Acute interstitial nephritis associated with gentamicin and lincomycin therapy.

A case of acute oliguric renal failure following gentamicin and linocomycin therapy is described. Renal biopsy showed an acute interstitial nephritis. This was associated with high serum gentamicin levels and the later development of ototoxicity. Withdrawal of antibiotics and conservative measures was followed by rapid recovery of renal function. Attention is drawn to the association between gentamicin and lincomycin therapy and the development of an acute interstitial nephritis.

[Tuberculosis of the penis (corpora cavernosa)].

Tuberculosis of the male urethra and corpora cavernosa is discussed and 3 patients with this rare condition are presented. These patients were among 63 patients who presented with urethral stricture during the past 2 years. The diagnosis was based primarily on histological appearance, and treatment consisted of a combined medical and surgical approach, viz. streptomycin, PAS, INH with or without rifampicin; urinary diversion and drainage of pus (if any); initial stage I urethroplasty after control of infection and stage II urethroplasty after 18 - 20 months.

Bladder tumours in paraplegic patients on renal replacement therapy.

Fourteen adult patients with end-stage renal disease due to spina bifida or spinal-cord injury underwent regular haemodialysis for 3-120 (mean 20) months. Seven of these patients received renal transplants. Urinary bladder tumours developed in four of the fourteen patients (three squamous-cell carcinoma in patients stil on dialysis, one poorly differentiated transitional-cell carcinoma in a patient after successful transplantation). All four had widespread metastatic disease and died within 8 weeks of the diagnosis of cancer. We recommend regular urological evaluation for patients with neurogenic bladder even after initiation of dialysis treatment. Total cystectomy before transplantation might greatly reduce the risk of these rare tumours.

A candidate for the cystic fibrosis locus isolated by selection for methylation-free islands.

A genomic sequence close to the cystic fibrosis locus with the characteristics of an HTF island has been selectively cloned and characterized. Two markers flanking this sequence, which is conserved throughout mammalian evolution, show a very much greater disequilibrium than that found with any existing marker. A single mutational event accounts for most cases of cystic fibrosis. The sequence is expressed, and is a candidate for the cystic fibrosis gene.

Biomarkers in occupational volatile organic chemical exposure.

Evidence exists that in certain groups of workers exposed to volatile organic chemicals, there is subclinical renal damage and dysfunction. Also, there is activation of biological mechanisms that are suggested links between volatile organic chemical exposure and renal disease. Notably, the workers studied are employed in factories where exposures are considered controlled, with on-site professional health and safety management. Recent studies continue to indicate an increased risk of renal disease in those exposed to volatile organic chemicals.
