[Indirect immunofluorescence in the serological diagnosis of chronic pyelonephritis (comparison with indirect haemagglutination and the Widal reaction (author's transl)].

Using indirect immunofluorescence, o-antibodies against the homologous causative organism were determined in 119 adults with chronic pyelonephritis, during a one to two-and-a-half year period. Humoral antibodies were determined in the same group of patients with the Widal reaction and indirect haemagglutination. The results were compared with those in a group of adults without renal disease, using the same serological reactions. In adults with chronic pyelonephritis the antibody titre (by immunofluorescence) was above normal limits (as obtained in the healthy group) in 63.1%. The corresponding results for the Widal reaction and indirect haemagglutination reaction were 16.5% and 12, respectively. Indirect immunofluorescence is thus clearly better than the other two serological methods in the diagnosis of chronic pyelonephritis.

[Treatment of hypertonus in diabetes mellitus].

When pathophysiological and pathogenetic aspects of hypertension are taken into consideration with special regard to diabetes mellitus the exhaustion of the "insulin enhancement" within the cerebrovisceral functional systems (Baumann) are discussed and the authors enter possible connections of diabetes mellitus to the renin-angiotensin-aldosterone system. After explanation of the diabetogenic and antidiabetogenic pharmacodynamic qualities of the antihypertensive drugs adequate therapeutic recommendations are proposed summarized in a figure. The authors conclude that for the present antihypertensive therapy in diabetics taking into consideration the references reported on there are sufficient possibilities of treatment for all degrees of severity of hypertension. Such preparations as Rausedan, Disotat, Dopegyt appear as particularly suitable; moreover, the beta receptor blockers, Haemiton, Depressan as well as Guanitil and Pargylin prove to be possible or without disadvantage, respectively. Especially when diuretics are described an exact control of the metabolism should be carried out.

[Is there a variation of death causes in diabetic patients].

On the basis of the post-mortem records of the Institute of Pathology of the University and District Hospital Rostock taking into consideration the clinical records the cause of 355 diabetics who died in the Rostock clinics from 1956 to 1972 were established and analysed. The results were compared with similar examinations of previous years, in which cases among others we established an increase of the mortality of diabetes and an increase of pyogenic infections and sclerosing vascular diseases as causes of death in diabetics. Special attention is paid to the factors of risk for the development of complications dangerous to life in diabetics and it is referred to the prophylactic and therapeutic consequences, respectively, resulting from this.

Diagnosis and therapy of primary hyperaldosteronism in general practice.

Taking into consideration an own observation of Conn's syndrome the diagnostics and therapy of the primary hyperaldosteronism is described. The recognition of Conn's syndrome is important for that account as it coincides with an operatively curable hypertension. Therefore, a diagnostic step programme is proposed. Thus, the first tentative diagnosis shall be made already by a general practitioner. Other specialised and highly specialised investigations have the task to ascertain the diagnosis and to find the right way in therapy.

[The effect of repeated spray treatment on the bacterial flora of the mouth cavity in patients with periodontal disease].

The effect of a four times repeated spray application on the clinical picture and the number of oral aerobic micro-organisms has been studied on 21 patients with periodontal disease during a stay at hospital. Comparative studies corroborated the effect of antiseptics added to the spray. The reduction of the number of micro-organisms did not continue during the four days of application so that there was a discrepancy between the clinical picture and the microbiological findings. Possible causes of this fact are indicated in the discussion.

[Clinical aspects of "Endophthalmitis phacoanaphylactica" (author's transl)].

29 cases of Endophthalmitis phacoanaphylactica were diagnosed out of a total of 1300 enucleated eyes examined histologically between 1966 and 1974. Clinically the correct diagnosis was made only once. Most frequent misdiagnoses were: phthisis, recurrent iritis, and panophthalmitis. Important factors for a correct clinical diagnosis are: 1. history of injury at least 2-3 weeks previously, 2. a cataract present at the onset of the disease, 3. an endophthalmitis, often associated with extreme i.o. hypotension. Treatment consists of an immediate and total removal of lens material, regardless of the hypotension or of inflammatory signs. The retinal architecture is usually astonishingly well preserved, even in cases with extreme destruction of the anterior segments.

