[Syphilitic encephalitis: a diagnosis to be kept in mind. Epidemiological considerations and description of a case].

Since the introduction of antibiotic treatment there has been a remarkable reduction in the incidence of "classical" neurosyphilis, and, when cerebral involvement occurs, a relative increase in abortive or monosymptomatic cases that are difficult to diagnose because of the prevalence of meninges and/or mental signs. A case of luetic meningoencephalitis is described with emphasis on diagnostic, prognostic and therapeutic aspects.

[Epidemiological aspects of imported malaria 1976-1985].

The comparative analysis of the cases of imported malaria in Lombardy and in Italy during the ten-years period 1976-1985 shows that the prevalent factors of risk are, in both case-lists, the young age, the work motivated travelling and the stay in endemic areas of tropical Africa, where P. falciparum is greatly prevalent. Interesting outcomes of comparison of the two consecutive five-years periods 1976-1980 and 1981-1985 are discussed. As far as the african provenience (decreased) and the work motivated travelling (increased) are concerned, the two case lists are consistent and agree, yet P. falciparum malaria cases are decreased in Lombardy only. The increasing habit of chloroquine chemoprophylaxis in Lombardy, the persistant sensitivity of P. vivax to this drug and the widespread extent of chloroquine-resistant P. falciparum strains in Africa are to be taken into account.

Routine application of the evaluation of yeasts susceptibility to the main antimycotic drugs.

We evaluated the susceptibility of 618 strains of yeasts belonging to the genus Candida, isolated from various biological specimens. Amphotericin B, Nystatin, Fluorocytosine, Clotrimazole, Miconazole, Econazole and Ketoconazole were tested with the Kirby-Bauer method, modified according to Drouhet and Dupont. The polyenes Amphotericin B and Nystatin and the antimetabolyte Fluorocytosine were the most effective drugs, while the imidazole derivatives (Clotrimazole, Miconazole, Econazole and Ketoconazole) generally showed a lower activity. We emphasize the importance of routine evaluation of the susceptibility of isolated yeasts to the main antimycotic drugs.

Pulmonary aspergillosis in a case of humoral immunodeficiency.

A case of recurrent pulmonary aspergillosis in a young girl with common variable hypogammaglobulinaemia and the treatment adopted is described. We employed the plasma-exchange, at the beginning of immunosubstitutive treatment, to provide a high serum concentration of all classes of immunoglobulins and to better control the patient's state of shock. Intravenous immunoglobulins and Amphotericin B were also given in combination with plasma-exchange, having a synergic therapeutic effect. The good condition of the patient, despite the recurrent episodes of fungal pneumonia, suggests that such an approach can lead to long-term, high quality survival in humoral immunodeficient patients.

[Scintigraphy in the study of varicocele].

Varicocele which is relatively common after puberty may cause even severe alterations to fertility. Sequential Isotopic Scrotal Angiography (SISA) was used for the diagnosis of this condition in both the recognition and confirmation phase. This dynamic and static survey based on the use of 99mTc-pyrophosphate proved able to discriminate between the various degrees of varicocele. No statement can as yet be made on the specificity and sensitivity of the examination since so few cases have so far been checked with other techniques (Doppler-Telethermography-Phlebography) and an insufficient number of postsurgical controls came to our observation.

An eleven year survey (1975-1985) of fulminant hepatitis: considerations on epidemiology and pathogenesis.

This review covers 42 patients with fulminant hepatitis treated at the Clinic of Infectious Diseases of University of Pavia over a period of 11 years (1975-1985). This group is 1.9% of the 2181 subjects hospitalized for viral hepatitis, and consists of 25 males (59.5%) and 17 females (40.5%) with a mean age of 44 years. 26 patients (62%) had a virus B hepatitis, whereas in the remaining 16 the non-B type was diagnosed. B hepatitis had an overall prevalence of 56.4%. No case of fulminant hepatitis showed prothrombin levels above 23 (overall average 14%) and bilirubin below 17 mg/dl (overall average 24.5 mg/dl). SGOT and SGPT values were 1684 and 1868 UI/l respectively. 24 patients (57.1%) developed coma after admission to hospital and 18 had this diagnosis upon entry. History evidenced a heroin habit for 6 patients (14.2%); antiblastic treatment for 13 (30.9%); a preexisting liver disease for 7 (16.6%); other major pathologies for 6 (14.2%); steroid anabolizers, barbiturates + pirazolone derivatives and oral contraceptives use for 4 (9.5%) respectively. The overall mortality rate was 90.5%. 17 subjects (40.4%) were treated in an intensive care unit with a mortality rate of 75%.

