[Congenital eccrine angiomatous hamartoma].

BACKGROUND: Eccrine angiomatous hamartoma is an uncommon skin disease with vascular and sudoral components: less than thirty cases have been reported in the literature. CASE REPORT: A 3.5 month-old female infant developed a painful angiomatous plaque on the abdomen which was first seen 15 days after birth. Histology showed numerous vessels and eccrine sudoral glands in the dermis. Exeresis could not be complete and was followed for 15 years. There has been a slight hyperpigmentation with localized hyperhidrosis over the plaque. This hypersudation confirms the diagnosis which could not be confirmed on the initial biopsies. DISCUSSION: This case of congenital eccrine angiomatous hamartoma could only be diagnosed late in the clinical course. This is the first case report on the abdomen, 80 p. 100 of the previous reports were on the limbs. After a 15 year follow-up, there was no progression of the hamartoma and pain regressed gradually. Without secondary development of localized hyperhidrosis, the diagnosis could not have been made. Careful comparison of pathology findings and clinical expression are essential for the diagnosis of eccrine angiomatous hamartoma.

Radiation dose to the lenses in CT of the paranasal sinuses.

The radiation dose to the lens during CT of the paranasal sinuses was measured in 20 patients. In 10 patients, a "standard" technique with axial and coronal sections was used. In the remaining 10 patients, overlapping axial sections for bidimensional reconstruction were obtained. Radiation dose was measured using thermoluminescent dosimeters. The mean dose was 22 mGy in patients who underwent "standard" CT and 42 mGy in those who underwent CT with overlapping sections. Dose was dependent on the total number of slices and of transorbital axial sections. Coronal sections did not statistically increase the radiation dose to the lenses. Although these doses were significantly below the critical dose for cataracts, CT with overlapping sections gives a considerably higher radiation dose. We recommend the bidimensional reconstruction technique only for selected cases (e.g. preoperative assessment of the roof of the ethmoid sinuses or in patients who can or should not maintain the hyperextended position required for coronal views.

Epidermal hamartoma presenting as longitudinal pachyleukonychia: a new nail genodermatosis.

BACKGROUND: Epidermal hamartomas involving the nail apparatus are uncommon. OBJECTIVE: Our purpose was to define the clinical, genetic, and histologic features of a unique nail abnormality. METHODS: We examined eight members of three generations who presented with an unusual nail abnormality, longitudinal pachyleukonychia, as a sole characteristic that appeared between the ages of 9 and 30 years. One nail was examined histologically. RESULTS: Histologic examination showed pronounced hyperplasia of the nail bed with papillomatosis. Cell nests of a sebaceous type were included in the epithelial digitations. This dysplasia was not accompanied by abnormalities affecting skin, hair, teeth, or sweat glands. CONCLUSION: This unique epidermal hamartoma appears to be inherited as an autosomal dominant condition.

Iobitridol 250 vs iohexol 240 in phlebography of th lower limbs. A double-blind clinical trial.

Iobitridol 250 (Xenetix 250) was compared with iohexol 240 (Omnipaque 240) in phlebography of the lower limbs by means of a multicenter, randomized double-blind trial in 70 patients. No significant difference was demonstrated between the 2 groups in terms of diagnostic efficacy, image quality or clinical safety. Only minor reactions were recorded in the 2 groups, essentially consisting of mild or moderate heat sensations. The only insufficiently informative examinations were not due to the contrast agent. Iobitridol 250 therefore appears to be a well-tolerated contrast medium, suitable for use in phlebography of the lower limbs.

[Treatment by thalidomide of chronic multiforme erythema: its recurrent and continuous variants. A retrospective study of 26 patients].

INTRODUCTION: The objective of this study was to evaluate the efficacy of thalidomide in the treatment of chronic erythema multiforme unresponsive to usual treatments. PATIENTS AND METHODS: Twenty-six patients with chronic erythema multiforme were given thalidomide 100 mg/day after other treatments had failed, particularly acyclovir and prednisone. RESULTS: Twenty patients had recurrent erythema multiforme and were given thalidomide at the beginning of an episode. The duration of the episodes was reduced by 11 days on the average. Six of the patients had subintrant erythema multiforme and were given continuous treatment. Lesions disappeared within 5 to 8 days and remission was maintained with low dose-treatment. DISCUSSION: The spectacular results obtained here should be verified in a controlled study.

[Localized bilateral and symmetrical neurofibromatosis].

INTRODUCTION: We report the first case of bilateral neurofibromatosis localized distally on the lower limbs. CASE REPORT: A 36-year-old woman presented about 20 nodular lesions of the plantar and both lateral aspects of both feet which developed progressively after age 30. Histology examination evidenced neurofibromas. Clinical features and laboratory results enabled us to eliminate von Recklinghausen disease in this mother of 3 children. DISCUSSION: This observation allowed us to situate symmetrical and bilateral neurofibromatosis in the Riccardi classification for genetic counselling. We conclude that the case was a type V neurofibromatosis in a sporadic bilateral form, rarely reported in the literature.

Glucose metabolism, enzymic analysis and product formation in chemostat culture of Hanseniaspora uvarum.