[Serogrouping of Escherichia coli from urinary tract infections (author's transl)].

Serological grouping with 44 anti-O-sera was performed on 215 E. coli strains isolated from urine of patients with significant bacteriuria. 95 strains (44.2%) were typable, 34 strains (15.8%) proved to be rough strains, and 86 strains (40%) were untypbale. The O-groups 2, 6, 9 and 75 were found to be more frequent in patients with chronic pyelonephritis. Among pregnant women with asymptomatic bacteriuria a typical frequency was not observed.

[Block-excision of intraocular processes. I. Tumors of the anterior uvea (author's transl)].

Tumors of the ciliary body and peripheral choroid are removed together with the adjacent cornea, sclera, iris and retina as a single "block". A lamellar preparation of the sclera is avoided because extent of the tumorinvasion into the sclera cannot be clinically reconized. The border of the tumor is found by fiber-optic-transillumination and thus the diameter of the block-excision is determined. The resulting defect of the globe is covered by a tectonic corneal graft. From 1971-1974 12 eyes have been operated with this method: 3 malignant melanomas of the iris invading the ciliary body; 5 tumors of the ciliary body and 4 extensive malignant melanomas involving also the peripheral choroid. The diameter of the block-excision varied from 5.5 X 5.5 to 14 X 18 mm. Simultaneous cataract extraction was performed in 5 eyes. Approximately 4 hours of the ciliary-body-circumference are excised. The preliminary results are satisfactory in view of the size of the excised tumors.

[Clinical findings in phakolytic glaucoma (author's transl)].

1300 enucleated globes were examined from 1966-1974: Among these 20 eyes (1.5%) revealed a "phacolytic glaucoma" histologically. The clinical diagnosis before enucleation were "secondary glaucoma" or "absolute glaucoma" (19) and once "uveitis dolorosa". "Phakolysis" was considered in only 3 eyes. 15 cases revealed surprisingly well preserved retinal structure - indicating probable good function. The only therapy possible consists of immediate cataract extraction - inspite of high intraocular pressure. Phacolytic glaucoma must be considered in the differential diagnosis of unilateral "acute glaucoma"; also in young patients with traumatic and complicated cataract.

[Block-excision of intraocular processes. II. Epithelial ingrowth into the anterior segment of the eye (author's transl)].

Extensive posttraumatic epithelial ingrowth was completely removed by block-excision of 10 to 11 nm diameter from 3 eyes. The cyst is excised together with adjacent cornea, sclera, iris and ciliary body as a single "block". Thus direct manipulation of the delicate wall of the intact cyst is avoided, the invading epithelium is totally eradicated and a recurrence impossible. The defect of the globe extending up to 6 mm beyond the limbus is covered by a tectonic corneal graft. This procedure was derived from a clinicopathologic study of 35 epithelial ingrowths among 1300 enucleations between 1966-1974. Attempts of excision confined to cornea and iris always have been incomplete. 25 enucleated globes with epithelial ingrowth revealed an attached retina in 16 eyes indicating probably useful visual acuity in case of early complete "block-excision".

[Results of surgical treatment in dislocated lenses. II. Intracapsular lens extraction in adults (author's transl)].

40 subluxated and dislocated lenses were extracted intracapsularly between 1970 and 1974. In 6 cases there was a displacement into the vitreous; in 4 an acute secondary angle-closure glaucoma was present (3 times due to dislocation of the lens into the anterior chamber). Surgical extraction with a corneal incision was undertaken under general anaesthesia after attachment of a flieringa ring to the globe. In 16 eyes a transpupillary vitrectomy was necessary. The morphological and functional results were satisfactory: decrease in visual acuity due to surgical intervention did not occur, and in 32 eyes vision was considerably improved.
