[Arthroscopic abrasion arthroplasty of the upper ankle joint. Long-term results in osteochondrosis dissecans].

From January 1984 to June 1987, a total of 27 patients received arthroscopic treatment to the ankle to remove cartilage fragments and curet the osteochondral bed, and control arthroscopy was possible after 4 months in 4 cases. Particular attention is paid in this report to 11 patients with a total of 13 lesions of the talar dome (stage III/IV according to Brandt/Harty), with discussion of the etiology, the treatment and the long-term postoperative results. On follow-up at 2 years, the results were assessed both subjectively and objectively as good in 7 and fair in 4 of these patients. The advantages of arthroscopic treatment over open surgery and arthrotomy are less severe traumatism, a lower complication rate and faster rehabilitation.

[Infection of the knee joint--an arthroscopic challenge?].

Unlike previous forms of surgical treatment, operative arthroscopy of the septic knee can prevent in most cases severe functional deficit or even ankylosis provided that arthroscopy is done within 1 week after the onset of the infection and that consistent and continuous passive motion is then carried out. In all other cases, open subtotal synovectomy is preferable to closed-irrigation suction drainage. In three-quarters of the cases presented, the functional results can be considered good. Reoperations have only been necessary when arthroscopy has been delayed by more than 1 week.

["Prognostic" analysis of late results of trochanteric femoral osteotomy following post-traumatic malalignment].

Post-traumatic axial malalignment of the lower limbs causes osteoarthrosis of the adjacent joints. Whether or not correction osteotomy is indicated depends upon the degree and the location of the deformity. Malalignment of the proximal femur should be corrected if the axis of the leg does not pass through the knee joint. If the axis is outside the central half of the tibial head, the indication for correction osteotomy is a relative one: varus deformities are more difficult to compensate and it is more urgent that these be corrected. Of 103 patients who underwent osteotomies of the proximal femur during the period 1973-1981, there were 73 in whom it was possible to follow up an average of 7 years after the realignment procedure. In nearly two-thirds of these patients the clinical symptoms noted before surgery had improved. In 10 cases of pseudarthrosis of the femoral neck, necrosis of the femoral head could not be avoided in spite of the surgical correction; nonetheless, the prognosis is better if the correction is done within the first 3 months after the trauma.

[Dislocation and subluxation of the patella].

Subluxations or dislocations are present in adolescents and young adults, they are caused by malalignments of the leg (rotation, axis, extensor mechanism). In subluxation, functional therapy is the method of choice, dislocations with haemarthrosis are an indication for arthroscopy. After the first dislocation without chondral lesion, consequent functional treatment has a recurrence rate of only 5 per cent. Recurrent dislocations need a reconstruction of the extensor mechanism to prevent chondral damage.

[Fixation or screws in arthrodeses of the upper ankle joint. A retrospective comparison of 76 patients].

In a retrospective study, the results of 76 patients treated by different methods for arthrodesis of the ankle joint were compared. With external-compression clamps, solid fusion was achieved in 48 patients after an average of 4.5 months, with lag-screw fixation in 25 patients after 3 months. The nonunion rate was only 4% in the group with external clamps, the followup time was 2 years (maximum 6 years, minimum 6 months). Eighty percent of the patients reported pain relief, 58% in the lag-screw reported being free of symptoms, 23% as did of the external-clamps group. Most of the complications after external fixation were related to pin-site infections (48%). Infection, instability and inability to undergo physiotherapy were the main problems after clamps. Arthrodesis with lag screws provides optimal biomechanical fixation, with early functional treatment and a fusion within 3 months. Compared to external clamps arthrodesis, the lag-screw group had fewer problems with skin and bone healing and could be discharged earlier.

[Postoperative classification of traumatic soft tissue damage as an aid to treatment. A simple system exemplified by the lower leg].

The prognosis of fracture healing depends upon the severity of injury sustained to the soft tissues and the blood supply left to the bone. The soft tissue injury cannot be precisely graded until after the fracture has been stabilized and radical debridement performed. Subsequent steps in the therapeutic strategy of compound fractures are determined mainly by the type and severity of soft tissue injury. Therefore fractures with soft tissue lesions need to be classified again after bone fixation and wound revision. Later treatment steps for restoration of the soft tissues are easily derived from the classification.

Early recognition of postoperative hematoma formation.

Postoperative hematoma formation must be treated as a potential infection. One fourth of all postoperative hematomas are already contaminated. Ultrasonographical examination is an effective method for early recognition of such postoperative hematomas. Ultrasonic diagnosis on a routine basis is not necessary, but it should be carried out as soon as clinical symptoms appear. Our postoperative late results with only one early and one late infection after 100 postoperative treatments of the hip joint and femoral shaft emphasize the importance of early diagnosis of hematomas.

Eighteen-year anamnesis of osteoid osteoma--a diagnostic problem?

The 18-year history of osteoid osteoma in a woman aged 43 is presented. The typical physical and radiographic findings are described. Total removal of the tumor resulted in prompt and permanent relief of symptoms. Diagnosis, histopathology, differential diagnosis, treatment, and prognosis of the tumor are discussed.

[Post-traumatic pain syndromes of the shoulder joint--differentiation and treatment].

The main symptom of posttraumatic trouble in the shoulder-joint is the painful immobility. We had to differentiate between genuine shoulder pain originating in shoulder alternation and shoulder pain in consequence of other diseases. Significant causes of posttraumatic shoulder pain are the impingement syndrome with tendon alteration or lesion or, due to fractures, the instable shoulder joint as well as shoulder rigidity. An exact diagnosis and, in consequence an exact treatment can only be accomplished basing on accurate history and specific clinical examination.
