The effects of some phosphodiesterase (PDE) inhibitors (dipyridamole, theophylline, papaverine and SH-869) on prostacyclin (PGI2) production have been studied in vitro and in vivo. PGI2 was bioassayed by Vane's superfusion technique. In rabbit aortic rings, only dipyridamole in concentrations from 1 to 12 microM was able to stimulate PGI2 biosynthesis in a dose-dependent manner. This effect was also detected with so-called "exhausted" rabbit aortic rings. The other PDE inhibitors used, both in microM and mM concentration, did not affect PGI2 biosynthesis. Dipyridamole was found to increase PGI2 production in healthy volunteers, when given both by infusion (8 micrograms/kg/min x 2h) and by oral administration (375 mg/day for seven days). Circulating PGI2 and PGI2 production induced by a 3-min period of ischaemia were increased by an average of 137% (p less than 0.001) and 30.8% (p less than 0.001) respectively. Saline and theophylline (as aminophylline) infusions used as controls did not affect PGI2 production.

The effects of bladder distension on the production of prostaglandins (PGs) were studied in an isolated rabbit bladder preparation. Increasing bladder distensions induced a proportional release of PGs together with a parallel increase in endovesical pressure; pretreatment with indomethacin prevented both. Infusion of exogenous PGs into the arteries of bladders previously filled and treated with indomethacin produced a rise in endovesical pressure similar to that observed subsequent to distension of the untreated bladder. Our results suggest that PGs and particularly PGE2 play a role in the maintenance of vesical tone during distension.

The authors have examined 1478 burned or scalded patients of the Plastic Surgery Institute of the University of Parma from 1967 to 1976; they have considered particularly 92 cases who showed more than 20% of burned skin. For these patients they have considered many data of medico-legal interest in order to facilitate the estimate of the personal damage in penal and civil law.
The author points out the importance of the medico legal doctor's presence in the survey also in cases of mortal road accidents. In fact, only the previous knowledge of the problems and data concerning the case can consent the expert to give their value to general findings which thus become meaningful in order to solve the case. To this purpose he refers to a case he personally observed in which through the interpretation of a single skin and lower bone wound it was possible to identify the part of the vehicle which had produced it.
