Contribution of p53 gene alterations to development of metastatic forms of follicular thyroid carcinoma.

Alterations of p53 suppressor gene as a possible indicator of the metastatic potential of thyroid carcinomas were evaluated in a cohort of 45 thyroid carcinomas. Well-differentiated papillary and follicular carcinomas were evaluated; the poorly differentiated and the undifferentiated forms were excluded from the studies. Tumors were divided into two groups: those giving no metastasis for > 10 years and those developing metastasis within 5 years. Gene alterations were tested by immunocytochemical detection of p53 gene expression and by determining loss of heterozygosity (LOH). Considering the two methods together, p53 damages were observed in two out of 11 papillary carcinomas without metastasis (18.1%), one out of nine papillary carcinomas with metastasis (11.1%), two out of 14 follicular carcinomas without metastasis (14.2%), and five out of 11 follicular carcinomas with metastasis (45.4%). Statistical X2 test showed significantly (p = 0.05) only between follicular carcinomas with and without metastasis thus p53 damage may have an impact for metastatic potential of follicular thyroid carcinomas.

[Value of thyroglobulin determination in the follow up treatment of patients with thyroid cancer].

Tg measurements were performed regularly in 96 thyroid carcinomas in order to evaluate metastases and local recurrencies. Elevated Tg level was detected in 11 patient. 7 of them were operated on because of local recurrencies and lymph node metastases. All patients that needed reoperation had elevated Tg level. During the follow up period no patient needed reoperation who had elevated Tg level or metastases detected by other diagnostic procedures. Summarising the results they found the Tg level measurements valuable in the follow up of patients with differentiated thyroid cancer. The Tg level measurements can supplement or sometimes replace 131J thyroid scanning.

Rapid cytological diagnosis during thyroid surgery.

The authors report the results of the use of intra-operative cytological examinations of the thyroid gland. In their hands, the method has a 96.3% accuracy in 757 patients with thyroid disorder. Benign conditions and malignant tumours can be correctly diagnosed. False negative diagnosis can arise with well-differentiated follicular carcinoma and the differentiation of follicular adenoma may be difficult. Atypical adenomas may give a false positive diagnosis. The incorrect diagnosis of chronic lymphocytic thyroiditis is unlikely when sufficient attention is given to the whole clinical picture.

[Management of lung and bone metastases from thyroid cancers of differentiated structures].

During a period of thirty years (1959-1989) 374 patients underwent surgery for differentiated thyroid carcinoma at the 1st Department of Surgery of the University Medical School in Debrecen. Distant metastases were found in the lungs or bones in 36 patients (9.6%). In 20 patients the first clinical sign of the primary disease was the distant metastasis. The aspects of surgical treatment depended on whether the metastases were solitary or multiple, capable or uncapable of radioiodine uptake. Whereas the majority of pulmonary metastases respond well to 131I-treatment the therapy of bone metastases is multidisciplinary. The 5-year survival rate was 47%, the 10-year one 19%.

Late prognosis of childhood and juvenile thyroid carcinomas.

The authors report on the biological properties and late prognosis of 16 children and juvenile patients operated on during the past 24 years for thyroid tumour who underwent regular followup. The clinical and morphological characteristics of the carcinomas are described, together with the late immune response of the patients undergoing complex treatment. The study is also concerned with the characteristics of the DNA content of the tumorous cell nuclei, the pregnancy success rate of the operated patients and the thyroid function of the children born.

[Prognostic significance of nuclear DNA content in highly malignant thyroid tumors].

The nuclear DNA content of tumour cells is correlated with the clinical outcome of a wide range of malignant disorders, among them thyroid neoplasia. The authors performed DNA determinations by means of the Feulgen cytofluorimetric method and demonstrate the connection between ploidy and biological aggressivity in cases of highly malignant thyroid tumours. The nuclear DNA content was measured in fresh surgical specimens from 53 patients suffering from malignancy, 13 of which proved pathohistologically to be highly malignant: 8 were anaplastic giant cell carcinomas, 3 squamous cell carcinomas and 2 were malignant lymphomas. 4 of the 8 giant cell carcinomas contained co-existent well-differentiated areas, namely follicular carcinomas in 3 patients and a papillary carcinoma in the 4th patient. 11 of the patients died of cancer, 9 within the 1st year. A different DNA distribution was found in the two survivors over a 10-year follow-up period. One of them showed a diploid cell population and the other facultative polyploidy, as opposed to the aneuploid pattern found in the 11 deceased patients. Both patterns confirm the correlation with a favourable outcome.

