Bell's palsy and stapedial reflex.

In Bell's palsy the stapedial reflex may be affected or not. The object of this paper was to study the evolution of the stapedial reflex and its latency pattern during the first weeks of paralysis and to correlate these parameters with the evolution of the disease. One hundred and thirty-three patients with Bell's palsy were evaluated, and the patients could be divided into 3 groups. Group 1 was composed of subjects in whom the stapedial reflex was present in the first week of paralysis and complete recovery of facial function occurred in 30-60 days. In group 2 the stapedial reflex reappeared between the eighth and thirty-fifth days post-onset and facial function recovered within 150 days. In group 3, the stapedial reflex reappeared between the thirty-sixth and seventieth day and some paralysis was still present after 1 year. These results suggest that the reflex is important for prognostic purposes.

Electrogustometry in hemodialysis patients.

Serous zinc levels and electrogustometry were studied on thirty patients undergoing periodic hemodialysis. After dialysis a better electrogustometric threshold (p less than 0,05) and also higher serous zinc levels were found. The two parameters showed no significant correlation. These results are suggestive of a toxical damage of taste receptors in the chronic renal failure.

Facial nerve function after cerebellopontine angle surgery and prognostic value of intraoperative facial nerve monitoring: a critical evaluation.

PURPOSE: Facial nerve monitoring is often used to predict postoperative facial function after acoustic neuroma tumor removal. In this study, three methods of predicting facial nerve function were compared. These methods used various parameters of the evoked electromyographic monitoring. MATERIALS AND METHODS: Thirty-four patients who underwent surgery for acoustic neuroma were retrospectively reviewed. Amplitude of ongoing electromyographic activity, stimulation current thresholds, and the amplitude of evoked response were analyzed. The predictive value of the three methods was compared with actual postoperative facial nerve function. RESULTS: One method predicted the final postoperative facial function in 90% of the patients, one method in 84%, and the final method failed to predict the final VIIth nerve function in patients with current stimulation thresholds greater than 0.05 mA. CONCLUSION: Analysis of prognostic value showed that one of the three studied proved superior in predicting facial nerve function.

[Experimental perilymphatic fistula: the electrocochleographic findings].

The aim of the present study was to evaluate the effect of experimental perilymphatic fistula on cochlear electrophysiological responses. Six albino guinea pigs, in which recording electrodes were implanted in order to record the electrocochleogram, were used. Experimental protocol investigated cochlear responses before the induction of the fistula in the round window membrane perforation and then two month later. Compound action potential thresholds increased immediately after perforation of the round window membrane. The thresholds rapidly recovered. This phenomenon was accompanied by clear-Out reduction in amplitude upon suprathreshold stimulation. This latter observation associated with a decrease in the amplitude of the threshold seems to be a specific audiologic sign of the fistula. The clinical implication are discussed.

[Longitudinal comparison of methods of auditory evoked potentials in defined multiple sclerosis].

The object of the present paper was to evaluate the diagnostic accuracy of ABR using ipsilateral masking in patients suffering from multiple sclerosis. Fourteen patients were examined. Each patient with normal audiogram underwent two ABR recording sessions during a follow-up period of thirty months. The ABRs were recorded under two experimental conditions applying standard testing procedures and ipsilateral masking, respectively. In 5 patients magnetic resonance imaging was also available. The findings were compared with a control group, composed of 20 normal subjects. Standard ABR testing gave evidence of a pathological value in 50% of the cases while the ABRs proved to be altered in 64% under ipsilateral masking. The outcomes were less favourable than those obtained with somatosensory or visually evoked potentials. The conclusions drawn confirm the importance of ABR in the follow-up of multiple sclerosis. On the other hand, ipsilateral masking does not appear to provide any significant advantages due to poor amelioration of sensitivity and doubtful specificity of the observed alterations.

[Diseases associated with Ménière's disease].

The object of the present paper was to investigate the presence of other diseases in patients suffering from Menière's disease and to identify any common etiopathogenetic factors. In a group of 100 patients there was a high incidence of three pathological associations: 1) anxiety and depression; 2) dyspepsia; and 3) vasomotor headache. The percentages were then compared with the data found in the literature and with the non menieric population. The association of dyspepsia proved statistically significant. The relationship between anxiety and vasomotor headache remains suspect while awaiting more precise definition of these diseases and findings more comparable with the total population.