AIDS: outcome of children born to HTLV-III/LAV infected mothers. Report of 15 Italian cases.

The characteristics of acquired immunodeficiency syndrome (AIDS) in children have been described in several reports. The occurrence of specific anti-HTLV-III/LAV seropositivity in children born to seropositive mothers is also becoming more frequent in Italy. We report 15 cases of seropositive children born to 14 seropositive, drug-addicted mothers. Among them, 5 died of AIDS, 8 developed a variable, non-progressive AIDS-related complex (ARC), while only two remained persistently asymptomatic. Liver disease was present in 2 patients, CNS disease in 4, gastrointestinal troubles were never a major problem. Only 2 cases of HBV infection and 3 of thrombocytopenia were observed. In our experience, homosexual behaviour of the parents was never a cause of family risk. Children presented with failure to thrive and/or signs of disease usually within the first five months of life. In one family the first child was affected by fatal AIDS while the younger child showed only passive, transient immunization by maternal specific IgG. One ARC patient, observed for over 3 years, showed apparently complete healing from the disease.

Mycotic vaginitis in pregnancy: a double evaluation of the susceptibility to the main antimycotic drugs of isolated species.

The authors examined 160 non-selected patients between the 28th and 40th weeks of gestational age with mycotic vaginitis. The chemosusceptibility of the isolated yeast to the main antimycotic drugs was evaluated through the Kirby-Bauer method as well as the determination of MIC. The antimycotic drugs tested were Nystatin, Miconazole, Ketoconazole, Clotrimazole, Amphotericin B, Econazole and 5-Fluorocytosine. The obtained data indicate a lacking effectiveness of Clotrimazole in contrast with the high percentage of therapeutical success reported in the literature. Better results were found with Econazole and the other antimycotic drugs tested. Nystatin is the most effective drug "in vitro". The authors furthermore highlight the utility of MIC determination, above all in the cases of recurrent vaginitis and when systemic therapy is undertaken.

Costs and benefits of diagnostic and therapeutic methods in Salmonella infections.

Type and number of specific diagnostic procedures; type, time and exitus of therapy were studied in the management of 335 hospitalized patients with Salmonella infection during a nine/years period (1976-1984). Sixty-three cases of S. typhi (18.8%), 26 of S. paratyphi B (7.7%), 48 of S. typhimurium (14.3%) and 198 other Salmonella species (59.1%) were identified. Positive cultures were obtained from blood in 18.8% of cases. In typhoid fever bacteriemia was detected in 60.3% of cases. The sensitivity of microorganisms to antibiotics was detected. Of 281 (83.8%) patients treated with "major" antibiotics (CAF, Ampicillin or Co-trimoxazole) clearance of Salmonella was obtained in 171 (60.8%). Stool "acidification" with lactulose was attempted in 54 patients (16.2%), of which 20 (37%) became negative. Charges of the bacteriologic cultures, serological tests, antibiotics and supportive therapy were estimated in order to assess the cost/benefit ratio of patients care. Length of stay and chemotherapy were respectively 20.7 (11-18) and 11 (7-17) days. Maximal cost/case ratio occurs in diagnosis for stool cultures, in therapy for Ampicillin. The introduction of an economic approach to use diagnostic and therapeutic procedures has significantly improved the management of Salmonella infections from 1976 to 1984.

Primary hepatic aspergillosis: report of a case.

The authors describe a case of primary hepatic aspergillosis in an adult patient with no evident immunological impairment. Because the finding is to be regarded as unusual, the possible pathogenetic role of alcohol and long standing exposure to asbestos is discussed.

[In-vitro microbiological evaluation of the action of gentamicin and sisomicin o schizomycetes of the Salmonella genus].

The in vitro activity of Gentamicin and Sisomicin towards 142 clinical isolated Salmonella strains was determined and statisically evaluated. The majority of the Salmonella strains was inhibited by concentrations of both antibiotics ranging between 0.75 mcg/ml and 0.02 mcg/ml in the Minimal Inhibitory Concentration (MIC) test and between 1.5 mcg/ml and 0.02 mcg/ml in the Minimal Bactericidal Concentration (MCB) test.

[2 cases of fibroepithelial polyp of the ureter].

Fibroepithelial polyps of the ureter, a mesodermal benign tumor, are rare. The first case of a fibroepithelial ureteral polyp associated with bilateral complete ureteral duplication and another case associated with bilateral hydronephrosis are reported. The first case suggested a malformative congenital etiology. The two fibroepithelial polyps were treated by conservative surgery.