Correlation of malignancy with the intracellular Na+:K+ ratio in human thyroid tumors.

Energy-dispersive X-ray microanalysis was applied on human intraoperative biopsy materials of different thyroid tumors. To ensure suitability of these tissue pieces for quantitative microanalysis in freeze-fractured, freeze-dried bulk specimens, sampling was carried out with strictly defined criteria. Benign adenomas and differentiated and anaplastic carcinomas were selected for the studies on the basis of pathohistological investigations of the same specimen. The results of the tumor cells were compared to those obtained in apparently normal human epithelial cells. The number of normal cells analyzed was 349, whereas in the tumors 408, 423, and 891 cells were measured in the benign, differentiated, and anaplastic groups, respectively. Intracellular monovalent contents were calculated as percentage of cell dry mass; then, Na+:K+ molar ratios were calculated for each cell individually. Due mostly to the increase of Na+ content, the distribution histograms of the Na+:K+ molar ratio show an increase in the number of cells with a higher Na+:K+ ratio with increasing malignancy of the tumors studied. The differences proved to be statistically highly significant by the chi 2 test. Thus, in human thyroid, increasing malignancy is associated with increasing intracellular Na+:K+ ratio. The results give further support to the theory of C. D. Cone (J. Theor. Biol., 30: 151-181, 1971) according to which the sustained depolarization of the cell membrane results in an increased rate of cell division.

[Preclinical and manifest hypothyroidism following thyroid gland operation].

62 patients who were strumectomized on account of carcinoma of the thyroid gland, hyperthyroidism and thyroiditis underwent an after-examination on an average 6 years after operation. In preclinical hypothyroidism which cannot be differed from euthyroidism on the basis of the thyroid hormone concentrations, or only by means of the TRH-tolerance test, an increased TSH-response after TRH-tolerance and a T3-preference in comparison to T4 were observed. In manifest hypothyroidism the compensatory mechanisms exhausted and despite the increased TSH-concentration in the serum the ability for TRH-stimulation and the concentrations of the thyroid hormone are reduced. In preclinical and manifest hypothyroidism are recommended ca. 160-170 micrograms thyroxin per day for the substitution therapy after operation of benign diseases of the thyroid gland and after operations of the thyroid tumour the suppression therapy with ca. 220 micrograms thyroxin per day.

Modification of the cytoprotective effect of prostacyclin on gastric epithelial cells of rats by colchicine.

The effect of prostacyclin (PGI2) and colchicine on fundic gastric mucosa of sexually mature female Wistar rats was investigated histochemically after restraint stress. Under PGI2 treatment the ulcer index decreased significantly; this effect was moderately inhibited by colchicine. The mucus content of epithelial cells was decreased during restraint and was increased by PGI2 treatment. This protective action was also inhibited by colchicine. The nuclear volume of fundic epithelial cells increased significantly in restraint and decreased following colchicine application. These phenomena were not influenced by PGI2. According to our results the cytoprotective effect of PGI2 is not accompanied by nuclear volume changes.

Simultaneous occurrence of Graves' disease with endocrine ophthalmopathy. Thyroiditis and thyroid carcinoma.

The simultaneous occurrence of Graves' disease with ophthalmopathy lymphocytic and subacute de Quervains' thyroiditis has been observed in two male patients suffering from follicular-type thyroid carcinoma. In one of them Sjögren syndrome and a mild form of diabetes were also diagnosed. It is assumed, on the basis of indirect evidence, that the humoral and cellular lesions determined by genetic factors and influenced by prolonged thyrostatic and steroid treatment are jointly responsible for tumour genesis.
