The effect of prostaglandin E2 on the cardiovascular response to angiotensin II in pregnant rabbits.

The rise in arterial blood pressure in response to angiotensin II was studied in the last third of pregnancy in rabbits. The response was compared with that of pregnant rabbits during infusion of prostaglandin E2 and F2alpha. Prostaglandin E2 significantly diminished the rise in diastolic pressure in response to angiotensin II. Prostaglandin F2alpha did not alter the response. Intravenous indomethacin elevated the blood pressure and caused an absolute increase in the pressor response. It did not mediate a change in the percentage rise in blood pressure in response to angiotensin II.

Outpatient termination of pregnancy.

Termination of pregnancy was performed under local anaesthesia as an outpatient procedure on 251 patients. The gestational ages ranged from 5 to 12 weeks. Either the portable Karman curette equipment or the syringe kits were used. The incidence of complications was low. Only 6 per cent of patients would have preferred general anaesthesia. The optimal gestational age at which to terminate pregnancies appeared to be between 6 and 10 weeks. Ninety-one per cent of patients used effective contraception after the termination.

Evacuation of retained products of conception in a treatment room and without general anaesthesia.

One hundred patients with an incomplete aboriton, 14 patients with retained products of conception after a second trimester termination, 5 patients with missed abortion and 2 patients with secondary postpartum haemorrhages (making a total of 121 patients) had a uterine evacuation in a treatment room using the portable Karman curette equipment. No general anaesthesia was used but patients were given intravenous pethidine or papavaretum (Omnopon) and diazepam. Four patients found the procedure painful but only two of them would have preferred general anaesthesia. There were no immediate complications but three patients required re-evacuation of the uterus.

Antenatal diagnosis of trisomy 13 with unexpected increase in alpha-feto protein.

A 24-year-old woman who had previously given birth to an infant with Down's syndrome was shown by chromosomal analysis of the liquor amnii to be carrying an infant with trisomy D. Routine examination of serum and liquor alpha-feto protein (AFP) in the antenatal period showed unexpected high levels of both, consistent with a neural tube defect. The fetus, however, did not have evidence of a neural tube defect but had scalp defects which were presumed to have allowed the leakage of AFP from the fetus into the liquor amnii and hance into the maternal serum.

Use of prostaglandin E2 for therapeutic abortion.

Termination of pregnancy was attempted with intravenous infusion of prostaglandin E(2) (5 mug./minute) in 52 consecutive cases. Abortion was successful in 50 of these and complete in 43. There were no serious side-effects.
