Single-unit recordings at dorsal raphe nucleus in the awake-anesthetized rat: spontaneous activity and responses to cutaneous innocuous and noxious stimulations.

In this study, we recorded the single-unit activity of the dorsal raphe nucleus (DRN) in rats tested first awake and, a few days later, anesthetized with sodium pentobarbital and recorded again. This was achieved by means of a small chronically implanted device supporting a 25 micron platinum-iridium wire as the recording electrode. In both the awake and anesthetized conditions, and in agreement with most of the studies performed at the DRN level, we found that a vast majority of the units, displaying small amplitude and long-duration action potentials, possessed a low level of spontaneous activity (0.2-4 Hz). Among these units, found in greater number under pentobarbital, it was possible to establish that this activity was regular or irregular, in accordance with the literature reports. However, as opposed to these studies, we determined that the 'regularity' was relative, only noticeable in more or less prolonged phases of activity. In particular, we never recorded the so-called 'clock-like' activity, largely reported as an unambiguous criterion for selecting the serotoninergic neurons. In both the awake and anesthetized conditions, the responses of the DRN neurons to peripheral mechanical innocuous and noxious stimulations were observed in only one-half of the units recorded and were weak in comparison to other results that we obtained at the nucleus raphe magnus level in previous studies. When present, these responses were excitation or inhibition, occurring during or after the stimulus application. These results question the direct involvement of the DRN in acute nociception.

Oxygen consumption and cardiovascular function in children during profound intraoperative normovolemic hemodilution.

The clinically acceptable limit of acute normovolemic, normothermic hemodilution, a standard procedure in scoliosis surgery, is not yet well defined. Eight ASA class I patients undergoing idiopathic scoliosis correction were administered a standard anesthetic with 100% oxygen and controlled ventilation. Hemodilution was accomplished by exchanging whole blood for 5% albumin in 0.9% saline. Blood gases, acid-base status, and circulatory variables were recorded prior to and after hemodilution, and every 30 min throughout surgery. The impact of hemodilution was judged by mixed venous oxygen saturation which was maintained at > or = 60%, while intravascular volume was maintained with the 5% albumin solution. Reinfusion of the autologous blood was completed by the end of surgery. In the eight controlled cases in which normovolemic hemodilution was studied, hemoglobin levels decreased from 10.0 +/- 1.6 g/dL to 3.0 +/- 0.8 g/dL. Mixed venous oxygen saturation decreased from 90.8% +/- 5.4% to 72.3% +/- 7.8%. Oxygen extraction ratio increased from 17.3% +/- 6.2% to 44.4% +/- 5.9%. Oxygen delivery decreased from 532.1 +/- 138.1 mL.min-1.m-2 to 260.2 +/- 57.1 mL.min-1.m-2, while global oxygen consumption did not decrease and plasma lactate did not appreciably increase. Central venous pressure increased and peripheral resistance decreased during hemodilution. Cardiac index increased, heart rate remained essentially constant, and left ventricular stroke work index did not decrease significantly. No patients suffered clinically adverse outcomes. Global oxygen transport and myocardial work can be maintained at extreme normovolemic anemia. Our evidence suggests that stages of normovolemic hemodilution more severe than previously reported may be clinically acceptable for young, healthy patients during normocarbic anesthesia.

Significance of touch for perceptions of parenting and psychological adjustment among adolescents.

OBJECTIVE: This pilot study set out to investigate whether a relationship exists between differential quality and quantity of physical contact experience and perceptions of parenting, psychological adjustment, and suicidal behavior among adolescents. METHOD: A self-report questionnaire assessed individuals' perceptions of how frequently they experienced pleasant (positive) and unpleasant (negative) touch from family and friends. Together with the Achenbach Youth Self-Report and the Parental Bonding Instrument, this questionnaire was administered to 142 male and 129 female students aged between 13 and 15 years (mean = 13.54, SD = 0.56) attending a randomly chosen metropolitan state high school and a coeducational private school. RESULTS: The major findings indicated that differential quality and quantity of physical contact experience was related to perceptions of parenting, psychological adjustment, and suicidal behavior, although a gender difference was observed. CONCLUSIONS: The findings suggest that physical contact experiences may reflect different parenting styles and may be a vulnerability factor for a range of difficulties among adolescents.

Rapid and sensitive non-radioactive assay for the detection of clonal gene rearrangements in B-lineage acute leukaemia.

