Report of a case resembling the 'fleck retina of Kandori' with ectodermal peculiarities and macula degeneration.

This report involves the case of a 36-year-old woman followed-up for nine years. The symptoms include unique, sharply-defined, irregular, yellow, large flecks of the retina combined with bilateral macula degeneration. The patient's rusty-red hair, enamel dysplasia, and ashen-gray skin color were also noted. It is argued that this case is very likely identical with the 'fleck retina of Kandori' and as such, the first case reported outside Japan. The literature is reviewed.

[Measurement of subretinal fluid in retinal detachment (author's transl)].

In surgery for retinal detachment, such as shortening of the eyeball, volume reducing, and indentating operations, knowledge of the volume of the subretinal fluid is important. A method based on ultrasonography was developed to measure the volume of subretinal fluid preoperatively. After establishing a topography of the fundus, the height of retinal detachment can be measured by ultrasonography at 49 defined points. The diameter of the eyeball is measured at 12 different points. From the height of retinal detachment and the diameter of the eyeball it is possible to calculate the subretinal space, which is divided into 60 segments. The total volume of subretinal fluid is calculated by adding the volumes of these 60 segments. The calculus is programmed on a computer.

[Experience with the new photocoagulator (Zeiss) (author's transl)].

The new Zeiss Xenon-photocoagulator has considerable advantages. The possibility of converging the light beam allows coagulating even prominent areas (folds) in lensless eyes without difficulty. From our experience Xenon coagulation with individually adapted coagulation offers advantages over argon-laser coagulation with relatively short coagulation times.

Radiation therapy of choroidal metastases from breast cancer.

Radiation therapy of 26 patients with choroidal metastases from breast cancer has been reviewed. At the time of initial diagnosis of choroidal metastases 13 patients had bilateral choroidal metastases. The median age of the patients was 54 years and the survival from the first symptom of choroidal metastases varied from 2 to 45 months with a median survival of 9.5 months. End results of radiation therapy had been recorded in 22 patients. Local radiation therapy improved vision permanently in 16 patients.

[Photocoagulation: a microsurgical welding method for ophthamology].

The author describes first of all the history of photocoagulation from its very beginning up to the most recent instruments. Then examples are given of the different possibilities of application. Besides the detachment of the retina and the tumors, the diseases of the vessels of the retina are playing an important role.

Diabetic Retinopathy

[Sclerodermie en coup de sabre with spontaneous filtering bleb (author's transl)].

In a 43-year-old female patient with sclerodermie en coup de sabre on the right forehead, the following changes of the right eye were observed: Partial lack of hairs of the eyebrow; distortion of the upper eyelid, similar to a coloboma; paralysis of the inferior oblique and the superior rectus muscles; decrease of corneal sensibility; formation of a spontaneous filtering bleb in the upper limbal region; ocular hypotension with changes of the papilla.

[Tension under controlled conditions in the encircling operation (author's transl)].

The complications, which occur in encircling-operations, such as String-Syndrome, scleral Nekrosis, glaucoma-development (due to narrowing the anterior chamber) as well as closure of the central artery most frequently happen, when the globe has been tightened too much. From the calculations we have make using the cerclage element in reducing the circumference and volume (Fig. 2) we have shown that an identation of the sclera more than 3 mm deep is not advisable. The Silicon-Cerclage-Materials, which we use, are tested by the tension-testing machine, which determines the modulus of elasticity. The stress, that is force per sectional area, is determined in relation to the amount of elongation in the material (Fig. 3). A round silicon rod with a diameter of 2 mm and a modulus of elasticity between 150 and 200 p/mm2 proved most effective. In addition, we have conducted experiments with the Cerclage-rods on enucleated human eyes (Fig. 4). In one experiment the intraocular pressure was determined in relation to the pull of the weight upon the globe (Fig. 5). In a second experiment the reduction of the circumference was related to the height of the intraocular pressure (Fig. 6). The elastic Cerlage-rod has the advantage of creeping, when tensed, it tries to regain its original length. However, the globe, owing to the pressure within it, works against the creeping. With the help of the stress-strain diagram (Fig. 7) and the experiments on the human eye we can determine more accurately the degree of the creeping. In index 1 we can see how much the circumference has been finally reduced including the creeping in relation to the height of the intraocular pressure. The intraocular pressure should not be increased above 35 mm Hg. Otherwise: 1. the circumference of the globe will be considerably more reduced; 2. the intraocular pressure will rise much more slowly when compared to results at the lower eye-tension and 3. the volume reducing-effect will diminish markedly. It is important to measure the intraocular pressure before the end of the operation in order to avoid the above mentioned complications.
