Effects of medical and surgical treatment on cerebral perfusion and cognition in patients with chronic cerebral ischemia.

The effects of medical treatment with and without cerebral revascularization procedures on cognition and cerebral blood flow were compared among 36 patients with extracranial occlusive cerebrovascular disease and cognitive impairments. Three comparable groups were studied. The first group (N = 18) received only medical treatment by control of risk factors for stroke (including hypertension, diabetes, and hyperlipidemia) and antiplatelet aggregant medication. The second group (N = 10) had the same medical treatment plus superficial temporal-to-middle cerebral artery bypass, and the third group had the same medical treatment plus carotid endarterectomy. Regional cerebral blood flow and cognition were monitored in all three treatment groups over a 3-year interval. All groups showed stabilization without expected rates of decline for both cerebral blood flow and cognition, but no statistically significant differences emerged among the treatment groups.

Transvaginal sonographic assessment of the endometrium in spontaneous and induced cycles.

Transvaginal sonography was used and endometrial response patterns of 3 groups of patients (Spontaneous cycles, hMG cycles, and Clomiphene Citrate cycles) were observed based on endometrial characteristics of thickness and texture. All groups demonstrated cyclical changes evident by transvaginal sonography. Clomiphene Citrate cycles showed retardation of endometrial growth during the proliferative phase and delayed endometrial development based on textural patterns. There exists a positive correlation between estrogen levels and endometrial thickness in induced cycles, which is not observed in spontaneous cycles. There were no correlations between hormonal levels of progesterone, FSH and LH, in relation to endometrial characteristics of thickness and texture.

Frequency of the -alpha 3.7 thalassemia deletion in the non-white Cuban population.

DNA analysis of the alpha-globin gene cluster showed that the frequency of the -alpha 3.7 deletion in the non-white cuban population is about 0.12, in agreement with a 47% admixture of the original African slaves introduced in the island. In the S-S homozygotes the frequency is slightly higher (.18), probably due to a selective advantage produced by the presence of alpha-thalassemia in sickle cell patients.

A case of hereditary persistence of fetal hemoglobin caused by a gene not linked to the beta-globin cluster.

The pattern of inheritance of several polymorphic restriction sites associated with the beta-gene cluster, and spanning a region of 52kb, demonstrates that a determinant for hereditary persistence of fetal hemoglobin (HPFH) segregates independently from the non-alpha globin gene cluster, as we postulated several years ago on purely genetical grounds. This finding provides additional evidence for the existence of diffusible factors affecting gamma-chain expression. Moreover, we have identified a "private" HincII polymorphism, in the vicinity of the epsilon gene in the family studied.

Use of enzyme-developed immunoblots scanned by densitometry to compare lot-to-lot variability of Russian-thistle extracts.

Two different lots of Russian-thistle extracts from five commercial sources were evaluated by isoelectric focusing, sodium dodecyl sulfate-polyacrylamide gel electrophoresis, and immunoblots of Immobilon transfers from these separations. The immunoblots were developed by an enzyme-linked method and scanned with a densitometer. The two lots of a standardized extracts produced scans that suggested uniformity in allergenic components. Several of the nonstandarized extract lots also appeared quite comparable. With further refinement, immunoblotting with densitometric analysis appears to offer an attractive way to standardize allergen extracts.

Epidural neurostimulation in the treatment of frostbite.

Epidural spinal cord stimulation (SCS) and conventional treatment produced good therapeutic effects in four young patients with frostbite of the lower limbs. The mechanism of action is unknown but the treatment resulted in rapid recovery with reduced pain and a more peripheral level of amputation. These preliminary results warrant further studies of SCS in the treatment of frostbite.

Alpha-thalassemia changes the cell density profile in sickle cell anaemia.

Several factors are implied in the haematological and clinical picture of sickle cell anaemia. Attention has been focused on the concomitant presence of -alpha-thalassemia and high levels of HbF, but contradictory results have been reported in different populations. We compared the blood cell density profile, obtained by the phtalate esther method, of normal subjects with those of patients with sickle cell anaemia - with or without heterozygous alpha-thalassemia. We found that the density profile of both groups of patients differs from normal subjects, and that a difference can also be demonstrated between normal alpha genotype patients with sickle cell anaemia and patients with heterozygous alpha-thalassemia. These results are in agreement with the findings obtained in other countries in which a gene from Caucasian to African populations have been demonstrated, and are different from the results obtained in populations of more pure African ancestry. It can be suggested, therefore, that these data, in addition with findings of other authors in different geographical areas, support the hypothesis that the genetic make up plays an important role in the haematologic and clinical picture of sickle cell anaemia.

Role of pancreatic duct occlusion with prolamine (Ethibloc) in necrotizing pancreatitis.

There is evidence that the pancreatic duct plays an important role in the evolution of necrotizing pancreatitis. We hypothesized that occlusion of the pancreatic duct and its smaller ductules with prolamine (Ethibloc) in opossums at risk of severe necrotizing pancreatitis would have a beneficial effect on the progression of the disease. Sixteen opossums underwent bile duct ligation below the entrance of the pancreatic duct. They were divided into four groups at 6 days. Group I (control, n = 5) opossums were killed for histologic observation of the degree of necrosis of the pancreas; group II (n = 5) underwent external drainage of the pancreatic biliary duct; group III (n = 4) had external biliary drainage and ligation of the pancreatic duct; group IV (n = 7) was treated with external biliary drainage and ligation of the main pancreatic duct after instillation of prolamine. Serum amylase, lipase, and calcium values were determined. The pancreas was examined by inspection and histologically at the time of death, and the severity of the disease was determined by quantitation of pancreatic tissue necrosis. All animals in groups II and III died 8 to 14 days after bile duct ligation, and all had severe necrotizing pancreatitis. All animals in group IV survived and were killed at 2 to 10 weeks after prolamine (Ethibloc) injection into the pancreatic duct. A mild edematous pancreatitis was observed in all seven animals. Prolamine (Ethibloc) provided dramatic protection from progressive necrosis. This study does not provide an explanation, but it allows for speculation that ductal injection interrupted the deleterious effect of proteolytic enzymes and their leakage into the interstitial space of the pancreas.

[Cutaneous mycobacteriosis caused by Mycobacterium chelonei var. abscessus].

We are commenting on a case of cutaneous mycobacteriosis (M. chelonei) in an asthmatic 60-year-old patient under continuous steroid treatment, who months after an accidental trauma on his hand, developed painful papular lesions, located over the trauma scar, and on the dorsal lateral side of the forearm. The lesions on the forearm were accompanied by epitrochlear adenopathies and a general reaction in the form of fever. Histologically the lesions showed a subacute inflammatory reaction with lymphocytes, histiocytes, polynuclear neutrophils and areas of necrosis. The staining of the exudate with Ziehl-Neelsen and the histological samples were negative, but the culture was identified as M. chelonei, of the abscessus variety. The process was cured with continuous therapy with cefotaxime and procodazol.