We have applied a simple and non-radioactive assay system used in conjunction with polymerase chain reaction amplification and high-resolution polyacrylamide gel electrophoresis to detect clonal immunoglobulin gene rearrangements in leukaemia patient DNA. This technique is as sensitive as isotopic methods, allowing the detection of clonal DNA diluted 1 in 10,000 in non-clonal DNA.

Adolescent suicide, depression and family dysfunction.

This study investigated associations between adolescents' perceptions of their family dynamics (McMaster Family Assessment Device, FAD) and depression, suicide thoughts and attempts. High school students (mean age 15.2 years) completed self-report questionnaires including the Beck Depression Inventory (BDI), the FAD, questions about suicidal thoughts, plans and attempts, deliberate self harm, and selected life experiences. Univariate analysis showed that family dysfunction measured on the FAD is associated with thinking and planning suicide, deliberate self harm, suicide attempts, as well as severe depression (BDI > or = 22). Despite this, stepwise regression indicated that family dysfunction influences suicide behaviors indirectly through other variables such as depression. The FAD is recommended as a useful addition to questionnaires seeking to identify vulnerability to both depression and adolescent attempted suicide in early detection studies.

Continuing medical education in marital and family therapy: a survey of South Australian psychiatrists.

OBJECTIVE: South Australian psychiatrists were surveyed to determine their impressions of the usefulness of marital and family therapy (MFT) in the management of serious psychiatric conditions and to ascertain their previous experience with Continuing Medical Education (CME) about family therapy. It was expected that psychiatrists' preferences regarding CME would be related to their clinical experience of the usefulness of MFT. METHOD: One hundred and twenty psychiatrists returned a questionnaire about their training, clinical and research interests, with ratings of the usefulness of MFT and CME preferences. This represented 65% of those eligible for the CME programme. RESULTS: Thirteen percent of the respondents found MFT to be extremely useful and a further 47% found it moderately useful in their current practice. There was evidence of a possible training effect: respondents who had previous CME rated MFT as more useful, especially for mood disorders. Furthermore, the treatment of mood disorders seemed to have a particular relevance in family psychiatry, making a statistically unique contribution to ratings of MFT usefulness in the respondents' total practice. Sixty-nine percent of the respondents requested further CME in family therapy. This represented 45% of all South Australian psychiatrists. Respondents who rated MFT as more useful in practice were significantly more likely to be interested in CME. CONCLUSIONS: There seems to be sufficient interest and clinical experience among psychiatrists for MFT to be included in CME courses. It is recommended that further training focus on major mental disorders, especially mood disorders and schizophrenia.

Four days' continuous infusion of cisplatin-5-fluorouracil and short daily infusion of high-dose leucovorin as induction chemotherapy for locally advanced head and neck cancer.

Cisplatin-fluorouracil (PF) is the most frequently used combination as induction chemotherapy (CT) for the treatment of locally advanced squamous cell head and neck cancer. This study was designed to evaluate the role of leucovorin (L) in the modulation of the therapeutic activity of PF continuously infused for 4 days. Between June 1990 and June 1992, stage III and IV previously untreated patients received PFL induction chemotherapy followed by surgery, radiotherapy, or both. The chemotherapy consisted of P 25 mg/m2, F 1000 mg/m2 both continuously infused for 4 days and L 250 mg/m2 infused for 2 hours before each daily infusion of PF. PFL was administered every 3 weeks for 4 cycles. The overall response rate at the completion of PFL was 91%, with 54% CR and 37% PR. Locoregional treatment was performed on 68 patients, 11 (16%) underwent surgery, 20 (29%) surgery plus radiotherapy, and 37 (54%) radiotherapy. Complete response status after both induction and locoregional therapy was 71%. Biopsies of the primary tumor or definitive resection specimens immediately after PFL therapy were available in 25 patients in CR. Pathological CR was found in 11 (44%). With a maximum follow-up of 44 months, the overall survival rate is 51% and the median survival has not been reached. PFL is highly active as induction chemotherapy. A randomized comparison between PF and PFL is necessary to define the place of leucovorin modulation in the treatment of head and neck cancer.

Induction of mammary tumors in rat by intraperitoneal injection of NMU: histopathology and estral cycle influence.

