Possible indications for the use of biodegradable implants are presented and the chemical compounds in question are briefly described. The results of a laboratory investigation to determine mechanical stability, tensile strength and molecular weight are reported, in order to characterize the polymers and co-polymers of the lactide under study.
Arthroscopy of the wrist is a helpful diagnostic procedure. The technique of the arthroscopy of the wrist is described. Initial experience in 22 diagnostic arthroscopies of the wrist is recounted. In six cases operative procedures followed the diagnostic arthroscopy.
The results of ankle joint arthroplasty are disencouraging. Replacement is only justified, when there is no infect or instability and axial deviations below 20%. After fusion in 80% release of pain, more than 50% unlimited walking and only 10% the severe walking restriction is seen. Therefore this treatment can be recommended in posttraumatic tarsal osteoarthritis.

The use of plastic boxes in functional treatment of bone fractures has satisfactorily helped to speed up non-invasive reintegration of patients to their normal environment. The time of treatment can thus be considerably reduced. Correct indication, short-interval follow-up checks in the initial phase, and the patient's cooperative attitude are essential to good success. Brace treatment of upper arm fractures, ulnar fractures, and closed tibial fractures have been three of many indications suggested by Sarmiento which have proved to yield good results at "Bergmannsheil Bochum".

Undesirable sequelae of spinal column injuries are chronic instabilities and decompensated axis shifts. If a poor functional result can be expected after the initiation of conservative treatment, indication for surgery should be considered as early as possible. The authors describe a surgical technique which can be employed both in emergencies and electively on account of its standardization; it consists of dorsal repositioning manoeuvres that result in short-distance fusioning of the spinal column to achieve stable functioning. However, repositioning via the dorsal approach can be effected with maximum success only if it is done within the first 10 to 14 days.
Of 91 thumb amputation injuries which were treated over a five-year period at the Department of Traumatology, University School of Surgery Homburg/Saar, 56% could be subjected to a critical follow-up evaluation. The evaluation scheme of Millesi "Evaluation and Registration of Hand Function" programmed for electronic data processing forms the basis for the study. Besides the specified criteria for sensitivity testing, a further method for objective testing is presented. The principle of this method is point stimulation of pressure and mechanoreceptors after eliciting a threshold perceptible vibration stimulus of an 80 Hz sinusoidal oscillation the intensity of which can be directly measured as a yardstick for the stimulus response. Electronic processing of all results of the patients investigated shows that the injured hand can be used to almost 100% in 38%. In 16 patients (31%), there is a reduction by 5 to 20%. In ten cases (20%), the reduction was rated as over 20% compared to a healthy hand and in 11% (six patients), the overall result is to be designated as poor with more than 40% reduction of use.
Conventional treatment of pyogenic knee joint infections leads to unsatisfactory results. Through early synovectomy, before cartilage damage and osteoarthritis appear, the infected focus can be "excised." Functional after-treatment avoids cartilage atrophy, wound adhesions, and muscle weakness. The excellent results after 26 knee joint infections confirm this.
Spinal injuries must be divided into stable and unstable lesions. Stable fractures are treated functionally, in unstable cases a decision must be made whether conventional treatment will be satisfactory. Severe kyphosis or chronic instability must be avoided. Open reduction and spinal fusion are recommended very early, if conventional therapy is not successful. Surgical risks are minimized by technical standards.
This study reports on the after-examination of 86 out of 107 patients with primary shoulder dislocation. Of greatest interest was the question which period of immobilisation would yield the best results. The most frequent type of dislocation was the subcoracoid position in 62% of all cases. In 44% the dislocations were associated with fractures especially of the greater tuberosity. The greatest number of dislocations occurred between 61 and 80 years of age (= 42%). The second highest number was found between 21 and 40 years of age (= 29%). In 47% of all cases the shoulder was immobilised for a week, in 40% between one and two weeks and in 13% for more than two weeks. The incidence of recurrence was 15% (13 patients). In 8 cases (= 9%) a recurrent dislocation of the shoulder resulted, whereas in 5 cases (= 6%) only one relapse occurred due to a repeated intense trauma. The function of the injured shoulder is most significantly impaired and rendered worse by the factors: higher age (greater than 41 years) and associated fractures. The duration of immobilisation itself has neither a significant influence on posttraumatic function of the shoulder nor on the incidence of recurrence. Therefore, an uncomplicated dislocation of the shoulder should be immobilised for a short period only. However, in cases of associated fractures, immobilisation will be necessary for a longer time.
Primary internal fixation of acute humeral shaft fractures is only seldom indicated. Following the late results, indication and technique must be very strict. This is shown by the number of 37 cases in seven years and the very low complication rate.
