Testosterone, 17 Ks, 17 beta E2 FSH-LH variations and hirsutism modifications during spironolactone therapy.

The research here reported concerns 9 hirsute women, four of them with PCO and five with idiopathic hirsutism, who underwent treatment with spironolactone. 4 non hirsute hypertensive cases served as control. For one year hair growth, testosterone, 17 Ks, estradiol and gonadotropins behaviours were studied in all of the patients. Results clearly show that the peripherical response (the hair) to the therapy is only just sufficient, and corresponds to a good reduction of the androgenic hormones in blood. However, there is also an LH gonadotropin secretion reduction which is statistically scarcely significant. If the therapeutic response of hair were good, fetal risk could be prevented with safe and contemporaneous contraception. However, since the response is scarcely sufficient, we do not think this therapy is more advisable than other ones.

Hormonal changes and hair growth during treatment of hirsutism with cimetidine.

The effects on hair growth by treatment with cimetidine have been studied. This drug has been given orally to 4 women with simple hirsutism and 3 women with peptic ulcer as controls for a period of 9 months. Hair growth slowed down in all of the treated women but the results were not statistically significant. A significant decrease in urine 17-ketosteroids has been observed, while plasma levels of testosterone, 17-beta estradiol, progesterone, FSH, LH and prolactin, did not change substantially. It is concluded that, on the whole, cimetidine doses not seem to induce such results on hair growth as to claim a role in the treatment of hirsutism in other current regimes.

Deficit of 21 beta-hydroxylase. Clinico-functional parameters.

The authors report the hormonal pictures of 5 young women with partial deficit of 21-beta-hydroxylase. They performed 17OH-progesterone, DHEA-S, plasma cortisol, free urinary cortisol, testosterone, ACTH and urinary 17KS assays. They then performed a suppression test by Desametazone and stimulus test by ACTH. The Authors stress the major role of 17OH progesterone, assayed in basal conditions and after ACTH stimulus, recalling the importance of early diagnosis. Authors also stress that in these patients testosterone is always significantly high.

Stimulation tests in adreno-genital syndrome induced by 21 hydroxylase deficit.

GnRH and TRH stimulation were performed on 4 patients affected by 21-beta-hydroxylase adrenal deficit. Plasmatic FSH, LH, 17-beta-estradiol, HPRL were assayed before and after GnRH, and HPRL before and after TRH. The results seem to prove that these tests are useless for the diagnosis of adrenal enzymatic deficit. On the other hand, they provide interesting additional information on physiopathological relations between adrenals and gonadotropins and between HPRL and adrenals.

Hyperprolactinemia and pregnancy. Clinical series.

We report 15 cases of pregnancy in 11 patients with hyperprolactinemia. These patients initially went to our Gynecologic Endocrinology Center for various menstrual troubles. They were all treated with bromocriptine, except one whose hyperprolactinemia was diagnosed when she had already started a gonadotropin therapy, since HPRL assays performed elsewhere had given normal results. In 6 patients we diagnosed prolactin-secreting pituitary adenoma. Only two patients underwent adenomectomy. All patients gave up bromocryptine as soon as their pregnancy was detected. Two patients had two subsequent pregnancies, another one had three. All pregnancies were single. The only twin pregnancy followed a gonadotropin therapy. One of the 15 pregnancies ended with abortion at the 12th week; another one (twin) with spontaneous delivery at the 37th week; 13 with term-delivery. They all had physiologic courses, except for one case of threatened abortion and one case of diabetes insipidus at the 9th month. None of the 15 newborns (7 SGA and 8 AGA) showed malformations. No sign or symptom of tumour growth was detected in the patients affected by pituitary adenoma.

Carotid Doppler examination: a correlation with angiography.

The Doppler ultrasound technique is a useful tool for the screening and follow-up of patients with extracranial atheromatous lesions of the carotid tree. Doppler examination was performed on 654 patients and compared with angiographic visualization of a total of 261 carotid arteries in 171 patients. The Doppler diagnostic criteria and their reliability are reported and discussed. This technique is not alternative but complementary to angiography.

Mathematical-statistical evaluation of hypophyseal and ovarian response to Gn-RH and to D-Leu6-des Gly10-LH-RH-EA stimulation in patients with amenorrhea.

In a group of 30 women with secondary amenorrhea the administration of D-Leu6-des Gly10-LH-RH-EA Analogue 25 mcg i.m. leads to a more intense and prolonged gonadotrophin response than synthetic Gn-RH 100 mcg i.v. The ovarian response to Gn-Rh is remarkable and lasts for approximately 24 hours; the stimulation with Analogue leads to a response which is slightly more intense than the one induced by Gn-RH, but probably of longer duration.

GnRH--test in menopause. Its possible use in identifying the subjects at risk for dysplastic endometrial pathology.

The Authors, after reviewing the results of their previous studies on the endocrine situation in post-menopausal endometrial carcinoma and considering what emerges from the present work, suggest GnRh-Test as a mean to better identify the subjects at risk for this neoplasia; in patients already affected with endometrial cancer, it could be an indirect index in recommending or not endocrine therapy.