The physiology of Hanseniaspora uvarum K5 was studied in glucose-limited chemostat cultures and upon glucose pulse. Up to a dilution rate of 0.28 h-1, glucose was completely metabolized in biomass and CO2. Above this value, increase in the dilution rate was accompanied by sequential production of metabolites (glycerol, acetate and ethanol) and decrease in cell yield. Similar results were observed upon glucose pulse. From the enzyme activities (pyruvate dehydrogenase, pyruvate decarboxylase, NAD and NADP-dependent acetaldehyde dehydrogenases, acetyl coenzyme A synthetase and alcohol dehydrogenase) and substrate affinities, the following conclusions were drawn with respect to product formation of cells: (1) pyruvate was preferentially metabolized via pyruvate dehydrogenase, when biomass and CO2 were the only products formed; (2) acetaldehyde formed by pyruvate decarboxylase was preferentially oxidized in acetate by NADP-dependent aldehyde dehydrogenase; acetate accumulation results from insufficient activity of acetyl-CoA synthetase required for the complete oxidation of acetate; (3) acetaldehyde was oxidized in ethanol by alcohol dehydrogenase, in addition to acetate production.

Role of magnetic resonance imaging in predicting late facial motor function after removal of vestibular schwannomas by the translabyrinthine approach.

Facial motor function was assessed in 61 patients 18 months after surgical resection, by the translabyrinthine approach, of vestibular schwannoma involving both the cerebellopontine angle (CPA) and the auditory canal. Pre-operative magnetic resonance imaging (MRI) was performed to measure the maximum extracanalicular diameter of the tumour between the porus and the farthest extension in the CPA on transverse slices and to calculate extracanalicular tumour volume. Post-operative facial motor function was graded according to the House and Brackmann system. There was a statistically significant relationship between late facial motor function and extracanalicular diameter. The best cut-off point for good and poor results was 20mm. There was no relationship between the tumour volume and the late post-operative facial motor function grade. In this study the best pre-operative radiological predictor of the late facial motor function in patients operated on by the translabyrinthine approach was the maximum diameter measured by MRI.

Type A dissection of the thoracic aorta: use of MR imaging for long-term follow-up.

PURPOSE: To evaluate routine magnetic resonance (MR) imaging for long-term follow-up in patients who undergo surgery for type A aortic dissection. MATERIALS AND METHODS: Ninety-two MR examinations were performed in 36 patients. Standard spin-echo images were obtained with electrocardiographic gating (n = 92) and rapid images with a fast low-angle shot sequence and intravenous administration of gadopentetate dimeglumine (n = 25). All segments of the native thoracic aorta were evaluated. Anastomoses of the prosthesis and periprosthetic hematoma were carefully analyzed. RESULTS: Of 22 complications that occurred in 18 patients, 18 were diagnosed at MR imaging (nine false aneurysms and nine aneurysms distal to the graft). Nine patients underwent reoperation. The findings at MR imaging correlated with those at surgery. CONCLUSION: Routine follow-up with MR imaging should improve long-term survival in patients who survive emergency surgical repair of type A aortic dissection.

[Pseudotumoral rectal endometriosis].

A case of rectal endometriosis, presenting in the form of acute obstruction, is described. The primary diagnosis was malignancy, but endometriosis was discovered at emergency operation. The diagnosis and management of rectal endometriosis are reviewed.

Crossover study of thalidomide vs placebo in Jessner's lymphocytic infiltration of the skin.

BACKGROUND AND DESIGN: An effective therapy is still unavailable for Jessner-Kanof lymphocytic infiltration of the skin. Thalidomide's efficacy was suggested in an open study. Twenty-eight patients were randomly assigned to receive thalidomide (100 mg/d) or placebo over a period of 2 months and were then switched to the other treatment. RESULTS: After the first period, 11 of 13 patients treated with thalidomide were in complete remission (CR), and there were two failures. There was no CR in the patients who received placebo (chi y2 = 17.5; P < .0001). After the second period, nine of 14 patients who had received thalidomide were in CR. Eleven of the 13 patients who had received thalidomide during the first period were given placebo (two were unavailable for follow-up). Ten of them were in CR: four were still free of lesions at the end of the second period, and six experienced a relapse of their lesions after a mean duration of 26 +/- 10 (SD) days. A total of 25 patients participated in the two study periods; CR was observed in 19 (76%) after thalidomide therapy and in four (16%) after treatment with placebo (chi y2 = 11.1; P < .001). Of 27 patients who received thalidomide, 16 (59%) were in CR after 1 month and 20 (74%) were in CR after 2 months. Two patients treated with thalidomide experienced neurologic changes that were not consistent with typical thalidomide-induced neuropathy. CONCLUSIONS: A therapeutic regimen of thalidomide administered at a dosage of 100 mg/d for 2 months is able to suppress the clinical symptoms of Jessner-Kanof lymphocytic infiltration of the skin. The long-term risk-benefit has still to be evaluated.

[Disseminated cutaneous neurilemmomatosis].

