[Malignant neoplasms of the nasal cavity and paranasal sinuses. Analysis of 216 cases of malignant neoplasms of nasal cavity and paranasal sinuses].

This study is a retrospective analysis of 216 cases of malignant tumours of the nasal cavity and paranasal sinuses, treated from 1965 to 1980. The mean age at diagnosis was 59 years, 70 per cent were men and 30 per cent women. There were 121 (56%) epidermoid carcinomas, and 31 (14%) adenocarcinomas. Three-year and five-year actuarial survival rate was 40 and 33 per cent respectively for epidermoid carcinomas, 72 and 50 per cent respectively for adenocarcinomas. Recurrences occurred in 46 per cent of epidermoid carcinomas and 42 per cent of adenocarcinomas. Early diagnosis is important to improve the survival rate, and radical primary treatment should be carried out to reduce the rate of recurrence.

[Cancer of the valleculal and laryngeal vestibule treated by supraglottic horizontal laryngectomy. Apropos of 173 surgically treated cases at the Institut Gustave-Roussy].

The authors make a report on a series of 173 supraglottal horizontal laryngectomies carried out from 1970 to 1984 at the Gustave-Roussy Institute of Villejuif. After recalling of operability conditions and tackling characteristics of this series are quoted variants required from tumours localisations diversity and importance of extension. Then are treated clinical and anatomo-pathological classification as briefly surgeries results. This work is specially concerning in survival by analysing different topographical clinical and histological criteria. Survival of 173 patients, having gone through an supra-glottal laryngectomy is 45.5% at 3 years and 31.3% at 5 years. At least, reasons of failure are analysed. This kind of partial surgery caring of voice, should be proposed to patients affected by vallecula or epiglottis carcinomas as regards of good functional and improved carcinological results.

Synovial sarcoma of the head and neck: an account of four cases and review of the literature.

Four cases of Malignant Synovioma of the head and neck are reported. It is a rare tumour with only 76 published cases in the world literature. Study of these 76 cases allows identification of the clinical features of this tumour and also highlights the problems of histological diagnosis. The study shows that the most effective treatment is radical primary surgery in association with post-operative radiotherapy and chemotherapy.

[Secondary reconstruction of the buccal floor by total skin graft after noninterruptive pelvimandibulectomy].

A technique of secondary reconstruction of the buccal floor after non-interruptive pelvimandibulectomy by thin-split skin graft enables insertion of a dental prosthesis to assist functional recovery. Results of 21 grafts of this type performed at the Institut Gustave Roussy, Villejuif, France are analyzed, this operative procedure enabling 14 of these patients to start taking solid food again.

[Reflections concerning the reconstruction of the pinna of the ear (author's transl)].

Reconstruction of the pinna of the ear is a difficult enterprise and overall results are poor. It is nevertheless essential to persist since despite the hazards of such surgery many patients wish to have an operation since they are unable to tolerate their deformity. Apart from the cutaneous problem, the most important point remains that of bitterness. There are still many partisans of the use of costal cartilage, but it would seem legitimate to also try new implantable substances, in particular microporous. Whatever the choice, it is important in all cases to establish a programme rather then improvise. This is the only way to reduce the number of operative stages, to avoid untoward scars and to decrease the number of "touching up" procedures. Despite the many difficulties which must still be resolved, a certain number of modern studies offer hope of an improvement in results.

[A rare and late complication of rhinoplasties (author's transl)].

The majority of failures after rhinoplasty are of operative origin and rapidly become evident. The two cases of complications reported here were on the contrary late since they occurred after respective periods of 4 and 10 years. They involved mucoid cysts by inclusion of nasal mucosa, requiring complete excision via an external approach.

Nodal failures in patients with NO N+ oral squamous cell carcinoma without capsular rupture.

BACKGROUND: The efficacy of postoperative irradiation of the neck after lymph node dissection in terms of prevention of cervical node recurrence (NR) has not been demonstrated in patients with NO squamous cell carcinoma of the oral cavity. METHODS: This multicenter retrospective analysis comprises 826 patients with squamous cell carcinoma of the oral cavity, all clinically NO. The primary tumor was treated by resection or brachytherapy. All patients underwent cervical dissection adapted to the site of the tumor. Fourty seven N+ patients with capsular rupture were excluded; 160 patients were N+ without capsular rupture (N+ CR-), and 619 were N-. Postoperative cervical irradiation was performed in 67 of 160 N+ CR- patients and in 49 of 619 N- patients. RESULTS: NR developed in 78 patients, associated with local recurrence in 33 cases and isolated in 45 cases. Twenty-six of the 45 cases of isolated NR occurred in the 619 N- patients (4%), and 19 occurred in the 160 N+ CR- patients (12%, p = .001). The 26 NR observed in the N- patients occurred in nonirradiated patients. Among the 19 NR observed in the N+ CR- patients, the incidence of recurrence was not significantly different between irradiated patients (6 NR of 67.9%) and nonirradiated patients (13 NR of 93, 14%). NR rates also did not differ according to the number of lymph nodes invaded nor according to the level of the positive nodes; 14 of 45 isolated NR occurred in a nondissected suprahyoid region. Of 779 patients, 255 (33%) subsequently developed a metachronous cancer; 153 upper respiratory and digestive tract tumors, 37 lung tumors, 33 esophageal tumors, and 32 other tumors. Isolated cervical failure was responsible for 40 deaths. CONCLUSION: The low NR rate in NO N+ CR- patients means that postoperative irradiation can be confined to N+ CR+ patients and, as a precautionary measure, to patients with more than 3 N+ CR-. Keeping irradiation in reserve allows the treatment of metachronous cancers, which are particularly frequent in these patients, in whom the 5-year survival rate is 54% in N+ CR- and 69% in N-.

