The initiation of de novo methylation of foreign DNA integrated into a mammalian genome is not exclusively targeted by nucleotide sequence.

The de novo methylation of foreign DNA integrated into the mammalian genome is a fundamental process whose mechanism has not yet been elucidated. We have studied de novo methylation in adenovirus type 12 (Ad12) genomes inserted into the genomes of Ad12-induced hamster tumor cells. De novo methylation of Ad12 DNA, which is not methylated in the virion, is initiated in two paracentrally located regions and spreads from there across the integrated Ad12 genomes. (i) After extensive cultivation of cloned Ad12-induced hamster tumor cell lines, the same segments in integrated Ad12 DNA in different cell lines become methylated or remain unmethylated, depending on their positions in the viral genome. (ii) When Ad12 DNA or Ad12 DNA fragments are transfected into hamster cells and permanent cell lines are established by selection for the cotransfected neomycin phosphotransferase gene, patterns of de novo methylation in terminally or internally located segments of Ad12 DNA are different from those in Ad12-induced tumor cell lines. (iii) A detailed study on the topology of the integrated viral genomes in the Ad12-transformed hamster cell lines T637 and A2497-3 and in the Ad12-induced hamster tumors T191, T1111(1), and T181 has been performed. Some of the integrated viral genomes are inserted into the cellular genome in an orientation colinear with the virion genome; others have been rearranged. An originally internally located Ad12 DNA segment has become transposed to the left-terminal sequences of the viral genome in several cell lines and tumors. In the complete Ad12 genomes, the internally located PstI-D fragment becomes extensively methylated at the 5'-CCGG-3' and 5'-GCGC-3' sequences. When this DNA segment has been juxtaposed to the left-terminal, hypomethylated fragment of Ad12 DNA in rearranged genomes, the PstI-D fragment remains unmethylated. We therefore reason that the initiation of de novo methylation in integrated Ad12 DNA cannot be directed exclusively by the nucleotide sequence. Other parameters, such as site of integration, conformation of integrates, mode of cell selection, or chromatin structure related to transcriptional activity, may play decisive roles.

[Janeway laparoscopic gastrostomy].

The percutaneous endoscopic gastrostomy is the procedure of first choice for the creation of a gastrostomy. If that is not possible a laparoscopic access should be preferred to a laparotomy. In tumor patients there seem to be some advantages of the laparoscopic Janeway gastrostomy compared to Kader-Stamm fistulas. We describe the technique and discuss the results of 5 patients without complications referring to current publications. In conclusion the Janeway gastrostomy is a good palliation because it is an effective, safe and simple method with easy handling, and little requirement for nursing care.

[Technique and results of laparoscopic vagotomy in chronic duodenal ulcer].

Advances made in the medical treatment of duodenal ulcer disease have lead to a sharp decrease in the number of patients who are referred for operative therapy. Today surgery may be indicated in patients with chronic and recurrent ulceration following failure of H. pylori eradication regimens and in patients with no or reduced compliance. Laparoscopic vagotomy procedures may offer this select group of patients an additional benefit and comfort, if their short-term and long-term results are comparable to those achieved following conventional open vagotomy. This is a prospective study on patients who underwent laparoscopic vagotomy using two different adapted methods evaluating short-term clinical and functional results. From October 1991 through April 1994 8 patients were studied. 3 patients received an anterior seromyotomy with posterior truncal vagotomy (according to Taylor) and 5 patients underwent anterior linear strip gastrectomy with posterior truncal vagotomy (according to Gomez-Ferrer and Morris). All patients had gastroduodenoscopy, secretory analysis of basal acid output (BAO) and maximal acid output (MAO) and evaluation of their H. pylori status performed pre- and postoperatively. Median follow-up was 15 months. 5 patients were completely asymptomatic, 2 patients complained of mild bloating or reflux and 1 patient had mild diarrhea up to 6 months postoperatively. None suffered an ulcer recurrency. All remained H. pylori positive following surgery. Secretory analysis of acid production showed a 50-70% drop in BAO and MAO after vagotomy. Both vagotomy procedures described have been specifically adapted and are well suited for a laparoscopic approach. They are expedient and easy to employ. As clinical and functional data demonstrate they achieve short-term results comparable to those seen after conventional surgery.

[Laparoscopic transperitoneal adrenalectomy].