INTRODUCTION: Neurilemmomatosis is a rare, non-inherited disease which has several features similar to type I neurofibromatosis. CASE REPORT: A 30-year-old man had more than 300 cutaneomucosal tumours. Most were sessile and of firm consistency on normal coloured skin. Histology reported masses of fusiform dermal cells S100 protein (+), NSE (-), actin (-). The cells had a polygonal ultrastructure with fine ramifications. Neurilemmomatosis (schwannomatosis) was diagnosed. No neurological or systemic disorder could be detected. The most bothersome tumors were destroyed. DISCUSSION: This is a typical anatomoclinical presentation of cutaneous neurilemmomatosis. This exceptional state raises the problem of differential diagnosis and nosological distinction from von Recklinhausen's disease. A characteristic feature in this case was the total absence of any malformation or associated tumour, notably neurological tumours. To our knowledge, this is the only documented case of disseminated neurilemmomatosis strictly limited to the skin.

Handling information and knowledge in the FORUM hypermedia authoring system: application to uterine magnetic resonance imaging.

Hypertext and hypermedia are used to access loosely structured information. The lack of conceptual model for hypertext application makes it difficult to represent the mental model of the author. Users are often lost in the information and the objective of the author is not achieved. In this paper, we describe an approach to handle information and knowledge in the FORUM hypermedia authoring system. It allows the author to create and maintain the hypertext more easily and facilitates the navigation of the reader. An application for learning uterine magnetic resonance imaging is also presented.

[Perforating verruciform collagenoma, an exogenous inclusion skin disease? Apropos of a case induced by calcium chloride].

INTRODUCTION: Perforating verruciform collagenoma, first described in 1963, designates papular and keratotic post-trauma lesions which show an aspect of epidermal perforation microscopically with large fragments of collagen issuing through fistulous chimneys, more or less pycnotic polynuclears and squamating or necrotic epidermal cells. Three publications have presented this diagnosis. CASE REPORT: We observed a patient with multiple lesions with macroscopic and microscopic presentations suggestive of perforating verruciform collagenoma. The lesions appeared after scratches and inoculation with calcium chloride. DISCUSSION: This case is similar to those observed after intradermal inoculation of calcium salts. In the 4 previous cases, perforating verruciform collagenoma was associated with trauma allowing the intradermal penetration of a particular foreign material (glass wool, vegetable debris, drugs or intravenous injections, wound caused by a metallic garbage bin). The exceptional nature of perforating verruciform collagenoma in highly frequent skin wounds would suggest that a very particular post-trauma process is occurring caused by the introduction of an exogenous substance within the derma: calcium chloride in our case, a non-specific material in the four previous cases.

[Intra-sphenoidal prolapse of the internal carotid artery. Computed tomography of 300 sinuses].

OBJECTIVES: Endonasal sphenoidotomy is a high risk operation: fatal complications due to direct lesion to the internal carotid artery have been reported. We conducted a prospective analysis of carotid prolapse within the sphenoid cavity in order to draw attention to this dangerous anatomic variant. METHODS: Tomodensitometry of the sinuses was performed in 150 patients (mean age 35 years; range 20-70) with inflammatory nasal symptoms of less than one year duration (mean 5.2 months). Cases with benign or malignant tumours, polyposis or a past history of sinus trauma were excluded. Transversal sections were analyzed by one radiologist and one ear-nose-throat specialist. The diagnosis of carotid prolapse was defined as an intrasphenoid protrusion of the artery at least as long as one-third of its circumference. Results of the analysis were expressed as percent for 300 sinuses. RESULTS: Prolapse of the internal carotid was found in 20.3% of the sinuses (n = 61). In all cases a thin (> 1 mm) bony structure surrounded the artery. No prolapse was observed into the posterior ethmoid cells. CONCLUSIONS: Intrasphenoid prolapse of the internal carotid artery is a major risk factor in nasal and sinus surgery. The high incidence (1 out of 5 sinuses) indicates the need for routine preoperative imagery to reduce surgical risk.

Physiological approach to heterologous human serum albumin production by Kluyveromyces lactis in chemostat culture.

Production of recombinant human serum albumin (rHSA) controlled by the constitutive promoter phosphoglycerate kinase was studied in Kluyveromyces lactis. It was governed by both cell concentration and glycolytic flow. The triggering of the fermentation metabolism by unfavourable culture conditions (pH, pO2, D) caused a decrease in the synthesis of the heterologous protein. The highest productivity (75 mg l-1 per h) and rHSA concentration (62 mg l-1) were obtained in chemostat culture with a dilution rate of 0.12 h-1 and with 38 g l-1 dry weight.

Development of high-cell-density fermentation for heterologous interleukin 1 beta production in Kluyveromyces lactis controlled by the PHO5 promoter.

The use of a phosphate organic form taken up by Kluyveromyces lactis removes repression of the PHO5 promoter and releases heterologous interleukin 1 beta synthesis while providing sufficient phosphate for growth. The oxidative metabolism of high-cell-density fed-batch and chemostat cultures was thus maintained under derepressed protein synthesis conditions. Interleukin 1 beta production was then growth-associated, an unusual mode of protein synthesis regulation under the control of the PHO5 promoter.
