Alcoholism and psychiatric disorder.

Over the past 50 years the relationship between alcoholism and psychiatric disorders, such as depression, anxiety and schizophrenia, has been the subject of a great deal of research. Psychiatric problems have been seen as both a cause and a result of alcoholism. Whatever the relation between alcoholism and psychiatric disorder is, it may have significance for the development of differentially effective treatment strategies. Several authors have argued that the presence and nature of psychiatric symptoms should form the basis of a classification system for alcoholics. Given the potential rewards of such a system for both understanding the aetiology of alcoholism and developing effective treatment strategies, it is important to have a clear picture of the nature and extent of this relationship. This paper reviews critically the published studies of the association between alcoholism and psychiatric disorder and outlines directions for future research.

The reliability of axis V of DSM-III.

The authors studied the reliability of axis V of DSM-III by analyzing ratings of 97 psychiatric inpatients made by a multidisciplinary team of clinicians. The intraclass correlation coefficient for ratings of the overall sample was .49, lower than the figure found during the DSM-III field trials. The psychiatric diagnosis, age, ethnicity, marital status, and sex of the patient did not significantly influence reliability. The authors recommend more explicit instructions in the administration of the scale, the use of a standardized interview, and more training for raters as ways of increasing reliability.

De Clérambault's syndrome--a nosological entity?

The consistency of de Clérambault's syndrome and its relationship to other diagnostic labels is examined in terms of operational criteria derived from the original description of the syndrome. Five new cases of erotic delusions are examined, and 53 cases reported in the recent literature are reviewed; none of these completely satisfies the diagnostic criteria, while the majority of patients actually suffered from another disorder, most commonly schizophrenia. The justification for the retention of the syndrome as a nosological entity is discussed, and it is argued that the term should be seen as principally of historical interest.

The reliability of axis II of DSM-III.

The reliability of the diagnoses of axis II of DSM-III was examined by measuring the levels of agreement among three psychiatrists who independently examined the same patients in everyday clinical settings. The kappa coefficient levels were lower than those reported from the field trials.

The relationship between in-service assessment and post-graduate examination performance in psychiatry.

A system of in-service assessment of trainee psychiatrists by their supervisors is reported. It has been used for four years in a large, service delivery organisation. Analysis of the data collected indicates that the supervisors' ratings were significantly related to subsequent trainee examination performance. From the individual scores however, it can be postulated that as a feedback aid to learning, its usefulness may have been limited by the narrow range of ratings given.

Performance correlates in a postgraduate psychiatric examination.

This paper has reported some aspects of an analysis of the correlates of success in candidates' first attempts at the postgraduate psychiatric examinations of the R.A.N.Z.C.P. These are developed to suggest some attributes which selectors of trainees could take into account. Profitable future areas for research are delineated.
