[Calcium and endocrine pancreas secretion. II. Effects of an infusion of calcium on insulin and glucagon secretion induced by intravenous administration of glucose].

IRI, IRG and blood glucose levels after I.V.G.T.T. in normal subjects were not significantly modified when I.V.G.T.T. was repeated during a prolonged calcium infusion (15 mg/Kg body weight per 90'). The Authors advance the hypothesis that no closed relationships exist between serum calcium levels and insulin secretion, in these experimental conditions; with regard to glucagon secretion, further experiences are of course required namely during alpha-cell stimulation.

Increased erythrocytic spermine in Duchenne muscular dystrophy.

Erythrocytes of patients with Duchenne muscular dystrophy have been found to have elevated levels of spermine, a polyamine with a membrane-stabilizing action. Although a specific, increased spermine concentration is an additional biochemical abnormality documenting the involvement of erythrocyte membrane in Duchenne muscular dystrophy. Normal level of spermine were found in female carriers.

[Oestrogen receptors determination in human breast cancer (author's transl)].

The AA. refer about the assessment of a charcoal-dextran method for the determination of oestrogens receptors in human breast cancer. Corrections for low affinity binding components are performed measuring bound 3H-17 beta estradiol in the absence and presence of non-radioactive 17 beta estradiol or diethylstilbestrol; the method has been previously studied on samples from pregnant rabbit uterus and then applied on human mammary tumors. Results achieved with the routine-utilisation of the present method are reported.

Cryoprecipitate and Factor VIII commercial concentrates: in vitro characteristics and in vivo compartmental analysis.

In vitro and in vivo characteristics of cryoprecipitates and three commercial factor VIII concentrates (Kryobulin, Hemofil and Koate) were comparatively studied. Factor VIII:C/VIIIR:Ag ratio was very low in all commercial concentrates without differences among them. Conversely, the decrease of factor VIIIR:WFRCof was proportional to the degree of purity. Factor VIII:C/VIIIR:WFRCof ratio was shown to be a reliable index of factor VIII complex denaturation. Crossed electroimmunoassay showed a faster migration of factor VIIIR:Ag only in commercial concentrates. Using a two-compartmental open model that accounts for endogenous synthesis of factor VIII:C, single-dose kinetics of factor VIII:C were studied in 23 patients with classic hemophilia. Good agreement between measured and fitted values of factor VIII:C plasma concentration was observed. beta half-life was shorter in high purity concentrates and longer in intermediate purity concentrates and cryoprecipitates.

The safety and efficacy of immunotherapy with aluminum hydroxide-adsorbed venom extract of Vespula spp. An open, retrospective study.

BACKGROUND: specific immunotherapy for Hymenoptera Venom (VIT) is considered a life-saving treatment in insect sting allergy. A few studies with depot VIT have been published, but they are mainly focussed on patients sensitized to Apis. METHODS: this retrospective study was designed to evaluate both efficacy and safety of depot VIT for Vespula. Thirty-six patients (age range 6-73 years) with a history of systemic reactions (grade III to IV according to Mueller) after a Vespula sting, and specific sera IgE RAST to Vespula at least class 2, were administered a depot preparation of venom reaching 50 microgram as monthly maintenance dose. After the first year the maintenance dose was administered every other month. Thirty-three patients were treated for a minimum of 5 years. Reactions to any new field sting of the relevant insect were recorded during the treatment and for 6 to 24 months after its interruption. RESULTS: the treatment showed an excellent tolerance, with only a few local side effects. Thirteen patients (11 Grade IV according to Mueller before VIT) under treatment showed only local reactions after each field sting (18 field stings in total) by the relevant insect. Four patients (3 Grade IV according to Mueller before VIT) had a total of 6 field stings after the interruption of the 5-year treatment, with only local reactions. CONCLUSIONS: according to our results, and in agreement with previous published studies, VIT for Vespula spp. with depot extracts has an excellent tolerance and is clinically effective.

A good response rate to recombinant erythropoietin alone may be expected in selected myelodysplastic patients. A preliminary clinical study.

The use of recombinant erythropoietin for treatment of anemia in myelodysplastic patients has so far produced poorer results than expected. Most clinical studies have been conducted without any selection of patients. In the present study we report our experience with the use of rhEPO in a population of selected MDS subjects. Only patients affected by refractory anemia according to FAB criteria, without significant WBC and platelets reduction, with normal LDH and short history of disease were eligible for the study and were treated with rhEPO at a dosage of 150 mg/kg three times a week for 2 months. Among the 12 so treated patients, 7 (58.3%) achieved complete remission, 2 partial remission and 3 failed to respond. This high response rate makes more than acceptable the cost/benefit ratio for rhEPO in RA patients and may identify a subgroup of patients that can be treated successfully with rhEPO alone.

Treatment of large cell lymphoma with the Fi2 regimen (doxorubicin, vincristine, cyclophosphamide, bleomycin and prednisone): 25-year experience.

The CHOP protocol is the reference treatment for large cell lymphomas, but several other schemes of different intensity have recently been studied with controversial clinical findings. We report here the results obtained at our institution with a CHOP-like regimen called Firenze 2 (Fi2), which is characterised by an original scheduling of chemotherapy administration. A total of 225 patients, who were diagnosed from 1974 to 1996, were included in this retrospective study. All patients received the Fi2 regimen as a first-line intervention. One-hundred and sixty-two (72%) achieved complete remission; the overall survival at 120 months was 51% with a disease-free survival of 67% (median follow-up = 78 months). The survival curve showed a stable plateau of 42% after 16 years, which remained stable for further 4 years. In a multivariate survival analysis, achievement of complete remission (p<0.001) and IPI index of 0 or 1 (p=0.05) were significantly associated with a better survival. Overall, the outcome of our patients was similar to that reported by others, but the distinguishing feature of our study is the very long follow-up of the patients. Our study confirms that first generation regimens are effective and can cure a substantial proportion of patients. The long-term results of our study are helpful to retrospectively identify high-risk patients whose prognosis is poor and who can be candidates for more aggressive schemes of chemotherapy.
