Alpha-1-antitrypsin (AAT) deposits in gall bladder adenocarcinoma and liver in partial AAT deficiency (Pi SZ phenotype).

Alpha-1-antitrypsin (AAT) immunoreactive inclusions were found in an adenocarcinoma of a gall bladder of a patient with Pi SZ phenotype who also had globular AAT accumulation in the liver. The inclusions did not react with antibodies against other plasma proteins, thus suggesting a primary synthesis of AAT in these cells. The presence of AAT in the tumor cells may represent resurgence of an oncofetal antigen, or, indirectly, may mean that AAT-containing cells normally exist in the human non-neoplastic gall bladder. The accumulation of AAT in tumor cells might be a basic process in malignant transformation.

Alpha-1-antitrypsin phenotypes in malignant lymphoma.

The alpha-1-antitrypsin (alpha 1 AT) Pi phenotypes have been determined by isoelectric focusing in a series of 228 patients having a histologically diagnosed malignant lymphoma and in 250 healthy controls. The Pi MZ phenotype occurred in 13 patients with lymphoma (5 . 8%) and in five of 250 healthy individuals (2%). Furthermore, one patient with a Pi SS and three patients with an abnormal unknown phenotype, migrating slower than Z, were found in the lymphoma group. No prevalence for a special lymphoma type was observed among the abnormal Pi phenotype patients. The increased incidence of abnormal Pi phenotypes in malignant lymphoma's support the hypothesis of the possible role of alpha 1 AT in development of immunopathological disorders.

PiM subtypes in bronchiolitis.

Decreased alpha-1-antitrypsin (alpha 1AT) levels have been observed in children affected by bronchiolitis; however, variant alleles Pi (S, Z) which are usually associated with reduction of the inhibitor do not show a higher incidence in children affected by this disease. Subtypes M2 and M3 of alpha 1AT have recently been considered to play a role in the development of allergic states in children as well as in chronic obstructive lung disease in adults. In the present study we have investigated the incidence of PiM subtypes in 98 children affected by bronchiolitis. No greater incidence of subtypes M2 and M3 was observed in these children than in controls. These results rule out the hypothesis of a pathogenetic role of M subtypes in the development of bronchiolitis in children.

[A catamnestic study of recovered alcoholics].

Fifty-one alcoholics, previously admitted to the hospital for medical care and treated with disulfiram, were re-examinated after 1-3 years. In thirty of them (58%) total abstinence was found; eighteen patients (35%) behaved so in spite of disulfiram withhold during the first year of self-administration. No side-effects were reported. General behaviour, mood, work, interest and familiar relationships improved. The treatment generally failed in the cases of secondary alcoholism as in marked neurosis or psychosis. The research proved the main role of family and environment in the successful cases. The treatment with disulfiram is useful but incomplete.

Characterization of alpha-1-antitrypsin by isoelectric focusing on an ultrathin polyacrylamide gel layer. An economic high-resolution system for determining PiM subtypes.

Through the use of ultrathin layer polyacrylamide gel isoelectric focusing it is possible to obtain a resolution of the bands of alpha 1AT so as to be able to easily recognize all six PiM subtypes. The optimal resolution of the PiM subtypes is obtained without deforming the pattern of the Pi phenotypes. In addition to high resolution, ultrathin layer polyacrylamide gel isoelectric focusing permits a notable reduction of fees to 1/5 of the usual.

Different behaviour of 19S thyroglobulin extracted from normal thyroid, simple goitre and toxic goitre in polyacrylamide slab gel isoelectric focusing.

19S thyroglobulin (19S TG) was isolated from normal thyroids, simple goitres and one case of toxic goitre (Graves' disease). With isoelectric focusing on polyacrylamide gel slabs (pH 4--6) several bands for each specimen were detected. The migration patterns of the toxic goitre 19S TG were clearly different from the patterns of normal 19S TG and simple goitre 19S TG.

Alpha-1-antitrypsin phenotypes in a group of newborn infants in Somalia.

Pi phenotype determinations were performed on umbilical cord serum specimens of 347 infants born in Mogadishu (Somalia) hospitals. Phenotyping was performed by isoelectric focusing on polyacrylamide gel slabs. Sera containing alpha1AT in types other than MM were also studied by crossed antigen-antibody immunoelectrophoresis on agar. The most frequently occuring Pi allele proved to be PiM. With respect to the other African populations studied so far, there was a high prevalence of the PiS and PiZ alleles. In particular, three individuals were found with the ZZ phenotype, which had not yet been observed in Africa.

[Complex duodenopancreatic injuries].

Injuries of the duodenopancreatic region are rare and difficult to diagnose and treat. The related high mortality is mainly due to the presence of associated lesions. Complex traumas (AAST grade IV and V lesions) require difficult surgical treatment with high postoperative morbidity and mortality rates. In a review of 200 pancreaticoduodenectomies performed for pancreatic head traumas the postoperative mortality was 31%. The authors present 6 cases of complex duodenopancreatic traumas, treated from 1995 to 1999. The aetiology was blunt trauma in 5 cases (83%) and a shotgun wound in 1 case (17%). In 3 cases, with a grade V lesion of the pancreatic head, a pancreaticoduodenectomy was performed. A case of a grade IV lesion of the tail of the pancreas was treated with distal splenopancreatectomy. Two cases of grade IV lesions of the third part of the duodenum were submitted to duodenal resection with direct anastomosis. One postoperative death was observed in a patient treated with duodenal resection. The overall mortality was 16%. A pancreatic fistula, which healed spontaneously, was observed in a case of pancreaticoduodenectomy.
