[Lumbar phlebography. Normal aspect and its value in diagnosis of herniated disk].

Our technique of lumbar phlebography involves selective catheterisation of the lateral sacral veins with or without catheterisation of the ascending lumbar veins. Injection of these veins, in association with abdominal compression, produces good filling of the intra-spinal epidural veins, particularly at L5-S1 level. The anatomy and radiological appearances of the epidural veins are discussed. The phlebographic signs of disc herniation are presented. The diagnostic potential of this technique seems very promising in disc pathology, especially in the diagnosis of L5-S1 herniation and lateral prolapse at all levels.

[Superselective arteriography of the hip. Technic, normal appearance and 1st results in primary osteonecrosis of the femoral head].

The arteries vascularising the femoral head were selectively catheterised via the axillary, or left humeral route. Injection into the posterio circumflex artery revealed an interruption of the superior capsular branches in necroses of the femoral head at their onset. In the more develped lesions either a persistence of the obstruction of the capsular branches or a revascularisation was found.

[Gamma heavy chain disease. A new case].

Description of a case presenting as an inflammatory polyarthritis. A uterine sarcoma was detected one year later and led to the death of the patient. The abnormal protein belonged to the gammal subclass. Biosynthesis experiments, performed on the marrow plasma cells, showed that light chains were not produced and that the molecular weight of the monomeric unit of the serum protein was slightly smaller than that of the protein secreted in vitro, suggesting the occurrence of a secondary and limited extracellular proteolysis.

[Chondroitin sulfate in the treatment of gonarthrosis and coxarthrosis. 5-months result of a multicenter double-blind controlled prospective study using placebo].

One hundred twenty patients with osteoarthritis of the knees and hips were entered into a randomized, placebo-controlled, double-blind trial designed to evaluate the effectiveness of chondroitin sulfate (Structum). The three-month treatment phase was followed by a two-month treatment-free phase to allow evaluation of carry-over effects. The main endpoint was use of nonsteroidal antiinflammatory drugs (expressed as mg of diclofenac equivalent). At completion of the three-month treatment phase, patients taking chondroitin sulfate (4 capsules/day) were using significantly less NSAIDs; this decrease persisted throughout the two-month treatment-free follow-up phase. The other parameters studied including visual analog scale assessment of pain, the Lequesne pain-function index, and overall patient and physician assessments, all showed a similar significant tendency. Tolerance was outstanding and no patients required premature withdrawal. These findings indicate that chondroitin sulfate is useful for the treatment of osteoarthritis, both as an agent slowly effective against symptoms and to reduce the need for NSAIDs. The carry-over effect of the drug suggests that intermittent administration may be appropriate.

[Chronic atrophying polychrondritis].

After the description of the clinical signs (chondritis, arthritis, eye signs, cardiovascular signs) and laboratory signs of chronic atrophying polychrondritis, the authors study its pathology and the electron microscope findings, its diagnosis and the most recently proposed criteria, its course and therapeutic difficulties, its classification and finally discuss recent biochemical and immunologic data and the pathogenesis.

[Percutaneous automated lumbar nucleotomy].

The results of percutaneous automated lumbar nucleotomy in the treatment of lumbar disc herniation were assessed in a series of 39 patients. The technique consists of mechanical decompression of the herniated intervertebral disc without total excision. Only one-space discs were treated by this method. Sciatica was the predominant clinical symptom in 30 cases, and lumbar pain in 9 cases. Good to very good results were obtained in 70% of patients with sciatica and in 55% of patients with lumbar pain. After 4 cases of nucleotomy performed after failure of nucleolysis were excluded, the proportion of very good results rose to 77% in sciatica. Conversely, it seems that a number of failed nucleotomies can be treated by nucleolysis. Nucleotomy is very well tolerated and deserves to be used as first-line treatment of single and radiologically well documented lumbar disc herniations.

[The carpal tunnel syndrome. Diagnosis and treatment].

The carpal tunnel syndrome is the most frequent cause of acroparesthesias of the upper limb. It affects women in 80% of the cases. Classically its diagnosis rests on clinical data; sensory manifestations, more marked at night, are seen earlier than motor manifestations which appear later. In the relatively frequent cases where the topography of the pain is atypical, electromyography represents a determining diagnostic factor. The treatment, initially, is based on steroids infiltrations within the tunnel, representing a true diagnostic test. In case of failure of the medical treatment, especially when there are signs of motor denervation, the surgical decompression of the median nerve is mandatory, with division of the anterior carpal ligament and possibly completed by a careful neurolysis.
