[The use of endoprostheses (stents) in airway stenosis].

Six patients with obstruction of the trachea were treated with silicone rubber endothracheal stents implanted with flexible bronchoscope. In every case the stent caused significant clinical improvement of the ventilation. At the postintubation stenosis the stent can result a final recovery, at the malignant processes the implantation seems to be a new palliative method.

Accidents

Electrical potential dissipation induced by free fatty acids in pea stem mitochondria.

Linolenic, linoleic, oleic, palmitic and stearic acids (FFA) collapse the electrical potential of pea stem mitochondria in the absence or in the presence of 0.5 mM Mg2+. Higher concentrations of this cation (5 mM) lower the rate of dissipation caused by linoleic, oleic and palmitic acids, while abolishing that induced by stearic acid. Carboxyatractyloside and ADP do not reverse the FFA-induced collapse both in the presence or absence of Mg2+. EDTA, EGTA or BHT do not influence the dissipation caused by FFA that, in addition, is not linked to lipid peroxidation evaluated as malondialdehyde or conjugated diene formation. Only linolenic acid sustains a peroxidation which, however, appears to be caused by its own oxidation catalysed by lipoxygenases rather than by membrane lipoperoxidation induced by this free fatty acid. These results suggest that neither the ATP/ADP exchanger nor lipid peroxidation appear to be involved in FFA-induced uncoupling in pea stem mitochondria.

[The role of HCV in the pathogenesis of post-transfusion hepatitis].

Between January 1987 and April 1988 following 462 open heart surgery operations, 83 patients with icteric hepatitis were seen. Among them 59 patients were anti-HCV positive with the Ortho anti-HCV test, these findings suggested a hepatitis C virus infection. The source of viral infection was searched, and a prothrombin complex concentrate which had been used during that period, seemed to be the potential cause of bloodborne hepatitis C. The results suggest that special care is required when using such blood products.

Cardiac Surgical Procedures

[Bilateral neonatal adrenal hemorrhage associated with hypoadrenalism].

A newborn is described, who was born in term with 3250 g weight, after intrauterine hypoxia. She was born with vacuum extraction, and suffered from birth trauma and intrauterine infection. At age of one day manifested an eclampsia, which was hardly diminishable. The serum Na level was 118 mmol/l. Bilateral adrenal apoplexy was visible with ultrasound. The newborn was treated with infusion of sodium chloride and glucose, with antibiotic and glucocorticoid hormone. Her condition improved rapidly. Her serum cortisol level at 8 and 15 day after stopping of 12 days prednisolon therapy was below 2.5 micrograms/dl, marking hypadrenia. At age 3 months the function of adrenals was normal: the serum cortisol level was 11.4 micrograms/dl, and after ACTH stimulation it run up to 44.5 micrograms/dl. By screening of 1491 newborns, 13 adrenal apoplexy was found. The other 12 were one-sided and symptomless.

Ultrasonographic screening of neonatal adrenal apoplexy.

A total of 1785 newborns were subjected in the course of 14 months to ultrasonographic screening. Adrenal bleeding among those born via caesarean section has not been recorded. Among 1490 born via vagina there were 14 (0.94%) with adrenal apoplexy and 80 were delivered with clavicular fracture (5.37%). There were 13 with unilateral bleeding. Hormone examination applied to one infant with bilateral bleeding confirmed adrenal insufficiency which normalized after three months. In the aetiology of neonatal adrenal apoplexy the dominant causative role is attributed to birth trauma.

Adrenal Gland Diseases

[Inclusion cysts. Case report].

Inclusion cysts which are developmental anomalies can be seen either along the midpalatine raphe or on the dental ridges as well as on other areas of the palate in newborns. Based on their localisation and histology these are classified into three types (Epstein's pearls, Bohn's nodules, Dental lamina cysts). These are often taken as natal or neonatal teeth. No treatment is indicated since the lesions will spontaneously disappear few weeks after birth.

[Comparative study of the rheological properties of artificial saliva types based on hydroxy-propyl-methyl-cellulose (methocel) and carboxy-methyl-cellulose (CMC)].

