For the collection of autologous cancellous bone transplants normally the anterior and posterior parts of the Os ilium are used. With proper techniques the conture of the pelvis and the muscle insertions can be conserved. All other collection places should only be used in unexpected situations and need of only small amounts of transplant. Serious complications are fractures and nervous lesions, which can be avoided by a careful procedure.
The treatment of acute and chronic osteitis should be standardized, using the following steps: débridement/sequestrectomy; stabilization; eradication of local infection; revascularization; filling-up defects--bone transplantation. The transplantation of bone is only successful if the defect area is prepared following steps 1-4 mentioned above. If possible, only autologous bone should be transplanted into an infected bone area. For the take of the transplanted bone, good vascularization of the infected area is of great importance. To obtain the best function possible, the Kinesitherapy should be carefully supervised by the surgeon.
A minimum of organised measures is mandatory in every clinic, where posttraumatic or postoperative bone infection may occur. Technical principles in the treatment of osteomyelitis are: debridement, stability, local anti-infection procedures such as irrigation drainage, PMMA-chains or Taurolin gel, filling of the osseous defect and adequate vascularity (muscle flap, muscle transplant with microvascular anastomosis).

Intraarticular fractures require a surgical therapy. The functional results are depending on the anatomical reconstruction of the joint surfaces and on early kinesitherapy. Remaining disabilities may be controlled in 2/3 of the cases to such an extent that no alterations are necessary in private or professional life. The overall result was very good in 23.8%, good in 36.0%, moderate in 22.3% and bad in 17.9%. These bad results are due to the rate of polytraumatized patients (18%) and the high rate of open comminuted fractures (27%).

Since the rate of mortality in severe sepsis could not be changed in the last years even not by sophisticated antibiotics, and since the mutants of bacteria resistant to antibiotics are permanently increasing, other possibilities must be taken into considerations to prevent or to treat infections. The improvement of the patient's own immune resistance by active or passive immunizations seems to be a cooperative or alternative way to overcome severe and chronic infections. In animal experiments the efficacy of gammaglobulin treatment of a severe infection was tested. The positive result i.e. improvement of the mortality from 75% to 45%, stimulated to a controlled clinical study, in which severe infected patients were additionally treated with gammaglobulin and compared with those without gammaglobulin treatment. Patients with gammaglobulin treatment showed a much better outcome of their infection than control patients. The rate of infections as well as time of hospital stay and other clinical and laboratory parameters were markedly improved in gammaglobulin-treated patients. Also the principle of active immunization was tested in animal experiments. Guinea pigs were orally vaccinated with heat inactivated pathogenic bacteria. In a following challenge infection vaccinated animals survived in a high percentage whereas control animals died. This success in vaccinating animals did lead to a controlled clinical study with patients suffering from chronic bone infection. Patients were treated orally with heat inactivated bacteria over a time of 8 weeks. Not only laboratory data indicating an improved immune response were changed by the treatment but also the clinical findings. These results indicate clearly that infections can be treated not only by attacks against bacteria but also by the improvement of the patient's own bacterial resistance.
Removal of septic foci in putrid infection of the bone is an important step in the treatment of post-traumatic osteitis. The methods employed so far, such as through-drainage and siphonage, as well as gentamycin-PMMA chains, have very obvious drawbacks although they have definitely achieved favourable results. Taurolin Gel , a preparation consisting of denatured collagen in combination with the chemotherapeutic Taurolin 4%, is theoretically capable of eliminating these drawbacks. Clinical application appeared justified on the basis of good results obtained with the drainage solution of Taurolin, as well as in the course of experiments. Results have been obtained mostly in a single application in 50 cases documented and followed up; these results were equivalent to those obtained by means of the other methods. In consideration of the advantages with regard to application and effect, the authors believe that testing of this substance on a larger scale should be recommended.

Chordomas are malignant tumors with lobulated structure which usually consists of vacuolated physaliferous cells in a mucoid stroma (Enzinger et al. 1969). These cells originate from remnants of the chorda dorsalis. Most chordomas are located either at the end of the vertebral column, i.e., in the sacrococcygeal region or at the base of the skull near the sphenooccipital synchondrosis. More than half of all tumors are located in the sacrococcygeal region. In contrast to most cases of sacrococcygeal chordomas which show a predominant presacral tumor infiltration we report on a patient suffering from a sacrococcygeal chordoma with a retrosacral tumor mass. Here, the presacral tumor infiltration became apparent only much latter. It was nevertheless of great prognostic relevance as tumor infiltration of the urinary bladder lead to lethal septicaemia.

Histological grading from I--III was carried out for 22 osteosarcomas. There was a positive correlation between higher histological grade of differentiation and higher rate of 2-years-survival. The death rate was 3,7 times higher for osteosarcomas of grade III than for grade I and II tumors. However, no significant differences as to prognosis and age of the patients, localization of tumor metastases, various stages and kind of therapy became evident. In spite of distinct regional differences, as subclassification according to 6 subtypes proposed by Scranton et al. (1975) was possible in all our cases. Statistically significant correlations between these subtypes of osteosarcoma and differences in survival time could not be found, neither for our own limited material, nor for the combined material, including the cases of Scranton et al.
The anti-infective preventive action of intraoperative disinfection with physiological solution containing antibiotics, appears to be firmly established, judging from numerous publications. Since germs are showing a world-wide increasing resistance, the search for antibiotics which continue to be effective must be kept up, but alternative suggestions are equally necessary. Some publications have already been presented indicating that success similar to that previously obtained with antibiotics is possible by using disinfectants in the prophylaxis and treatment of infections. Hence, a comparative study was conducted using the non-absorbable broad-spectrum antibiotic Nebacetin and the disinfectant Taurolin in a 1% solution with a group of patients subjected to a total of 7699 major and minor surgical operations. Prospectively, early infections during the stay of the patient in the hospital, als well as disturbed wound healing, were included. No significant difference was found in the number of infections and the rate of disturbed wound healing. Indirectly, it is possible to conclude with the help of results from literature that the disinfectant Taurolin is suitable as an addition to intra-operative rinsing in the prophylaxis of infections.
Guinea pigs were orally and subcutaneously immunized against Pseudomonas aeruginosa. Whereas 93% of untreated control animals died after a subsequent IP infection, vaccinated animals developed an efficient protection. The mortality rate in orally pretreated animals was only 20% - 30%. Some immunological parameters were also improved by vaccination. IgG and IgM concentrations in serum increased, as did the agglutination titer against bacteria. Oral vaccination with heat inactivated pathogenic bacteria seems to be a promising procedure to improve the resistance of patients to bacterial infections.