The laparoscopic transperitoneal adrenalectomy combines the advantages of the conventional transperitoneal approach with the well known advantages of minimally invasive surgery. The positive personal experience in 5 patients and the results of the literature are reported. According to these data we are dealing with a safe procedure requiring an acceptable operating time, little postoperative pain of short duration, fast recovery and a short postoperative in-hospital stay. Indications are benign non-hormone- as well as hormone-producing adrenal lesions. Until now there are no definitive size limitations. Malignant tumors should continue to be operated conventionally.

[A new miniaturized flicker test for detection of florid retrobulbar neuritis].

A new transportable miniaturized flicker test equipped with green LEDs is contrasted with the conventional Tübingen flicker test, based on brightness perception of intermittent white light. The results are compared with those of former studies. In this study we examined 45 eyes suffering from florid optic neuritis. Another group of 114 eyes were either normal (except minimal ametropia) or suffered from a non-inflammatory or non-refractive impairment of central visual function. In all groups only one eye of each subject was examined. Using the criteria of Trauzettel-Klosinski, the new miniaturized "green" flicker test for the detection of a florid optic neuritis showed a specificity of only 50.9% (83.3%) and a sensitivity of 91.1% (64.4%). (The corresponding results of the conventional Tübingen flicker test are shown in parentheses.) Alternatively, analyzing the data with the help of a logistic regression, the "green" flicker test turned out to have a specificity of 92.9% (93.8%) and a sensitivity of 60.5% (44.2%). For further examinations using the flicker test it is useful to consider the duration and reproducibility of each brightness match by repeating the test several times.

[Surgical revision of the pilonidal sinus with single shot antibiosis].

For a therapy of the pilonidal sinus there are different surgical and non-surgical methods to be found in literature. In our surgical wing 140 male patients have been operated on this disease from September 1990 to July 1992. 16 cases out of these 140 were re-operations. Using a special surgical technique we applied the excision with a primary closure under single-shot antibiosis in 139 cases. By modifying this antibiosis prophylaxis we could reach a primary success healing rate of up to 96%. A retrospective study among our patients showed a recurrence rate of 5%.

Subcutaneous low-molecular-weight heparin fragmin versus intravenous unfractionated heparin in the treatment of acute non massive pulmonary embolism: an open randomized pilot study.

Low-molecular-weight heparins have been extensively investigated in the treatment of deep venous thrombosis but limited data are available concerning their use in pulmonary embolism. In an open, pilot, randomized study, we compare the safety and efficacy of Fragmin, a low-molecular-weight heparin with those of unfractionated heparin in 60 patients with non massive pulmonary embolism (Miller Index < 20). Thirty one patients received unfractionated heparin intravenously and 29 received a fixed dose of 120 Anti-Xa IU/kg of Fragmin administered subcutaneously twice a day for 10 days. There was no pulmonary embolism recurrence nor major bleeding in either group during the treatment period. The decrease in pulmonary vascular obstruction on perfusion lung scan between day 0 and day 10 was 17 +/- 13% in the Fragmin group and 16 +/- 13% in the heparin group (NS). These results indicate that Fragmin may be a safe and effective treatment of submassive pulmonary embolism.

[Pulmonary embolectomy in pulmonary embolism: surgery and endoluminal techniques].

Since Trendelenburg's first attempts in 1908, the techniques of embolectomy have progressed considerably. The reference method remains embolectomy under cardiopulmonary bypass, the development of which has reduced the operative mortality to 30-40% instead of 60% when embolectomy was performed without cardiopulmonary bypass. In the last few years, several techniques have been developed to perform embolectomy by percutaneous endoluminal methods. These procedures are difficult to initiate, little experimented in humans or still at the experimental stage in animals. Advances in the medical treatment of massive acute pulmonary embolism have reduced the indications of embolectomy which has become the exception reserved for the most seriously ill patients in whom the other methods are contraindicated or have failed.

[Thromboembolic disease and cancer].

Many anomalies in primary haemostasis, coagulation and fibrinolysis have been described which favour thrombosis in cancer patients. Cancer is often associated with other risk factors for thromboembolism which explain much of the increased frequency of thrombosis in these patients. Thromboembolism may precede cancer and sometimes is the inaugural manifestation leading to the diagnosis of cancer. Cancer occurs more frequently in patients with idiopathic recurrent venous thrombosis than in those with thrombosis due to other risk factors for thromboembolism. The results of antivitamin K therapy are disappointing: in retrospective series, recurrent thromboembolism occurred in 9% of the treated patients and severe haemorrhage in 27%. New therapies should be assessed in cancer patients with thromboembolism.

