[Generalized postoperative peritonitis: the meeting of surgery and resuscitation].

Fifteen cases of generalized post-operative peritonitis (GPP) are reported. Twelve patients underwent surgical treatment with mortality rate of 25%; three patients did not undergo surgery with mortality rate of 66,6%. Diagnostic, prognostic and therapeutic aspects of this severe post-operative complication are considered to underline the importance of a precocius diagnosis and a timely reanimatory and surgical treatment for a favourable prognosis.

Spontaneous appendicocutaneous fistula: case report and literature review.

A report is presented describing an exceptional case of spontaneous appendicocutaneous fistula (only six cases in the literature). Chronic appendiceal fistulas--that is primary communications between the appendix and an adjacent hollow viscus or the skin--occur rarely. Various types of appendicular fistula are considered and the literature is reviewed. Symptoms depend on location of the fistula and organ involved. The pathogenesis of such an unusual lesion is discussed. Appendectomy and excision or closure of the fistula is the radical therapy when the diagnosis is made.

[Right partial diaphragmatic eventration. Diagnostic and therapeutic aspects].

The Authors report a case of asymptomatic right partial diaphragmatic eventration, casually discovered and surgically repaired by imbrication of the hemidiaphragm. They confirm the almost completely asymptomatic and benign character of this disease, and discuss the difficulty of the differential diagnosis with various diseases of the right cardiophrenic angle, particularly the pericardial cysts. Among the diagnostic techniques the Authors advocate the priority of the liver scanning, the echotomography, the C.A.T., the pneumoperitoneography and, in case, the celiac axis arteriography. They consider the plastic repair of the diaphragm performed by a thoracic approach to be indispensable to the confirmation of the diagnosis and, of course, to a radical therapy.

[Mucocele of the appendix].

Reference is made to two cases of mucocele of the appendix in a brief discussion of this form, its frequency, and the difficulties associated with its preoperative diagnosis. Mention is also made of the problems arising in the differentiation of benign and malignant mucoceles. Some conclusions are drawn with regard to treatment.

[Treatment of onychophagia].

Personal experience in the hypnotic management of nailbiting is presented. Confidence in this type of treatment is rested on the results observed in a set of 32 cases, 28 of which were fully and definitively resolved. An exhaustive account is given of the subject and the method applied. Stress is laid on its technical and therapeutic advantages.

[Late complications of latero-lateral choledocho-duodenostomy. Physiopathology, diagnosis and therapy. Apropos of 6 cases].

Laterolateral choledochoduodenostomy is commonly used in the treatment of lithiasis and both benign and malignant stenosis of the main bile duct, and is regarded on all sides as safe and free from late complications. The latter, however, were noted in 6 personal cases and consisted of pain (5/6 cases), fever (4/6 cases), and dyspepsia (3/6 cases, icterus (1/6 cases). Surgery revealed recurrent or residual lithiasis (4 cases) and stenosis (4 cases). It is felt that the reflux of duodenal material into the main bile duct after creation of the anastomosis led to ascending cholangitis, stagnation and hence stenosis of the opening. Closure of the opening and extensive papilloplasty is not followed by immediate postoperative sequelae.

Sheep red blood cell membrane structure: an immunological probe.

Pronase digested sheep red blood cell stromata were employed as probe in order to investigate erythrocyte membrane arrangement by immunological way. Antiserum from rabbits immunized with erythrocyte ghost pronase residue assayed for its hemolytic activity against intact sheep red cells, showed an high titer, in a good agreement with the presence in the same residue of externally located membrane antigens. Immunoelectrophoretic analysis was performed between the solubilized residue and precipitins produced by rabbits immunized respectively with the following antigens: intact sheep red blood cells (SRBC), sheep erythrocyte stromata (SRBCS), lipid complex (LC), sphingomyelin complex (SF), stromata after phospholipase A treatment (SPLA), stromata after phospholipase C treatment (SPLC), stromata after phospholipases A and C treatment (SPLAC), and pronase treated stromata (SP). Antigen/antibody reaction with anti-SP antiserum showed an additional precipitation line: this fact is discussed in view of a possible enrichment of the fraction after pronase stromata digestion, and/or enhancement of the immunogenicity.