Apparent viscosity at different shear rates was measured for two types of saliva substitutes based on methocel and CMC, and was compared with that of human whole saliva. Results indicate, that fresh mixed human saliva is viscoelastic, exhibiting pseudoplasticity. Methocel containing artificial saliva and human whole saliva are similar in their rheological properties, while the preparation based on CMC is almost a Newtonian liquid. From the results it can be concluded that the saliva substitute containing 0.5% of methocel appears to be a better substitute for natural saliva than of containing CMC as far as rheological properties are concerned.

Prolactin response to morphine in intact and adrenalectomized lactating rats.

To examine the hypothesis that the increased adrenocortical activity during lactation induced the loss of the prolactin (PRL) -releasing effect of morphine, we studied the effect of morphine in adrenalectomized (ADX) and sham-operated primiparous lactating Wistar rats. Animals were adrenalectomized 4 days after delivery. On day 11 of lactation (7 days after ADX), pups were separated from their mother 2 h before morphine or haloperidol injection. Intravenous injection of 5 mg/kg morphine did not change plasma PRL levels in the sham-operated lactating rats, but it resulted in a significant increase of plasma PRL levels in ADX lactating animals, with or without corticosterone replacement. Catalepsy following 10 mg/kg i.v. morphine was also markedly enhanced in ADX lactating animals. The PRL response to 0.5 mg/kg haloperidol was higher in ADX lactating animals than that in the controls. Morphine given 2 h after haloperidol treatment resulted in a further increase of plasma PRL in ADX, but not in the sham-operated lactating animals. These results suggest that adrenal hyperfunction may lead to a loss of sensitivity to morphine during lactation.

Adrenalectomy

[When is surgical correction of the terminal ileum during appendectomy indicated?].

During 1416 appendectomy operations we examined the ileum and found 42 cases of terminal ileum infraction (ileum terminale duplex) caused by the cicatrisation of the mesenterium. Nine of the forty-two had characteristic symptoms of terminal ileum infraction syndrome (stomachodynia and pain in the right lower part of the abdomen 4 or 5 hours after meals, distensions). Deliberation was performed in this cases at the time of the appendectomy operations. Following the operations they had no symptoms. When the infraction is severe or the patient has typical complaints of terminal ileum infraction syndrome necessitates the deliberation of the terminal ileum in our opinion.

Appendectomy

[Representation of the increase of fetal biparietal and thoracic diameter on a new (14-zone) ultrasonic diagnostic table].

From the somatic development of the fetus one may conclude to its functional maturity and if birth is near to its life prospects too. Thus the ultrasonographic examination of the biparietal and thorax transversal diameter reflecting the somatic development is of prognostic significance too. The authors composed a new table to increase the information value of the ultrasonographic finding, for the uniform designation of the given BPD and THD (biparietal and thorax diameter) sizes and for the better recognition of the tendencies of BPD and THD changes. A new system of symbols and representation method were elaborated for the description of standard positions. According to the gestation weeks 14-14 BPD and THD size-ranges were separated. The so called "UFD" Ultrasonographic index of the Fetal Development gives information besides the gestation time also on the BPD and THD standard position of the fetus and demonstrates illustratively the direction and rate of deviation of the standard positions from the average. The system provides the possibility of a better information to the obstetrician or consulting physician who evaluates the US finding, a uniform interpretation of the findings and serves better the clinical diagnostics.

[Hepatitis C virus antibody in Hungarian blood donors, persons at risk and patients with post-transfusion hepatitis].

The authors screened blood donors, persons at high risk of infection and patients with post-transfusion hepatitis by the use of the new hepatitis C antibody test. 1.7% of persons donating blood acceptable for use according to current criteria were found positive. This ratio was markedly higher among donors displaying an elevated transaminase level. Among the various patients tested 85% of the haemophiliacs, 14% of the transfused haematology patients, and 40% of those on dialysis were found positive. In two groups of patients with post-transfusion non-A, non-B hepatitis the ratio of positives was 69% and 92%, respectively. The frequency of hepatitis C antibody was not sizably higher in the health personnel tested than that found in blood donors. The results indicate hepatitis C virus to be the main source of post-transfusion non-A, non-B hepatitis in Hungary too. As expected, HBc antibody revealing a previous contact with hepatitis B virus was found very frequent in patients with coagulopathies and in those on dialysis. A relatively high anti-HBc prevalence was found among health personnel and blood donors too. Authors intend to emphasize the need of thorough consideration of indications in hemotherapy although improvements in the screening of donations can diminish the risk of virus transmission by blood or blood derivatives.

