[A case of papillary tumor of the pancreas].

The authors report a case of papillary cystic tumor which is a rare form of pancreatic tumor of exocrine origin. A young woman presenting with a painful abdominal mass was operated on for a tumor of the head of the pancreas. Gross and microscopic examination revealed a papillary cystic tumor of the pancreas. Tests for histologic markers found progesterone receptors. A review of the literature found approximately 100 cases of papillary cystic tumors of the pancreas. This type of tumor is seen mainly in young women presenting with an abdominal mass. After complete resection of the tumor, these patients usually make an uneventful recovery. Since regrowth of the tumor may occur these patients should be submitted to prolonged follow-up.

[Cardiac rhabdomyoma in the adult. Echocardiographic diagnosis and successful surgical treatment].

Cardiac rhabdomyoma is a rare tumour (190 cases in the literature). Since the first operation, in 1959, the tumour has been removed in about 30 patients. Our case is the first cardiac rhabdomyoma diagnosed in live adults. The patient was a 59-year old woman without any significant history. Echocardiography showed a single tumour in the right atrium. A tentative diagnosis of myxoma was made and the patient was operated upon. At pathology, the tumour turned out to be a rhabdomyoma. After a 2.5-year follow-up period, the patient remains asymptomatic. Cardiac rhabdomyoma almost exclusively occurs in children. Only six adult cases have been published, all diagnosed at autopsy. Cardiac symptoms are either those of a valve disease or arrhythmias. Treatment is surgical for single tumours. Multiple rhabdomyomas are associated with Bourneville's disease (tuberous sclerosis) in 50 p. 100 of the cases, which entails a sombre prognosis. In our patient, the presence of a single tumour at the age of 59 without any sign of tuberous sclerosis and the fact that the tumour could be entirely removed were factors of favourable prognosis.

[Adenocarcinoid of the stomach. Histochemical and immunohistochemical study using optical and electron microscopy].

A case of gastric adenocarcinoid was described by light and electron microscopy and immunohistochemistry. The tumor contained exocrine, endocrine and amphicrine cells. In endocrine cells, serotonin and pancreatic polypeptide were demonstrated. It was suggested that these gastric adenocarcinoid cells originate from totipotent immature cells of endodermal origin.

Immunohistochemical study of pheochromocytomas. An investigation of methionine-enkephalin, vasoactive intestinal peptide, somatostatin, corticotropin, beta-endorphin, and calcitonin in 16 tumors.

Immunohistochemical investigations were carried out on 16 pheochromocytomas for a study of their immunoreactivity to methionine-enkephalin, vasoactive intestinal peptide, somatostatin, corticotropin, beta-endorphin, and calcitonin on serial semithin araldite sections. All antiserums except anti-beta-endorphin, selectively stained a variable number of distinct tumor cells. Methionine-enkephalin-immunoreactive cells were the most frequent. The neuropeptide content of pheochromocytomas appears highly diverse and unpredictable. These findings are supportive of the concept of multisecretory APUD cells of neural crest origin and rule out any of these neuropeptides as reliable immunohistochemical markers for tumor chromaffin cells.

[Role of formalin in the occurrence of sclerosing lesions of the bile ducts. Experimental approach in dogs. Apropos of 3 clinical cases].

The authors experimentally reproduced in dogs, sclerosing lesions of biliary tract similar to those that they noticed in clinical practice, after the use of a formalin solution for sterilization of the hydatid cysts of the liver. These lesions have been found with formalin solutions more concentrated than those used in surgical treatment of liver echinococcosis. Nevertheless, even with the use of lower concentrated solution in every case, inflammatory scars are produced in the liver and the biliary tract with persist several months after the formalin injection consequently, the authors recommend to forsake the abandonment of this product for surgical treatment of hydatid cyst.

[Ajmaline-induced hepatitis. A case report with ultrastructural study].

The authors report the case of a patient with ajmaline hepatitis. The clinical presentation suggested angiocholitis; serum bilirubin concentration and the activity of alkaline phosphatase were markedly increased; serum transaminase activity was moderately increased; the prothrombin time remained normal. After interruption of the drug, the outcome was favorable, with complete recovery within 2 months. Histologic examination of a liver specimen obtained early after the onset of jaundice showed inflammatory cells in the portal tracts and mild hepatocytic lesions. Electron microscopy disclosed dilatation of the endoplasmic reticulum. The microfilamentous network of the hepatocytes was reduced and disorganized. Biliary canaliculi were enlarged with absent or blunted microvilli. There was evidence for passage of bile products from the biliary caniculi into the space of Disse. These aspects are reminiscent of those observed in animals after the administration of cytochalasin B. It is suggested that ajmaline may, in some patients, trigger an immune response which then alters the microfilamentous network of the hepatocytes, and may, thereby, produce cholestasis.

[Electron microscopic study of the sera of hepatitis B surface antigen (HBs Ag) subjects (author's transl)].

Electron microscopic observation of sera from 59 subjects(HBs Ag-negative controls, HBs Ag-positive asymptomatic blood donors, and HBs Ag-positive patients with acute viral hepatitis, chronic persistant or active hepatitis and alcohol-induced liver damage) was capable of specifying the morphology of the HB virus (spherical particles, rod-shaped particles and Dane particles) and of correlating the aspect of the observed viral material with the nature and evolution of the liver damage. The viral particles are observed in small amounts in asymptomatic blood donors and in patients with acute viral hepatitis, but are present in very significant quantities in chronic hepatitis. Dane particles and viral particle aggregates are more numerous in chronic active hepatitis than in chronic persistant hepatitis. The detection of HBeAg in the serum, especially found in chronic active hepatitis, correlates with the presence of Dane particles. In most cases, the observed viral particle aggregates do not seem to be associated with an immunological process.

[Characteristic ultrastructural features of parathyroid gland cancer. A report on 9 cases (author's transl)].

Authors have studied 9 parathyroid carcinomas under electron microscope (3% of their parathyroid lesions). Two types of the ultrastructural features were significant: 1) architectural: basal membrane abnormalities, sparse desmosomes, absence of nerve fiber; 2) cytological: nuclei atypism and abnormal mitochondria in tumors cells resembling dark chief cells. Also noticed were signs of cell hyperactivity and/or trouble in parathormon production or regulation.
