneurysm by digital subtraction angiography].

The aim of the study was to determine whether digital subtraction angiography (DSA) can be used in the diagnosis of aortic dissection. The study group consisted of six patients with aortic dissection including one of type 2 and five of type 3 by the DeBakey classification. DSA findings were compared to cineangiographic findings. The results were as follows; In all the patients, the diagnosis of aortic dissection was possible using DSA. Especially as to the entry, which was not detected by other non-invasive methods, there was an excellent correlation between cineangiograms and DSA (sensitivity 100%). Of the 20 vessels in the abdomen, which included the celiac, superior mesenteric, and right and left renal arteries, three vessels (15%) were not identified by DSA. Causes of the failure were the low spatial resolution and the artifact due to the motion of the patients and intestinal gas movement. The accuracy rate of the abdominal branch involvement was 70%. While using a prototype equipment, the preliminary study showed that DSA is a safe and very useful method not only for diagnosing aortic dissection, but for evaluation of the entry, extension of the dissection, and involvement of major branches of the abdominal aorta and for a follow-up study.
