[Early diagnosis of neoplasms in Baden-Württemberg].

In the Federal Republic of Germany an official cancer screening program was started on July 1, 1971. The following sites are included: for males rectum and prostate, for females breast, cervix uteri, corpus uteri, ovaries, rectum. Since January 1, 1975, in addition skin, male genitalia, bladder and kidneys are screened. The official reports only deal with suspects and not with confirmed cases. The cancer registry of Baden-Württemberg is particularly involved in evaluating the results of the screening program. Of all confirmed cancers of the cervix uteri more than 50 per cent are detected by screening. Non-invasive forms represent a little more than 50 per cent of the total. Approximately one case in six of cancer of the breast or cancer of the prostate is diagnosed by screening procedures. In all other sites the percentage is 5 or less. The participation in the program varies by age and sex. Half of the women in the age group from 30 to 44 participate in the program but only about 3 per cent in the group over 75. In males, the program is aimed at the higher age groups. The total expenditure of the social health insurance in 1976 was 63 billion DM, for all screening measures 403 million DM, that is 0.6 per cent of the total amount. The contribution of the program in relation to the total cancer incidence is discussed.

[Evaluation of fetal lung maturity using a modified lecithin-sphingomyelin determination and Clements' foam test].

The modified thin layer chromatographic method for the determination of the phospholipids lecithin and sphingomyelin from amniotic fluid is useful in estimating fetal pulmonary maturity. The foam test of Clements is a simple rapid method for screening of suspicious cases of pregnancies at risk and of great value as bed side test even performing by the doctor. In comparing Clements-Test with thin layer chromatographic for L/S-Ratio determination we found a good correlation of 81,8% of all cases.

Amniocentesis

[The combination of etomidate and fentanyl as a short acting and introduction anaesthesia in the dog in experimental medicine (author's transl)].

The effects of a combination of etomidate (1.0 mg/kg bw) and fetanyl (0.005 mg/kh bw) on haematology, blood gases, acid-base balance and haemodynamics were investigated in 8 mongrel dogs. The only significant change in haematology was leucopenia. Blood gas values and acid-base balance initially showed a depression of respiration and a tendency to slight metabolic acid dosis. Decreased heart rate caused diminution of cardiac output, while systemic and pulmonary resistance as well as stroke volume were not altered significantly.

[Experimental studies on the castration osteoporosis in the rat. II. Behavior of bone minerals].

In Wistar rats the bone mineral content of calcium magnesium, phosphor, sodium and potassium was investigated 10 months after complete ovarectomy. 20 of ovarectomized animals obtained in this time 0,65 mg ethinyloestradiolsulfonate/kg/month orally. The ovarectomy decreased significantly the content of calcium and magnesium of femur, humerus, vertebra thoracis and lumbalis. After therapy with the oestrogen these minerals were increased, but not significantly. These results did not correlate with the degree of histomorphometrically established osteoporosis. Phosphor, sodium and potassium were not changed significantly. The role of oestrogen dosis was discussed.

Animals

[The effect of carbenoxolone on stress ulcers in rats].

By an immobilisation with a 72 hour's interruption of intake of fluid and nutrition stress ulcers were produced in the stomach of Wistar rats. Through a sound 34 animals received daily 25 mg/kg/body weight carbenoxolon. In them ulcers were found in nearly the same number as in the 30 controls. A conspicuous accumulation of ulcers in the pre-stomach or glandular stomach was not observed. A protective effect of the carbenoxolon could not be proved.

Animals

[Abnormal myocardial blood flow pattern after non-transmural infarction (author's transl)].

While coronary angiography was within normal limits in three patients with recurrent severe attacks of angina and non-transmural postero-lateral wall myocardial infarction, myocardial perfusion scintigraphy demonstrated an abnormal distribution pattern. In addition to recanalised thrombosis, embolism or spasm, abnormalities in the microcirculation are possible causes of the discrepancy between the coronary arteriogram and the scintigram. Myocardial perfusion scintigraphy is of particular diagnostic significance in this group of patients with coronary heart disease but normal coronary angiography. Treatment in the presence of this finding consists of administration of platelet-aggregation inhibitors.

Adult

[Compromized myocardial perfusion in arrhythmias (author's transl)].

In 7 patients with arrhythmias of various origin the myocardial scintigram displayed either a diffuse or circumscript defect of the perfusion. The coronary arteriogram was normal in all patients. The localized defect of the perfusion in 2 patients was in the region of the upper part of the interventricular septum. Both had a left bundle brunch block. A correlation between the perfusion defect and the electrophysiological abnormality seems probable. The perfusion defect in one of the patients is most probably caused by a previous myocarditis followed by fibrous changes. In the other 6 patients the cause for the perfusion defect is not obvious. A history of myocarditis is missing. The presence of "small vessel disease" in those patients has however to be considered. Our results point to the relation between an abnormality of the microcirculation and arrhythmias in younger patients.

Adult

[Successful treatment of a supraventricular re-entry tachycardia in a patient with WPW and sick-sinus syndrome with permanent rapid atrial and QRS-inhibited ventricular pacemaker stimulation (author's transl)].

A supraventricular re-entry tachycardia was successfully treated in a patient with WPW and sick-sinus syndrome by permanent rapid atrial pacing and ventricular demand pacing. The use of a demand pacemaker with bipolar electrode avoided slow ventricular rates resulting from second degree A-V block during rapid atrial pacing. The trigger system of this pacemaker was not disturbed by unipolar atrial stimulation. Ventricular rates responded satisfactorily to physical and to emotional stress as a result of improved A-V conduction in these circumstances. Rapid atrial pacing has prevented the re-entry tachycardia. In an emergency the atrial pacemaker can be inhibited by a magnet, but otherwise discharges permanently at a constant rapid rate.

[Diagnosis of paranephritic abscess].

The paranephritic absecess belongs to the more infrequent clinical pictures. On the basis of a not common fistulography of the lumbar region on the right, in which we succeeded in a complete description of the cavitary system of the kidneys retrospectively the diagnostic approach in the paranephritic abscess is briefly described. Hereby, as a rule, no too extensive investigations are necessary, as far is at all thought of the possibility of a paranephritic abscess. With typical clinical findings alone - pain and exteriorly visible swelling - the diagnosis seems to be nearly ascertained. Radiologically in many cases the general radiograph of the abdomen (references to radio-positive concrements) and the intravenous pyelogramme are enough. When a fistula is present one tries to find the starting point of the process by means of a contrast remedy filling. In the majority of patients an operative treatment follows.
