Transient symptomatic cryoglobulinemia in gram-negative bacteria infections.

Mixed polyclonal cryoglobulinemia was observed in seven patients suffering from severe gram-negative bacterial infections and/or septicaemias and presenting with arthralgia and purpuric manifestations on admission. Cryoglobulins disappeared after recovery from infection in all of them and were never found during a longterm follow-up. In our opinion, gram-negative bacteria may induce the synthesis of cryoglobulins via a non-specific T-independent B-cell stimulation triggered by cell-wall lipopolysaccharides. This kind of infections, especially if chronic or relapsing, might play a role in the aetiology of some so-called essential mixed cryoglobulinemias.

[2 cases of annular pancreas].

The AA. happening to observe two cases of annular pancreas, took the opportunity to study its etiopathogenetic, anatomo-clinical, diagnostic and therapeutic features. In particular, even considering the validity of the recently proposed duodeno-plastic modelling approach, they affirm a more valid technic, the latero-lateral duodeno-duodeno anastomosis surmounting the annular pancreas when one is not dealing with pre-stenotic duodenal dilations of elevated degree.

[Therapy of multiple myeloma. Study of hospital patients: 81 cases in 16 years' observation].

The results achieved with different approaches of chemotherapy protocols applied to 81 patients affected by multiple mieloma and followed up for a duration of 16 years in the geographical area pertaining to the Saronno County Hospital are presented. All the patients are divided into sub-groups, according to the type of treatment they required while being monitored over the years. Each single sub-group is evaluated according to the following criteria: objective response to therapy; median survival rates; toxic effects due to drug exposure. The results are analyzed with reference to the most relevant literature on the matter. A comprehensive retrospective review of emerging data suggests an overlapping median survival--30 and 31 months--in patients given monochemotherapy as such as in those given polychemotherapy. Evidence is also made for a clearly meaningful increase of survival in treated patients compared with that of untreated ones. Median survival depends significantly on the initial stage of the disease. Both therapeutical effectiveness and toxicity are shown to be higher, in accordance with the current literature, in polychemotherapy which include vincristine--especially when a large tumor cell mass is pointed out--compared with therapy based on alkilating agents and prednisone.

Secondary and essential cryoglobulinemias. Frequency, nosological classification, and long-term follow-up.

The clinical and immunochemical classification of 166 patients with cryoglobulinemia are presented. 52% of the cryoglobulins were detected in patients hospitalized because of various causes. The most frequent association was with liver and lymphoproliferative diseases (in particular with Waldenström's macroglobulinemia); two thirds of the patients with liver diseases presented with a purpura-arthralgia syndrome, and the hepatic involvement was only brought to evidence by subsequent laboratory investigation. The presence of cryoglobulins did not usually interfere with the clinical course of the basic condition. In 79 patients the cryoglobulinemia was considered idiopathic. In order to study the natural course of the disease in the idiopathic cases, a group of 35 patients had been followed up for 8-17 years: 37% developed a membranous proliferative glomerulonephritis, and more than half died of renal insufficiency; in 11% cirrhosis was diagnosed after a period of 4-9 years; another 11% developed a lymphoproliferative disease; of the remaining 40%, 2 are still asymptomatic, and the others are affected by purpura-arthralgia syndrome.

Cephalosporins. VI. Synthesis and structure-activity relationships of new 7 beta-[(Z)-2-alkoxyimino-2-(2-aminothiazol-4-yl)acetamido]cephalosporins with a tetraxolopyridazine at the 3-position.

The synthesis and in vitro activity of 7 beta-[(Z)-2-alkoxyimino- 2-(2-aminothiazol-4-yl) acetamido]cephalosporins with a tetrazolo[1,5-b]pyridazine at the 3-position are described. These cephalosporins showed excellent activity against Gram-negative bacteria, including beta-lactamase producing strains. The most interesting compound of the series was 7 beta-[(Z)-2-(2-aminothiazol-4-yl)-2-methoxyimino acetamido] -3-(8-carboxytetrazolo[1,5-b]pyridazin-6-yl)- thiomethyl-3-cephem-4-carboxylic acid (9, FCE 20485) because of its extraordinarily long half-life and marked in vivo activity.

[Passage from the thigh to the retroperitoneum through the obturator foramen of an enormous recurrent lipoma].

The authors refer to an enormous lipoma relapsing from the thigh and the retro-peritoneum space which they have observed. Underlying the rarity of the case, they reveal their reasons whereby one must consider the origin of this lipoma the medial region of the thigh, and the foramen obturatorium the passage-way of the same lipoma from the thigh to the retroperitoneum. Furthermore considering the frequent impossibility of complete removal of these extremely developed lipomas rendering a reserved quod vitam prognosis.

[Short-term effects of cimetidine in patients with bleeding duodenal ulcer. Presentation of cases].

The authors here present a work dealing pith two groups of subjects, both groups having the diagnose of bleeding duodenal ulcer; one group using therapy antacid alone, the other using cimetidine and antacid. This last is more affected in controlling bleeding then just using antacids alone. Moreover the use of cimetidine reduces the period of bleeding, increases the number of subject who stop bleeding and reduces the amount of blood transfused in these subjects. We are also convenced that the use of cimetidine even prolong in time, does not protect these subjects from having other relapse. Therefore propose the Billroth II and from own experience this is a good therapy to prevent other bleeding.