In order to obtain an experimental model we induced mammary tumors in female Sprague-Dawley rats. The carcinogen N-nitroso-N-methylurea (NMU) was injected intraperitoneally (i.p.) at doses of 50 mg/kg body weight when animals were 50, 80 and 110 days old. Tumor sizes were measured with a caliper and their growth parameters and histopathological properties were tested. For 100 rats, 88.4% of developed lesions were ductal carcinomas, histologically classified as 52.8% cribiform variety, 30.6% solid carcinoma. Metastases in liver, spleen and lung were present. Other primary tumors were detected with low incidence. The influence of the rat estrous cycle during the first exposure to intraperitoneal NMU injection was studied. The latency period in estrus, proestrus and diestrus was 82 +/- 15, 77 +/- 18 and 79 +/- 18 days, respectively. Tumor incidence was significantly higher in estrus (95.2%) than proestrus (71.4%) or diestrus (77.4), (P < 0.01). Mean number or tumors per animal was similar among the three groups (4.4 +/- 3.2, 3.8 +/- 3.6, 3.2 +/- 1.8). The procedure described appears to be the simplest method for inducing experimental mammary tumors in rats.

The rabbit kidney tubule utilizes glucose for glutamine synthesis. A 13C NMR study.

The metabolism of variously labeled [13C]- and [14C]glucoses, used at a physiological concentration (5 mM), has been studied in isolated rabbit kidney tubules both in the absence and the presence of NH4Cl. When present as sole exogenous substrate, glucose was metabolized at high rates and converted not only into CO2 and lactate but also, in contrast to a previous conclusion of Krebs (Krebs, H.A. (1935) Biochem. J. 29, 1951-1969), into glutamine. Absolute fluxes through enzymes of glycolysis and gluconeogenesis and of enzymes of three different cycles operating simultaneously were assessed by using a novel model describing reactions of glucose metabolism in conjunction with the 13C NMR and, to a lesser extent, the radioactive data obtained. The presence of NH4Cl (5 mM) caused a large stimulation of glucose removal and a large increase in lactate, glutamine, and glycerol 3-phosphate accumulation. Under this condition, the stimulation of glutamine synthesis was accompanied not by an activation of citrate synthesis but by an inhibition of flux through alpha-ketoglutarate dehydrogenase. The resulting depletion of citric acid cycle intermediates was compensated by anaplerosis at the level of pyruvate carboxylase. The "futile" cycle involving oxaloacetate, phosphoenolpyruvate, and pyruvate, which was intense in the presence of glucose alone, was greatly stimulated by the addition of NH4Cl.

Non-steady state model applicable to NMR studies for calculating flux rates in glycolysis, gluconeogenesis, and citric acid cycle.

We present a mathematical model for calculating most reaction rates of glycolysis, gluconeogenesis and citric acid cycle in mammalian cells. The model also includes cycles such as the "phosphoenolpyruvate (PEP)-->pyruvate-->oxaloacetate-->PEP" cycle and the "pyruvate-->acetyl-CoA-->citrate-->citric acid cycle-->oxaloacetate-->PEP--> pyruvate" cycle. The model, which does not require steady state conditions, is based on a set of equations, each one describing the fates of a given carbon of a selected intermediate. These fates are expressed as ratios of integrated transfer of this carbon to corresponding carbons in subsequent metabolites. At each bifurcation, the sum of all proportions adds up to 1. Among several calculation routes to determine a proportion value, we chose the one that was based on the most reliable parameter determined experimentally. The data introduced in the model are the micrograms of atom of traced carbon measured on each carbon of a number of products (corrected for natural tracer abundance). These incorporations can be measured by 13C NMR, gas chromatography-mass spectroscopy, or 14C counting. Thanks to its flexibility, this model can be applied to data obtained with substrates other than glucose under many experimental conditions.

Estrogen receptor-associated proteins: possible mediators of hormone-induced transcription.

The estrogen receptor is a transcription factor which, when bound to estradiol, binds DNA and regulates expression of estrogen-responsive genes. A 160-kilodalton estrogen receptor-associated protein, ERAP160, was identified that exhibits estradiol-dependent binding to the receptor. Mutational analysis of the receptor shows that its ability to activate transcription parallels its ability to bind ERAP160. Antiestrogens are unable to promote ERAP160 binding and can block the estrogen-dependent interaction of the receptor and ERAP160 in a dose-dependent manner. This evidence suggests that ERAP160 may mediate estradiol-dependent transcriptional activation by the estrogen receptor. Furthermore, the ability of antiestrogens to block estrogen receptor-ERAP160 complex formation could account for their therapeutic effects in breast cancer.
