[The efficacy of echography in the diagnosis of gallbladder carcinoma].

The preoperative diagnosis of carcinoma of the gallbladder is very difficult because the clinical manifestations of this disease are nonspecific and often indistinguishable from those of cholecystitis. Radiography, including oral cholecystograms and i.v. cholangiograms, give aspecific findings. Ultrasound scans were performed on 13 gallbladder carcinomas: only "massive" cancer was diagnosed correctly, because sonography does not offer specific patterns for "infiltrating" and "fungating" carcinomas.

[Risk factors and sensitivity to certain antibiotics in hospital urinary infections].

Urinary infections contracted in hospital are one of the most important health problems. The present paper looks at 549 hospitalised patients and 548 subjects referred to the laboratory for routine tests over a period of 6 months. The results showed a higher number of hospital infections in the Medicine Department and this number was directly proportional to certain risk factors such as the presence of a catheter at home, the greater age of patients and a poorer physical condition. The strains most frequently isolated were E. coli and Pseudomonas spp although percentages were different between out-patients and hospitalised patients. The infections in question might be reduced and, at least partially, controlled by means of stricter hygiene on the part of personnel and by a moderate use of vesical catheterism.

[Assessment of human papilloma virus colposcopic findings of the cervix (in the Italian colposcopic classification].

HPV-induced flat condylomatosis is not only much more frequently observed on the hexocervix than the florid type, but it also induces colposcopic changes in the epithelium without completely altering its morphology. HPV-induced changes have now been largely identified and are recognisable in colposcopy. They can therefore be classified in the various patterns that characterise colposcopically the transformation epithelium, with undeniable advantages not only from the point of view of their correct classification but also from that of their prognostic evaluation. This is important because viral modifications ANTZ have a different diagnostic and prognostic significance from those observable in the notice squamous epithelium. A classification proposal of the various findings of flat HPV condylomatosis based on italian colposcopic classification is therefore presented.

Microleakage of mixed Class-V restorations placed with GIC-bases or dentin bonding agents.

The aim of this study was to compare the microleakage of a new generation of light-cured glass-ionomer cement bases and multistep dentin bonding agents with materials of the previous generation. Mixed class-V non-retentive cavities were prepared in freshly extracted third molars and filled with the test materials. After 250 thermocycles, the teeth were immersed in a dye solution for 48 h, sectioned longitudinally and evaluated under a stereomicroscope. The new bonding systems (i.e. Clearfil New Bond, Gluma Bond, Scotchbond 2 and Vitrabond) showed statistically significant lower microleakage values than the previous materials (i.e. Scotchbond DC, Ketac Bond, GC lining cement and 3M GIC). This in-vitro study emphasized the efficacy of the new generation of glass-ionomer cement bases and multi-step bonding agents.

[Arterial hypertension with hypokalemia].

After a careful review of the literature on physiopathological role of the renin-angiotensin-aldosterone system in cases of arterial hypertension with hypokalaemia, a simple diagnostic procedure is suggested to differentiate between this and secondary hypertensions which, though few, are often curable. Three clinical cases examined in the medical department of Casalpusterlengo Hospital are presented in which the study of the renin-angiotensin system proved decisive for a correct diagnosis of the hypertension.

Stereospecific disposition of flunoxaprofen enantiomers in human beings.

The absorption and disposition kinetics of the enantiomers of the nonsteroidal antiinflammatory drug flunoxaprofen were studied in six healthy volunteers after oral administration of either R,S(+/-)-flunoxaprofen or R(-)-flunoxaprofen. The apparent values of the volume of distribution and systemic clearance of the S(+)-enantiomer were significantly lower than those of the R(-)-enantiomer. There was no significant difference in the absorption and elimination half-lives between the two isomers. The S(+)- to R(-)-isomer plasma concentration ratio increased with time with an apparent inversion half-time of about 50 h. This observation suggests metabolic inversion of R(-)- to S(+)-enantiomer, although the possibilities of stereoselective bioavailability or interaction between the two isomers can not be excluded.

Relationship between the efficiency of muscular work during jumping and the energetics of running.

The running economy of seventeen athletes was studied during running at a low speed (3.3 m X s-1) on a motor-driven treadmill. The net energetic cost during running expressed in kJ X kg-1 X km-1 was on average 4.06. As expected, a positive relationship was found between the energetic cost and the percentage of fast twitch fibres (r = 0.60, n = 17, p less than 0.01). In addition, the mechanical efficiency during two different series of jumps performed with and without prestretch was measured in thirteen subjects. The effect of prestretch on muscle economy was represented by the ratio between the efficiency of muscular work performed during prestretch jumps and the corresponding value calculated in no prestretch conditions. This ratio demonstrated a statistically significant relationship with energy expenditure during running (r = -0.66, n = 13, P less than 0.01), suggesting that the elastic behaviour of leg extensor muscles is similar in running and jumping if the speeds of muscular contraction during eccentric and concentric work are of similar magnitudes.

The effect of pre-stretch on mechanical efficiency of human skeletal muscle.

The mechanical efficiency of positive work was studied in six subjects performing three different types of exercises. On the first occasion the subjects ran on a motor-driven treadmill at 3.33 m s-1; the second and the third exercises consisted of performing rhythmical vertical jumps for 1 min both in rebound (RJ) and no-rebound (NRJ) conditions. The mechanical efficiency calculated in NRJ, which reflects only the conversion of biochemical energy into mechanical work, was found to be lower than the corresponding observation in RJ, 17.2 vs. 27.8% (P less than 0.001), respectively. These differences could not be explained by only the storage and recoil of elastic energy occurring in RJ compared with NRJ. The calculated extra work delivered 'free' was greater than the potential elastic energy which could be stored within the leg extensor muscles (187 vs. 124 J for each jump, P less than 0.05). It is likely that other factors might be responsible for the extra work found in NRJ. It was suggested that the difference in the length of time to perform positive work between a simple shortening contraction and a stretch-shortening muscular activity could be also responsible for the enhanced efficiency observed in RJ. This suggestion was supported by the high relationship (P less than 0.001) found between the time to perform positive work and the mechanical efficiency measured in jumping and estimated during running.