[First isolation of Borrelia burgdorferi (Lyme spirochaeta) from ticks in Hungary].

31 field collected Ixodes ricinus adult ticks were investigated for the Lyme-disease spirochete. 5/31 Ixodes ricinus contained Borrelia burgdorferi. The spirochete was successfully cultivated in four cases, and they were found in two ticks by immunofluorescence technique and dark field microscopy, as well. Two of the isolated strains were tested by Western blot. The antigen pattern in both strains showed marked bands in 41 and 60 kD antigen, but only a week band appeared at 32 kD, with the lack of the OspB. The mechanism of infection and the way of prevention is discussed.

Animals

Macrophage containing factor XIII subunit a in salivary glands of patients with Sjögren's syndrome.

Minor labial salivary glands obtained at biopsy from patients with Sjögren's syndrome were investigated by immunomorphological methods for the presence of monocyte-derived macrophages. According to our observations published earlier the immunomorphological detection of factor XIII subunit a is a useful marker for recognizing cells of monocyte/macrophage lineage. Factor XIII subunit a was detected by a highly sensitive immunoperoxidase staining, and cells containing this coagulation enzyme were characterized by double immunofluorescence stainings. Factor XIII subunit a+ cells were found to be highly accumulated at the interface of normal tissue and peritubular infiltrate. In double immunofluorescence labelling systems factor XIII subunit a+ cells were simultaneously labelled by RFD7 and Dako-antimacrophage monoclonal antibodies. They also expressed HLA-DR antigen as revealed by a reaction with RFDR2 monoclonal antibody. The results suggest that monocyte-derived tissue macrophages are present in salivary glands of patients with Sjögren's syndrome and have a characteristic distribution. It can be assumed that they have a role in the demarcation of peritubular inflammation and thus have an effect on the progression of the disease.

[Incidence and role of hypertension in the development and recurrence of acute myocardial infarct in a 5-year case load].

Authors treated and checked 474 patients with acute myocardial infarction (AMI) during 5 years. The occurrence of hypertension was 37.15% of the total number of patients but this rate was 51.37% for women patients. Obesity, hyperlipidaemia and diabetes mellitus were the most frequent of the risk factors on the patients with hypertension. Asthma cardiale and acute arrhythmias were slightly more frequent early complications and cardial decompensation was a slightly more frequent late complication. The number of myocardial reinfarctions was the same both in the case of hypertensive and normotensive patients under the circumstances of parallel normalization and keeping the tension and serum cholesterol on the normal level.

[Genetic and evolutionary analysis of cases of Darier's disease].

In the past 10 years the authors have observed 27 people (13 men and 14 women) suffering from Darier's disease in Somogy county. Out of these 22 cases were familiar and 5 isolated. The praevalence of dysceratosis follicularis vegetans was found 1 to 16,000 in the area. In the 3 familiar cases the dysceratotic papules of Darier's disease followed the naevus-lines, drawing out the Arnosan-triangle in the meantime. In the case of one of the nonfamiliar occurrences the Darier's diseased papules running stripelike also wrote out the naevus-lines, strictly localized to one side of the body. The stripes composed by the dysceratotic papules ran bipolarly in these cases too; heading from a point of the lumbosacral part of the median line of the back towards the navel. According to the authors the direction of the development of the epidermal elements of the skin is expressed in the early phase of embryonic life by the dysceratotic papules running bipolarly and following naevus-lines as embryological rudiments.

[Correlation of the length of the preoperative period and the severity of appendicitis].

1364 appendectomy were performed by the authors in two hospitals (1976 to 1982 in City Hospital Nagyatád and 1983 to 1988 in Hungarian State Railways Hospital Szolnok). Patients were divided into groups according to degree of inflammation of appendix. Length of the elapsed time from the appearance of first symptoms to operation and degree of inflammation of the appendix were fixed. It is found that time-factor did not appear to be the most important determinant of degree of inflammation in a certain part of cases.