[Apropos of a case of idiopathic cryoagglutininemia with cryoglobulinemia and bone marrow lymphoplasmacytoid infiltration].

A case of idiopathic cryoagglutininaemia, with cryoglobulinaemia and lymphoplasmacytoid infiltration of the bone marrow is reported. The case is not held to be an independent clinical entity but an aspect of immunoproliferative disease, similar to Waldenstrom's disease. An association of steroids in high doses and cyclophosphamide led to a definite improvement in the clinical and haematological picture and this has continued for the past 4 years with little or no treatment.

Cephalosporins. III. Synthesis and structure-activity relationships of 7-vinylenethioacetamid:o cephalosporins with a tetrazolo-pyridazine at the 3-position.

The synthesis and in vitro activity of 7-vinylenethioacetamido cephalosporins with a tetrazolo-pyridazine at the 3-position are described. These cephalosporins showed good activity against Gram-positive and Gram-negative bacteria. 7-[(Z)-beta-carboxyvinylenethio-acetamido]-3-[(tetrazolo[1,5-b]pyridazin-8- amino-6-yl)-thiomethyl]-3-cephem-4-carboxylic acid (K 13176, 21) was significantly more active in vitro and in vivo than cefazolin against Gram-negative bacteria.

[The paraproteinemias. Clinico-biological study of hospital cases].

90 cases of paraproteinemia treated for 12 years, have been studied. The results, documented by a clinical and bio-humoral point of view, have been compared with those of largest and latest casistics. The Authors tried to see if there were some connections between serology, radiology, histology and clinical pictures of different cases of paraproteinemia both as far as the classes and the types. The clinical, registrar and hospital information about the homogeneous population of Saronno have been asked in order to consider the course of the illness continuously. Also the follow-up has been compared with the data of the specific literature. The obtained data are almost similar to those of the most accurate works which have been described.

[Target cimetidine treatment of peptic ulcer. Considerations and assessment of a new dosage].

32 patients suffering from peptic ulcer have been selected. Of these 12 formed part of a previous series treated with cimetidine and subsequently relapsed; the other 20 were selected on the basis of endoscopic and functional examinations and were treated for the first time with cimetidine. The drug doses used were 1.2 g for 30 days and 0.3 g for a further 60 days. The purpose of the study was to check for possible recurrences by comparing the results obtained and their maintenance in the long term using these doses. 2 months after suspension of maintenance treatment there has not been any recurrence of the acute pain and dyspeptic symptomatology.

[Cryoglobulinemia syndrome and liver diseases. Case reports].

Six cases of an association of liver disease with a cryoglobulinaemia syndrome are described. An account of their clinical, body fluid and anatomopathological pictures is followed by a discussion of their possible causes in the light of the relevant literature. It is felt that no assessment can be made of the primary nature of either disease in associations of this kind. In addition to cases in which prior liver disease or exposure to hepatitis virus can be shown, in fact, there are other situations in which liver disease was present before the manifestation of cryoglobulinaemia. Repeated antigen stimulus triggers the antibody response leading to the formation of cryoprecipitating complexes. In most cases, the nature of such antigen is not known, since HB virus or any other virus, bacterial product, or cell catabolism product either eliminated in insufficient quantities, or produced in abnormal quantities, may be responsible.

A long-term follow-up study in essential cryoglobulinemia.

In a case series of 56 patients with essential cryoglobulinemia, 35 were followed-up for 4-13 years (mean 7 years). A membranous proliferative glomerulonephritis, which in about half the cases showed a progression to renal insufficiency, was the commonest complication, observed in more than one third of the patients. In 2 patients hepatic cirrhosis became manifest after a completely asymptomatic period and in 2 others a lymphoproliferative disease appeared 2 and 8 years after the onset of purpura. In 51% of patients the intial clinical pattern did not change. In searching for a correlation between the development of nephropathy and cryoglobulin characteristics, none was demonstrated studying the cryoglobulin level, the presence of autoantibody and the complement components.

Serum complement pattern in essential mixed cryoglobulinaemia.

In twenty-six patients affected by essential cryoglobulinaemia, 188 determinations of serum complement components (SCC) were made. A peculiar pattern was observed which was characterized by: (a) low levels of early components (Clq, Cls and C4), (b) normal levels of C3 and high concentrations of late components (C5, C9) and (c) CH50 values significantly lower than normal. No relationship could be observed between early SCC and C3 levels. Thirty-three crossed immunoelectrophoreses were performed in thirteen patient's blood samples. The C3c peaks were not different from normal. Follow-up data (156 serum samples from twenty-four patients) during a 6-40 month period showed a non-homogeneous SCC behaviour. However, no relationship was found between the complement concentrations and clinical score. These findings suggest that SCC abnormalities are related to a complement hyposynthesis, which could be caused by a reduced C2 production or by a negative feedback effect of active components or their fragments.

New broad-spectrum alkylthio cephalosporins.

A series of 7-substituted alkyl-thio-acylaminocephalosporins with the following general formula were prepared and tested for in vitro antibacterial activity: (formula: see text). We tried in our research to find any relationship between antibacterial activity and pharmacokinetic properties on the one hand, and chemical structure on the other. The most interesting products were also studied for their in vivo antibacterial activity in experimental acute systemic infections in the mouse.