Alterations of the subcommissural organ in the hydrocephalic human fetal brain.

We have studied the subcommissural organ of two hydrocephalic brains, of 20 and 21 gestational weeks and of two normal brains, aged 19 and 23 gestational weeks. Both hydrocephalic cases presented a size reduction of the subcommissural organ compared to the normal cases; only in one case, there were also alterations of the morphological components of the subcommissural organ, suggesting different pathogenic relationships between hydrocephalus and dysplasia of the subcommissural organ.

NADPH-d activity in the islands of Calleja: a regulatory system of blood flow to the ventral striatum/pallidum?

We have used dihydronicotinamide adenine dinucleotide phosphate diaphorase (NADPH-d) histochemistry to study the anatomical relationships between the islands of Calleja (ICs), ventral striatum (VS) and ventral pallidum (VP), and the perforating branches of the anterior communicating and anterior cerebral arteries traversing the olfactory tubercle. The granule cells of the ICs are intensely positive for NADPH-d, a marker for neuronal nitric oxide synthetase (NOS), and closely surround all arterioles perfusing the VP and most of the arterioles supplying the VS. In contrast, they are not related to the arteries destined for the dorsal striatum. On the ground of the vasodilatory properties of the nitric oxide, we propose that the ICs may play a role in the regulation of blood flow to specific centres of the limbic forebrain.

Transient NADPH-diaphorase activity in motor nuclei of the foetal human brain stem.

NADPH-d activity is a marker for neurones which synthesize nitric oxide (NO). In the present paper we study the NADPH-d activity in the human brain stem between 16 and 24 gestational weeks (GW). The adult distribution of NADPH-d containing neurones is established at 24 GW. Between 19 and 21 GW, neurones of the facial, hypoglossal and ambiguus nuclei show transient NADPH-d positivity. Therefore, in the human brain stem there are two chronological patterns of neuronal nitric oxide synthase (NOS) expression, one that appears during prenatal development and persists during the whole life, and another one, present only at a precise foetal moment. The transient NADPH-d activity in motor nuclei suggests that NO is involved in early regulatory processes of the human brain development.

NADPH-d (dihydronicotinamide adenine dinucleotide phosphate diaphorase) activity in geniculo-tectal neurons of the rat.

Retrograde transport of horseradish peroxidase (HRP) and dihydronicotinamide adenine dinucleotide phosphate diaphorase (NADPH-d) histochemistry were used in order to determine whether NADPH-d-positive neurons of the ventral lateral geniculate nucleus (LGv) project to the superior colliculus (SC). Our results show that intensely stained NADPH-d neurons are restricted to the lateral half of the magnocellular division of LGv (LGv-MC), where they represent 50% of the total number of retrogradely labeled neurons. These findings indicate: (1) that LGv provides an important NADPH-d input to SC, and (2) that within the population of geniculo-tectal neurons, which constitute a morphologically well defined neuronal type, there are two different subclasses, one being NADPH-d positive and the other NADPH-d negative.

In vitro and in vivo viability assessment of unpurified pancreatic islet tissue.

The viability of porcine collagenase-prepared islet preparations (n = 16) was classified by 31P-NMR spectroscopy, staining by neutral red and trypan blue, and in vitro insulin secretion following glucose challenge. Vital islets exhibited a phosphate diester/phosphate monoester (PDE/PME) ratio of 0.5-0.9, a staining score of 18-30 and an insulin secretion responding well to glucose challenge. Damaged islets performed at a PDE/PME of 0.2-0.49 and a staining score of 9-17 and necrotic islets had 0.0-0.49 and a staining score of 9-17 and necrotic islets had 0.0-0.19 and 0-8, respectively. The islets of the latter two groups did not adequately respond to glucose. The in vivo function following autotransplantation of these islets into the spleen was investigated in five recipients of more than 3000/kg vital islets of which 4 expressed daily normoglycemia (< 200 mg%), normalized intravenous glucose tolerance (K = -2.21), and a prolonged survival (mean +/- SD) of 167 +/- 12 days compared to five recipients of > 3000/kg damaged islets (K = -0.814) (P = 0.0017) and a survival of 86 +/- 21 days (P = 0.0096). It is suggested that 31P-NMR spectroscopy is a valuable and practical method to predict islet graft viability prior to transplantation in order to assure good graft function in the recipient.
