Tibia osteotomies in sheep were stabilized in nail-fixed and wire-fixed external devices with static longitudinal compression. During the 8-week study programme the mechanical situation of the osteotomies was checked via continuous measurements of interfragmentary tension, used as a functional indicator of stabilization. Simultaneously the test animals were marked by polychromatic fluorescent pigments. The histological findings made at the site of osteotomies showed that the temporal and local progress of ossification was influenced by the degree of stability of osteosynthesis. Under stable osteosynthesis conditions primary bone healing occurred analogous to findings in flattened experimental shaft fractures. On grounds of varying stability performance of external pressure osteosynthesis differences in secondary healing of osteotomies were observed due to interfragmentary instability.
Histomorphological diagnosing very much depends on subjective empiric data. In histomorphology of the endometrium nomenclature as well as diagnostic classification lack uniformity and are full of controversy in literature. This results in opposing assessments of dignity of endometrial hyperplasias and prognosis of varying grades of carcinoma differentiation. One of the reasons is the lack of objective and reproducible diagnostic categories, not yet found. Morphometry with so-called computer-assisted interactive pattern recognition systems might offer the solution to numerous problems. Since there are no specific parameters for diagnostic identification of endometrial hyperplasias and carcinomas, only a combination of sensitive and reproducible parameters can allow for diagnostic identification. We report on our first computer-assisted pattern recognition studies and results that appear to provide an adequate basis for leaving descriptive diagnosing and turning to objective diagnosing. Such an objective diagnosing might not only be of clinical and therapeutic significance but also provide a new evaluation of the today's significance -- still full of controversy -- and prospective potency of endometrial hyperplasias. In computer-assisted interactive pattern recognition also lies the possibility of a contribution to objective tumour grading that appears to have high prognostic significance.
A frustrating clinical problem of recurrent streptococcal pharyngitis affected the clinical health, economic well-being, and social function of a family of four. Numerous attempts using conventional methods of treatment failed to permanently eradicate group A beta-hemolytic streptococcal infection from this family. Their pet dog was finally discovered to be a carrier of group A streptococci and was treated concurrently with all family members. Subsequently, no group A streptococcal infections recurred. This report illustrates the importance of detailed investigation of the patient's environment and contacts, including nonhuman contacts, in cases of recurrent infection with a streptococcal agent.

Seventy-seven patients aged more than 60 years from the iodine deficiency area of southern Lower Saxony with hyperthyroidism, diagnosed by assessment of thyroid parameters, showed a nonspecific disease picture with surprising frequency when compared with younger patients. Predominant characteristics were weight loss (86%), general weakness (78%), tachycardia (74%), reduced appetite (66%), exertional dyspnoea (60%), apathy, lack of initiative or depression (48%), and uncharacteristic abdominal complaints (20%). A goitre was not palpable in 30 patients (39%). Laboratory diagnosis was hampered by relative or absolute TBG deficiency and conversion inhibition in the form of a low-T3 syndrome as evidence of non-thyroidal changes in generalised disorders. Neither thyroxine nor triiodothyronine were increased in ten patients (13%). Only by additional routine estimation of the thyroxine-binding index (T3 in vitro test) as indicator of free binding capacity could the diagnosis be ascertained in these cases. The poor prognosis be ascertained in these cases. The poor prognosis of hyperthyroidism in the older age group necessitates generous use of specific diagnostic laboratory investigations in cases with suspicious symptoms.

The article reports on the measurement of static compression during the consolidation phase of tibia osteotomies in sheep. Nail-fixed and wire-fixed compression and distraction devices were used for external stabilization. During the 8-week study period, the magnitude of the interfragmentary pressure and its changes were measured via pressure meter units. The links between the extent of interfragmentary tension and the degree of stability of osteosynthesis, are explained, and attention is also drawn to the differences in the stability performance of the nail-fixed and wire-fixed compression and distraction devices used in the animal experiments.