Squamous cell carcinoma of the pyriform sinus: retrospective study of 351 cases treated at the Institut Gustave-Roussy.

The first part of the study was devoted to 199 tumors treated by surgery, either conservative for the smallest tumors (18 cases) or radical (181 cases), with systematic postoperative radiotherapy. The 3-year survival rate was 48% and the 5-year, 33%, with a 12% local recurrence rate, a 7.5% neck recurrence rate, and 27.6% rate distant metastases. Histologic correlations were developed. The second part of the study reported 152 cases treated by external radiotherapy alone either as a variant of our treatment protocol for the small-sized tumors (31 cases) or, for the major part (121 cases), as a result of surgical inoperability or patient refusal. The former subgroup had a variable survival rate (65% at 3 years and 40% at 5 years) equivalent to similarly staged patients treated with conservation laryngeal surgery, whereas the prognosis of the latter subgroup was poor. The two main causes of failure were the inability to apply the curative treatment protocol in 35% of patients ineligible for a surgery and the high risk of distant metastases in the 65% of patients able to undergo the usual management.

Value of high-frequency US for preoperative assessment of skin tumors.

The purpose of this study was to evaluate the accuracy of high-frequency ultrasound (US) in the preoperative assessment of skin tumors. A US scanner with a 20-MHz probe was used to visualize and evaluate 70 skin lesions (38 clinically suspected melanomas and 32 suspected basilar cell carcinomas [BCCs]) before surgical resection. A US morphologic study and a Doppler analysis of vascularity were performed for each tumor. Of the 70 tumors, 62 were clearly visualized, including 19 melanomas, 12 nonmalignant nevi, and 31 BCCs. Most lesions were hypoechoic. In 13 of 19 proved melanomas, the difference between the histologic and US measurements was equal to or less than 0.2 mm. Vessels were visualized in melanomas with thicknesses greater than 3 mm. All BCCs were visualized, and in 29% of cases of BCC, tumor size at US was greater than that at clinical examination. High-frequency, high-resolution US is a simple, reliable, noninvasive method for accurate preoperative assessment of skin tumor dimensions. This technique allows surgical planning to be adapted and reexcision to be avoided. However, its role is limited in the differential diagnosis of malignant and benign skin lesions.

[Tumors of the mandible. Justification of a second reconstruction after recurrence on the first revascularized bone graft].

The use of free vascularized bone grafts for mandibular reconstruction has led to a consideration of the new entity of recurrent tumour on bone grafts. Exeresis of the mandible was performed in 4 patients with bone tumours: osteosarcoma 1, osteochondroma 1, carcinoma 1 and ameloblastoma 1. A free vascularized bone graft was used for the reconstruction. Tumours recurred with a delay of 10 to 18 months after the initial reconstruction. The frequency of tumoural recurrence can be measured in our series of 46 patients (24 with fibular grafts, 15 with scapular grafts, 4 with radius grafts and 3 with iliac bone grafts). Recurrence rate at our institution was 4.3%. The 2 other cases involved patients from other institutions. The main factor involved in tumoural recurrence is the aggressive nature of the primary tumours, even after extensive initial resection as performed in our patients. Free bone grafts were invaded as were neighbouring tissues. Secondary reconstruction of the mandible was performed in 3 cases with a fibula graft. In the fourth patient, no operation was possible due to the diffuse nature of the invasion and the poor status of the patient's vessels and general health. Local extension, and not the tumour histology, is the main factor in deciding upon reoperation. The explosive nature of recurrences is linked more to the aggressive character of the tumour than to the status of the revascularized free bone graft. Secondary reconstruction is justified after recurrence on bone grafts depending on the complexity of the major resection involving extensive tissue exeresis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Multiple free transplants in microsurgery for cervicofacial reconstruction].

Microsurgical techniques open the way for much creativity and originality for cervicofacial reconstruction and tumoral repair. Between 1976 and 1994, we performed 481 free transplants for cervico-facial reconstruction. A recently developed strategy has been the use of multiple free transplants, either simultaneously or successively. Between 1990 and 1994, we operated and reconstructed 9 patients using multiple free cervico-cephalic transplants after multitissular and agressive exeresis for tumoral or traumatic lesions. In 3 cases, the transplants were made simultaneously and in the 6 others successively. Three patients received 3 free transplants and 6 patients had 2. In 8 cases, each of the transplants was micro-anastomosed on different vessels. In one case, the two transplants were anastomosed in series, the most distal portion being connected to the most proximal portion (in comparison with the cervical vessels). One death occurred at 6 weeks due to infectious complications. The final result for the 8 other patients was good both morphologically and functionally. Indications for multiple free transplants include: complex tissue loss, multiple and massive tissue loss of the cervicocephalic region. This situation occurs particularly in case of voluminous tumours invading several structures, in multiple tumours and in firearm wounds. These series of patients allowed a definition of recommended techniques in case of difficulties in these very elaborate surgical procedures and confirmed that if the strategy is correctly planned, it is possible to greatly improve the quality of survival in these patients in terms of aesthetics, morphology and function.