"Euthyroidal' goiter and menstrual disorders.

We selected 5 patients, between 18 and 31 years of age, presenting menstrual irregularities and thyroidal goiter with no apparent functional change (normal, T4 and T3 tests). These patients underwent further examinations: TSH (basal and after TRH stimulation), HPRL (basal-circadian rhythm and after TRH stimulation), FSH and LH (basal and after Gn-RH stimulation), thyroidal scintiscan, search for antithyroid antibodies. Basal and post-stimulation TSH was fundamental in 2 cases of subclinical hypothyroidism. The positive antibody search confirmed the antoimmune origin. In the same patients, the HPRL circadian rhythm and TRH test highlighted a secondary hyperprolactinemia which would not have been revealed by a single basal sample.

Long-term treatment of Parkinson's disease with L-Dopa and Dopa-decarboxylase inhibitor: therapeutic results and side effects.

The authors present a review of their clinical experience in the treatment of 127 Parkinsonian patients with L-Dopa alone and in association with a Dopa-decarboxylase inhibitor; the follow-up lasted more than 7 years. In all cases the dosages of L-Dopa, either alone or associated with the decarboxylase inhibitor, were fairly low; the strategy was to reach in each case the minimal effective dose instead of the maximal tolerable one. After 7 years of treatment about 60-65% of the patients had shown improvement, even though to a lesser degree than during the first and second year of therapy; the remaining 35-40% were unchanged or worse. The authors conclude that, despite the decreased efficacy of the therapy with time, the L-Dopa treatment remains of great significance in lengthening and in improving the quality of life of Parkinsonian patients.

The rationale for patch-graft angioplasty after carotid endarterectomy: early and long-term follow-up.

A prospective study was undertaken in March 1980, at the Vascular Surgery Department of the Padua University, Medical School, to establish whether patch graft angioplasty is useful in preventing restenosis after carotid endarterectomy (CE). Seventy-four patients underwent 86 CE (bilateral in 12 cases) for atherosclerotic disease involving the carotid bifurcation. Thirty-eight (51.4%) patients presented TIA's or non hemispheric symptoms of cerebrovascular insufficiency; 30 (40.5%) were asymptomatic and 6 (8.1%) had partial nonprogressing or fixed strokes. All operations were performed under general anesthesia, with pharmacologic hypertension and systemic heparinization; in all cases, continuous EEG monitoring and 'stump pressure' measurement were employed. The operation was performed without a temporary intraluminal shunt in the patients showing tolerance to carotid clamping. The protection of the shunt was required only in patients with EEG monitoring changes (17). All carotid arteriotomies were extended into the internal carotid artery to overpass the end of the endarterectomy. Overpass was also used in the proximal edge of the arteriotomy, in the common carotid artery. The distal intima was never fixed with stitches and the arteriotomy was routinely closed with a PTFE patch graft angioplasty. Early results of cerebral protection were excellent. No patient presented permanent or transient postoperative neurological problems and no patient died in the postoperative period for causes related to the operation. This is substantiated by results we achieved during the period 1970-1979 in 192 patients, when all carotid endarterectomies were routinely performed without a shunt, with figures of 2.5% of postoperative stroke and 1.5% of mortality. Longterm follow-up (from 6 to 36 months) was completed in 51 patients (60 operations).(ABSTRACT TRUNCATED AT 250 WORDS)

[Carotid stenosis and obliteration of the contralateral carotid. A prospective study of the risks of a carotid endarterectomy intervention and its long-term results].

From March 1980 to July 1988 a consecutive series of 256 patients (p.) underwent 301 carotid endarterectomy + patch with routine use of continuous intraoperative EEG monitoring and selective use of an intraluminal shunt (IS) for the presence of an atherosclerotic plaque concerning the internal carotid artery (ICA). Patients were divided in two groups: the first (42 p.) marked by contralateral ICA occlusion, the second (214 p.) without contralateral ICA occlusion (259 CEA). Immediate peroperative, long term and global (immediate and long term) outcomes were prospectively and comparatively studied. A temporary IS was inserted in 27 p. (64%) of the group I and in 38 p. (14%) of the group II. Immediate permanent postoperative neurological deficit occurred in 1 p. of group I (2.38%) and in 2 p. (0.9%) in group II. Immediate postoperative mortality was 0% and 0.9% in group I and II respectively. All p. had neurological valuation and Echo-Doppler of operated ICA and of the contralateral ICA every 6 months (middle follow-up 44 months). New neurological symptoms compared in 5.8% of p. of group I and in 5.23% of p. of group II with a stroke rate of 0% and 2.32% respectively. There were 2 restenosis of operated ICA, both of them in p. of group I, that underwent reoperation. In the two groups the principal causes of deaths were myocardial infarct and cancer; in the group I no death was due to stroke versus 1.86% in the group II.
