Changes in pituitary hormones serum levels in bromocryptine-treated parkinsonian patients.

In the course of a clinical trial with 2alpha-bromoergocryptin in Parkinson's disease, the serum levels of several pituitary hormones have been studied in the assumption that the drug active on nigro-striatal dopaminergic system might also interfere with hypothalamus-protuberantial neurotransmission, and have effects on the function of the pituitary. No changes in serum levels of FSH, LH, STH and TSH were detected for every dose of the drug employed. Only prolactin serum levels diminished since the beginning of the treatment, the decrease being significant (p less than 0.05 and p less than 0.01). This effect on prolactin does not change in the dose range considered. Clinical improvement was observed for doses of drugs above 15 mg/day, whereas the effect on prolactin secretion occurred with the dose of 7.5 mg/day.

[Neuroendocrine aspects of Parkinson's disease. Therapeutic/effect of a prolactin inhibitor].

On the basis of a reassessment of the aetiopathogenetic problem and the neuroendocrine implications, the therapeutic effectiveness of a prolactin inhibitor, 2-alpha-Br-ergocryptine (CB 154), in Parkinson's disease is assessed. Five patients were treated for a total of two weeks using doses between 10 and 15 mg/die. CB 154 was found to act as a dopaminergic receptor agonist at nigro-striatal level, considerably improving tremor and rigidity and to a lesser extent bradykinesia and total disability.

Mental impairment in Parkinson's disease. The role of anticholinergic drugs.

The trial has been carried out on 33 treated Parkinsonian patients and 14 healthy controls. A series of psychometric tests were employed in order to show a possible intellectual deterioration in Parkinsonian patients. Bender's test showed greater motor-perceptive deterioration in Parkinsonian patients respect to control, the Wechsler-Bellevue Intelligence Scale evidenced in patients significant deterioration of mnesic and psychomotor capacity. When considering drug treatments patients undergoing combined treatment or anti-cholinergics alone obtained worse results in all tests made whilst patients on L-Dopa alone obtained the best results.

Haloperidol decanoate in schizophreniform disorders. Clinical and neuroendocrine aspects.

The recently synthesized depot neuroleptic haloperidol decanoate (HD) has been mainly used in the treatment of chronic schizophrenia. In this trial 23 outpatients, 11 male and 12 female, suffering from schizophreniform disorders, according to the DSM III, were treated with 50 mg to 250 mg of HD, every 4 weeks for 12 months. At the end of the trial, the mean dosage of HD administered and BPRS (Brief Psychiatric Rating Scale) mean scores were significantly lower than the initial ones (70.8 mg VS 108 mg, and 21.5 VS 45.7 respectively). In 10 male patients, plasma levels of haloperidol and PRL were evaluated during the first four weeks of treatment. A positive correlation between haloperidol levels and clinical improvement and a significant increase in PRL levels in correspondence with haloperidol plasma peaks, but not with clinical improvement, were observed. These data indicate a dissociation between the effect of haloperidol after HD injection on the limbic dopaminergic system and the effect at the level of the tubero-infundibular system.

[Treatment of anxiety in Parkinson's disease with bromazepam].

The Authors observe that anxiety is a rather frequent symptom in parkinsonian patients. The possible mechanisms are reviewed. In a double blind cross-over trial, a new antianxiety agent (Lexotan, Roche) was compared with Placebo, to delineate the therapeutic effect in anxious parkinsonian patients. A noteworthy activity was shown by the active drug, while after Placebo a deterioration of anxiety was demonstrated. Tremor was also alleviated by Lexotan and the importance of emotional troubles on tremor seems to be confirmed.
